Epikeratophakia for Keratoconus: A Case Report with 30 Years of Follow-Up.

BACKGROUND: Epikeratophakia is a refractive surgical procedure used to correct aphakic eyes, hyperopia, and keratoconus and is often performed in children. In this report, we present the long-term effects of epikeratophakia on the progression of keratoconus in a patient who underwent surgery. Case Presentation. The patient was a 17-year-old boy with keratoconus who had difficulty wearing hard contact lenses. As a solution, he underwent right eye epikeratophakia with a plano-powered lenticule. We followed up the patient for 30 years. Although the progression of keratoconus ceased in the operate

Epikeratophakia, a valid surgical alternative for the treatment of selected patients with aphakia.

Epikeratophakia is a safe and valid surgical alternative for the correction of aphakia if spectacles or contact lenses are not tolerated and if ocular lens implantation is contraindicated. This report reviews our experience with 36 epikeratophakia procedures using lyophilized lenticules, with a mean follow-up of 19 months.

Epikeratophakia: technique modifications and visual results compared to the national study.

Twenty-eight epikeratophakia procedures have been performed at the University of Minnesota since December 1985. This report describes several technique modifications and compares the refractive and visual outcome of these cases with the national results. There have been 17 aphakic cases (13 adult and four pediatric), seven myopic procedures, and four keratoconus cases. Of the adult aphakic cases, 80% were within 3 diopters of emmetropia at six months, with four cases showing an undercorrection. In the myopic group, five cases showed an overcorrection at six months and all patients were within

Secondary intraocular lens implantation vs epikeratophakia for the treatment of aphakia.

We analyzed retrospectively the results of 30 consecutive secondary intraocular lens implantations and 30 epikeratophakia procedures performed by one surgeon for adult aphakia. Specific indications for epikeratophakia were criteria that excluded patients as candidates for intraocular implants. Follow-up of all patients was at least six months. Preoperative and postoperative uncorrected and best corrected visual acuity and endothelial cell counts, as well as power predictability and complications rates, were compared. Preliminary results indicated that postoperative visual acuity, power predict

Epikeratophakia: the surgical correction of aphakia. I. Lathing of corneal tissue.

Epikeratophakia is a newly developed form of refractive corneal surgery for the correction of hyperopia. In this procedure, a piece of donor corneal tissue is lathed to a specific dioptric power. The epithelial cells are removed from the recipient cornea, and the donor tissue is sutured to the front of the eye. The resulting change in corneal curvature provides the visual correction, as, for instance, in the correction of aphakia. We describe here the computational analysis used to lathe human donor corneal tissue for use as epikeratophakia grafts. With these equations, the precision of the ob

A prospective study of the use of hyperopic epikeratophakia grafts for the correction of aphakia in adults.

The results of a prospective clinical study of hyperopic epikeratophakia grafts for the visual rehabilitation of aphakic patients are presented. Visual acuity data are given for 21 aphakic patients who have been followed for up to six months postoperatively. With spectacle overrefraction, on the average, patients were within three lines of their preoperative potential vision at three months postoperative and within two lines at six months postoperative. Three months postoperatively, two patients achieved visual acuities of 20/20, and 57% had 20/40 or better with spectacle overcorrection. Six m

[Corneal reinnervation after lamellar keratoplasty in comparison with epikeratophakia and photorefractive keratectomy].

Corneal sensitivity was examined with the Draeger aesthesiometer in 8 patients after lamellar keratoplasty, 7 patients after epikeratophakia and 82 patients after photorefractive keratectomy. We examined 9 different points in each cornea. The corneal sensitivity of patients after lamellar keratoplasty was normal 1 year after operation. A correlation with the depth of the lamellar dissection was demonstrated. Patients with lamellar keratoplasty with pterygium showed a sensitivity decrease in the nasal parts of the lenticule even after one year. Three years after epikeratophakia the corneal cent

[Corneal topography of patients with unsatisfactory visual acuity after epikeratophakia for aphakia].

Corneal topographic characteristics of 5 patients who obtained unsatisfactory vision after aphakic epikeratophakia, with at least nine months follow-up, were reported. All grafts were apparently decentered relative to visual axis. The contour of cornea showed highly irregular pattern of power distribution. These findings suggest that to mark visual axis before surgery, lathe corneal lenses carefully and do standard operation should be the basis of successful epikeratophakia.

Glare and contrast sensitivity in contact lens corrected aphakia, epikeratophakia and pseudophakia.

The effects of glare on contrast sensitivity and high contrast Snellen acuity were investigated in a group of unilaterally aphakic patients with normal fellow eyes. In spite of relatively good visual acuity and apparently satisfactory surgical results, there was a marked reduction in contrast sensitivity in epikeratophakia and contact lens corrected aphakic eyes, relative to fellow normal eyes, especially in the presence of glare (P less than 0.05). Although the results from the pseudophakic patients were more variable, there is some evidence to suggest that epikeratophakia provides inferior v

Long-term follow-up of refractive and keratometric results of pediatric epikeratophakia.

To examine the long-term stability of epikeratophakia and its effect on the development of young eyes, the change in corneal curvature and refractive error over time were reviewed in pediatric patients who underwent epikeratophakia for optical correction of aphakia. Over an average follow-up time of 28.0 months, corneal curvature flattened by 1.15 diopters, or 0.53 D/yr. The average change in refractive error was -2.81 D over 23.6 months, or 1.49 D/yr. Patients younger than 1 year of age at surgery became significantly more myopic over the period of follow-up (4.75 D) than patients older than

Histological study of epikeratophakia tissue lenses for myopia removed from two patients.

Epikeratophakia tissue lenses for the correction of myopia were removed from two patients as a result of postoperative complications such as stromal folds, complaints of glare, and decreased visual acuity. Light and electron microscopic analysis showed protrusions in Bowman's membrane and the anterior stroma corresponding to corneal folds that had been observed clinically. The epithelial cells in these areas contained many vacuolations and a thickened basement membrane. Hemidesmosomes were decreased in number. The tissue lenses had been cut 1.5 mm larger in diameter than the trephined recipien

Epikeratophakia for keratoconus. Long-term results using fresh, free-hand made lamellar grafts.

Epikeratophakia using fresh, free-hand made corneal grafts was done in 16 patients with keratoconus. The follow-up period averaged 27.8 months (range 13-45 months). A significant improvement of visual acuity was obtained (p = 0.002), and 14 of the 16 eyes (87.5%) achieved a corrected visual acuity greater than or equal to 6/12. The spherical equivalent and the cylindrical refractive error were reduced (p less than 0.05), and a significant flattening of the central corneal curvature was obtained (p less than 0.002). The mean postoperative central corneal astigmatism was 4.25 D. Postoperatively,
