McArdle disease and carbohydrate ingestion before exercise: Timing on exercise tolerance, clinical relevance, and application to real world settings.

PURPOSE: McArdle disease (MD) is a rare (1:100000) genetic myopathy whereby the enzyme glycogen phosphorylase is not expressed in skeletal muscle, resulting in the blockage of glycogenolysis and presenting as physical activity intolerance. To circumvent the blockage in glycogenolysis, current clinical practice guidelines advise individuals with MD (IWMD) to ingest 37 g of sucrose 5-10 minute (min) before undertaking exercise. METHODS: The current investigation undertook randomized repeated measure clinical exercise trials on five IWMD to compare exercise tolerance under different timing of

Type 2 diabetes mellitus' impact on heart failure patients' exercise tolerance: a focus on maximal fat oxidation during exercise.

OBJECTIVE: To explore the impact of type 2 diabetes mellitus (T2DM) on exercise tolerance and fat oxidation capacity in patients with heart failure (HF). METHODS: We retrospectively analyzed 108 Chinese patients with HF who were divided into a diabetic group (T2DM group, n = 47) and a non-diabetic group (non-T2DM group, n = 61). All subjects completed cardiopulmonary exercise testing (CPX). We determined their fat oxidation (FATox) by indirect calorimetry. RESULTS: In the HF patients, the peak oxygen uptake (VO2) value was 14.76 ± 3.27 ml/kg/min in the T2DM group and 17.76 ± 4.64 ml/kg/min i

Understanding exercise (in)tolerance in sickle cell disease: impacts of hemolysis and exercise training on skeletal muscle oxygen delivery.

Sickle cell disease (SCD) is characterized by central (cardiac) and peripheral vascular dysfunctions, significantly diminishing exercise capacity and quality of life. Although central cardiopulmonary abnormalities in SCD are known to reduce exercise capacity and quality of life; the impact of hemolysis and subsequent cell-free hemoglobin (Hb)-mediated peripheral vascular abnormalities on those outcomes are not fully understood. Despite the recognized benefits of exercise training for cardiovascular health and clinical management in chronic diseases like heart failure, there remains substantial

Metronome-Paced Incremental Hyperventilation May Predict Exercise Tolerance and Dyspnea as a Surrogate for Dynamic Lung Hyperinflation During Exercise.

BACKGROUND: Dynamic lung hyperinflation (DLH) has been evaluated based on decreased inspiratory capacity (IC) during exercise load. However, this is not routinely done in clinical practice. We have developed a convenient method of metronome-paced incremental hyperventilation (MPIH) and reported its usefulness. In the present study, we compared these two methods for evaluating DLH and examined whether our MPIH method can be used to predict DLH during exercise. METHODS: DLH was measured by MPIH and constant load exercise (CLE) in 35 patients with stable COPD. DLH was defined as the most decreas

Exercise tolerance in patients with chronic thromboembolic pulmonary hypertension after balloon pulmonary angioplasty.

BACKGROUND: Balloon pulmonary angioplasty (BPA) is an effective therapeutic alternative for patients with chronic thromboembolic pulmonary hypertension (CTEPH), which improved pulmonary arterial compliance (CPA) and pulmonary vascular resistance (PVR). OBJECTIVE: To investigate whether the CPA is a predictor of exercise tolerance after BPA. METHODS: The correlations between changes in each parameter and changes in six-minute walking distance (6MWD) were evaluated by Pearson's test. The determinants of functional capacity that was defined as 6MWD ≥440 m were assessed with a logistic regressio

miR-130a-3p promotes fiber type transition and improves exercise tolerance in mice.

BACKGROUND: Skeletal muscle plays various roles in physiological stress, such as movement, hormone secretion and oxidative metabolism. MicroRNA (miRNA) is involved in skeletal muscle development and fiber type transformation. Our previous studies have shown that miR-130a-3p is an important regulator of glycolipid metabolism and participates in the aerobic oxidation process. However, its potential impact on skeletal muscle development and muscle fiber type transformation by enhancement of aerobic metabolism of nutrients remains unexplored. METHODS: Mice with knockout and overexpression of miR-1

Resting lung volume phenotypes in COPD: implications for exertional dyspnoea and exercise tolerance.

BACKGROUND: Lung volumes and dyspnoea vary markedly at a given forced expiratory volume in 1 s in COPD. We aim to investigate whether hyperinflation (high total lung capacity (TLC)) adds value to simpler inspiratory capacity (IC) in predicting mechanical-ventilatory impairment and exertional dyspnoea in these patients. METHODS: 345 patients with mild to very severe COPD (190 men) underwent incremental cycling with measurements of dyspnoea (0-10 Borg) and operating lung volumes. Resting volumes by body plethysmography were compared with the 2021 z-score-based Global Lung Initiative standards.

Exercise tolerance, lung function abnormalities, anemia, and cardiothoracic ratio in sickle cell patients.

