Intraoperative continuous monitoring of facial motor evoked potentials in acoustic neuroma surgery.

The preservation of facial nerve function is one of the primary objectives in acoustic neuroma surgery. We detail our method of continuous intraoperative facial motor evoked potential (MEP) monitoring and present criteria for the preservation of facial nerve function to avoid postoperative facial nerve palsy. Our study population was comprised of 15 patients who did not (group 1), and 20 who did (group 2) undergo facial MEP monitoring during surgery to remove acoustic neuromas. In group 2, we continuously stimulated the facial motor cortex at 5- or 10-s intervals throughout surgery. Electromyo

Abnormalities of somatosensory evoked potentials in konzo--an upper motor neuron disorder.

OBJECTIVE: To determine whether the somatosensory pathways are involved or not in konzo. METHODS: In 1998, 21 konzo subjects (15 females and 6 males; mean age 21 years) underwent a SEP study with a two-channel-equipment (Medtronic Keypoint, Denmark) whereas in 2000, 15 subjects (7 females and 8 males; mean age 21 years) participated in a study with a 4-channel-equipment. RESULTS: Most subjects (19/21 in 1998 and 12/15 in 2000) showed normal median SEPs. The remainders had no median cortical responses. All 21 subjects in 1998 and 9 out of 15 in 2000 showed abnormalities of tibial SEPs mainly

Reconsiderations about the abnormalities of somatosensory evoked potentials in motor neuron disease.

The frequency of involvement of sensory pathways in motor neuron disease (MND) remains the matter of controversy. For this reason the purpose of the present work was to test how often sensory system involvement might be detected by somatosensory evoked potentials (SEP) studies and then to verify the presence of alteration of the sensory conduction and to detect the frequency of abnormalities of somatosensory peripheral, spinal, subcortical and cortical potentials in MND. SEP were tested after median nerve stimulation at the wrist, recorded from Erb's point, Ce2, Ce7 and scalp. Pearson's correl

Motor and somatosensory evoked potentials in tuberculous meningitis: a clinico-radiological correlation.

OBJECTIVE: The sensory and motor functions in severe tuberculous meningitis (TBM) may be difficult to assess clinically and may be helped by evoked potential studies. Lack of motor and somatosensory evoked potential studies in TBM prompted the present study. METHODS: All the patients with TBM underwent detailed neurological evaluation and cranial CT scan study. Motor and somatosensory evoked potentials to both upper and lower limbs were carried out bilaterally. The outcome was defined on the basis of 3 month Barthel Index (BI) score into good (BI > 12) and poor (BI < 12). RESULTS: Forty-one

Normal postexercise facilitation and depression of motor evoked potentials in postpolio patients.

We studied the effects of exercise on motor evoked potentials (MEPs) elicited by transcranial magnetic stimulation in healthy subjects and postpolio patients. Subjects performed repeated sets of isometric exercise until the muscle fatigued. In both groups, the mean MEP amplitude immediately after each exercise set was approximately twice that of the baseline amplitude, indicating similar postexercise facilitation, and after fatigue was approximately half that of the baseline amplitude, indicating similar postexercise depression. We conclude that the intracortical component of central fatigue i

Online detection of class-imbalanced error-related potentials evoked by motor imagery.

Objective.Error-related potentials (ErrPs) are spontaneous electroencephalogram signals related to the awareness of erroneous responses within brain domain. ErrPs-based correction mechanisms can be applied to motor imagery-brain-computer interface (MI-BCI) to prevent incorrect actions and ultimately improve the performance of the hybrid BCI. Many studies on ErrPs detection are mostly conducted under offline conditions with poor classification accuracy and the error rates of ErrPs are preset in advance, which is too ideal to apply in realistic applications. In order to solve these problems, a n

Motor Evoked Potentials Double Train Stimulation: Optimal Number of Pulses per Train.

PURPOSE: In neurophysiologic intraoperative monitoring, double train transcranial electrical stimulation (dt-TES) for motor evoked potentials (MEP) consists of an initial, facilitating train of stimuli followed by a second, testing, train. The optimal number of pulses per train has not been determined. The authors wanted to determine the optimal combination of pulses per train that produced the highest MEP amplitude. METHODS: Dt-TES of 4 + 4, 2 + 7, and 7 + 2 pulses were attempted in 20 patients and compared with single train TES of nine pulses. Latencies of the MEP responses were also compar

Motor evoked potentials from the pelvic floor in patients with multiple sclerosis.

The use of motor evoked potentials (MEPs) to study the integrity of pelvic floor motor innervation is poorly described in the literature. This study evaluated the clinical use of pelvic floor MEPs in 16 women with multiple sclerosis. Lower urinary tract dysfunction was assessed with urodynamic investigations. Transcutaneous magnetic stimulation was applied over the motor cortex and spinal roots, and MEPs were recorded from the puborectalis, the external urethral sphincter, and the abductor hallucis muscles. In many patients, responses from the pelvic floor muscles could not be evoked, and cent

Decreased postexercise facilitation of motor evoked potentials in patients with cerebellar degeneration.

We studied the effects of exercise on motor evoked potentials (MEPs) elicited by transcranial magnetic stimulation (TMS) in 18 normal (control) subjects and 11 medication-free patients with cerebellar degeneration. Subjects performed repeated sets of isometric exercise of the extensor carpi radialis muscle until the muscle fatigued (subject became unable to maintain half maximal force). MEPs were recorded before and after each exercise set and for up to 30 minutes after the last set. The mean amplitude of MEPs recorded from the resting muscle immediately after each exercise set was 218% of the

Somatosensory evoked potentials following posterior tibial nerve stimulation predict later motor outcome.

Posterior tibial nerve somatosensory evoked potentials (PTN-SEPs) were performed on 50 neonates at high risk of future neurodevelopmental impairment just before their discharge from the neonatal intensive-care unit. The close association of sensory pathways with motor tracts and the need for sensory input and integration for normal motor functioning would indicate that CNS lesions producing motor deficits may be detected by this method. Follow-up of these infants revealed a highly significant relationship between bilaterally abnormal PTN-SEPs and the presence of cerebral palsy at three years o

Amplitude and latency characteristics of spinal cord motor-evoked potentials in dogs.

The motor-evoked potential can be reliably recorded in anesthetized dogs by use of percutaneous placement of active recording electrodes near the dorsal lamina of the vertebral column. Two types of responses were observed in this study; short (less than 5.5 ms at T9-10)- and long (greater than 5.8 ms at T9-10)-latency waves. Short-latency waves are larger in amplitude and appear with higher stimulus intensities than do long-latency waves. Short-latency waves are conducted at greater than 80 m/s and may not reflect pyramidal tract activation. The safety of using higher intensity stimuli to gene

Somatosensory evoked potentials in lacunar syndromes of pure motor and ataxic hemiparesis.

Syndromes of hemiparetic lacunar infarction include pure motor hemiparesis and ataxic hemiparesis. Twelve such patients were evaluated with somatosensory evoked potentials. Potentials were delayed or absent in all 4 patients with ataxic hemiparesis and in 1 of 8 patients with pure motor hemiparesis. Unlike pure motor hemiparesis, ataxic hemiparesis appears to be associated with abnormal somatosensory evoked potentials. These abnormalities suggest that disturbance of afferent pathways are important in ataxic hemiparesis.

The efficacy of motor-evoked potentials on cerebral aneurysm surgery and new-onset postoperative motor deficits.

Surgical clipping may cause stenosis of parent arteries or occlusion of perforating arteries in cerebral aneurysm surgery. To prevent postoperative motor deficits, motor-evoked potentials (MEPs) have been used. This enables to detect cerebral ischemia. However, the rate of false negatives (motor deficits with preserved MEP) has been relatively higher than in aortic surgery. We hypothesized that postoperative motor deficits with preserved intraoperative MEP do not always represent false negatives. We reviewed medical records of patients for cerebral aneurysms surgery with transcranial MEP monit

Detection of motor pathway suppression by transcranial and transesophageal motor-evoked potentials during thoracic epidural anesthesia.

PURPOSE: Transcranial motor-evoked potentials (TcMEPs) are widely used to detect intraoperative spinal motor pathway impairment. We developed a novel transesophageal motor-evoked potentials (TeMEPs) technique that directly stimulates the spinal cord from the esophagus at the lower cervical to upper thoracic level. This study compared the ability of TeMEPs and TcMEPs to detect reversible motor pathway suppression. METHODS: Ten pigs (25.0 ± 0.6 kg) were anesthetized with propofol and remifentanil. Motor pathway suppression was induced using T9-T10 level thoracic epidural anesthesia. After basel

F-wave and motor-evoked potentials during motor imagery and observation in apraxia of Parkinson disease.

INTRODUCTION: The amplitudes of F-waves and motor-evoked potentials (MEPs) increase during imagination or active motor performance. The aim of this study was to investigate F-wave and MEP facilitation during assessment of apraxia. METHODS: Eight Parkinson disease (PD) patients with apraxia, 11 patients without apraxia, and 8 healthy volunteers were enrolled. F-waves and MEPs were recorded during 4 states (resting, imagination, observation, and active movement). RESULTS: The mean amplitude of the F-waves increased significantly during imagination and active movement as compared with at rest i