Many patients with sickle cell disease (SCD) have a reduced exercise capacity and abnormal lung function. Cardiopulmonary exercise testing (CPET) can identify causes of exercise limitation. Forty-four consecutive SCD patients (27 HbSS, 11 HbSC, and 6 HbS-beta thalassemia) with a median age (interquartile range) of 26 (21-41) years underwent pulmonary function tests, CPET, chest x-ray, and echocardiography to further characterize exercise limitation in SCD. Peak oxygen uptake (V'O2 -peak), expressing maximum exercise capacity, was decreased in 83% of the studied patients. V'O2 -peak correlated

Muscle metabolic determinants of exercise tolerance following exhaustion: relationship to the "critical power".

We tested the hypothesis that muscle high-energy phosphate compounds and metabolites related to the fatigue process would be recovered after exhaustion during recovery exercise performed below but not above critical power (CP) and that these changes would influence the capacity to continue exercise. Eight male subjects completed single-leg, knee-extension exercise to exhaustion (for ∼180 s) on three occasions, followed by a work-rate reduction to severe-intensity exercise, heavy-intensity exercise (<CP), or a 10-min passive recovery period, in random order. The muscle metabolic responses to ex

Exercise tolerance and quality of life in elderly patients with chronic atrial fibrillation.

BACKGROUND: Atrial fibrillation is the most common arrhythmia affecting the elderly. Although the risk of cardioembolic stroke is well defined, the effects of chronic atrial fibrillation on exercise tolerance and quality of life have been less well quantified. METHODS: We compared a group of 52 elderly patients with chronic atrial fibrillation to a group of 48 control patients in sinus rhythm. Each patient underwent an interview that incorporated the Short Form-36 Health Survey (SF-36) to quantify individual perceptions on quality of life. In addition each person underwent physiologic testing

Insulin resistance limits glucose utilization and exercise tolerance in myophosphorylase deficiency and NIDDM.

Myophosphorylase deficiency [McArdle's disease (MD)] produces a defect in muscle glycogenolysis in which muscular work is limited by delivery of external sources of substrate, primarily glucose and nonesterified fatty acids, to meet energy demands associated with exercise. In the present study, we evaluated an unusual patient with both MD and non-insulin-dependent diabetes mellitus. We hypothesized that insulin resistance would limit transport of extracellular glucose to skeletal muscle during exercise, resulting in impaired exercise performance that was reversible by insulin infusion. The eff

Assessment of improvements in exercise tolerance following pulmonary valve replacement using physical accelerometry.

BACKGROUND: Pulmonary valve replacement may be required for patients with pulmonary valve disease. Exertional symptoms related to cardiovascular disease are an indication for pulmonary valve replacement; however, quantification of symptom improvement postoperatively is challenging, and previous studies have yielded mixed data. Accelerometry has the potential to remotely monitor changes in exercise tolerance following pulmonary valve replacement. METHODS: Individuals (n = 18) age >13 years scheduled for catheter-based or surgical pulmonary valve replacement were prospectively enrolled. Partici

Combined Neurocognitive and Exercise Tolerance Testing Improves Objectivity of Buffalo Concussion Treadmill Test.

CONTEXT: The Buffalo Concussion Treadmill Test (BCTT) is a standard assessment of exercise tolerance utilized for exercise prescription following concussion and to inform decisions regarding return to play. One limitation of the BCTT is that interpretation of test results is dependent on individuals' self-report of symptom exacerbation with exertion. Symptoms following concussion are significantly underreported or unreported. Combining objective neurocognitive assessment with exercise tolerance testing may enable clinicians to objectively identify those requiring further assessment or rehabili

Inorganic nitrate supplementation and blood flow restricted exercise tolerance in post-menopausal women.

Exercise tolerance appears to benefit most from dietary nitrate (NO3-) supplementation when muscle oxygen (O2) availability is low. Using a double-blind, randomized cross-over design, we tested the hypothesis that acute NO3- supplementation would improve blood flow restricted exercise duration in post-menopausal women, a population with reduced endogenous nitric oxide bioavailability. Thirteen women (57-76 yr) performed rhythmic isometric handgrip contractions (10% MVC, 30 per min) during progressive forearm blood flow restriction (upper arm cuff gradually inflated 20 mmHg each min) on three s

Intravenous Iron Replacement Improves Exercise Tolerance in COPD: A Single-Blind Randomized Trial.

INTRODUCTION: Iron deficiency affects exercise capacity because of the critical role iron plays in the optimal functioning of skeletal muscle metabolism. We hypothesized that intravenous iron may improve exercise tolerance, quality of life (QoL), and daily physical activity (DPA) in patients with chronic obstructive pulmonary disease (COPD). METHODS: This was a placebo-controlled, single-blind, parallel-group, randomized clinical trial. Iron deficiency was defined as a ferritin level<100ng/mL or a ferritin level between 100 and 299ng/mL with a transferrin saturation<20%, with or without mild

[Dynamic hyperinflation -- the main mechanism of decreased exercise tolerance in patients with COPD].

Decreased exercise tolerance in patients with COPD is the result of involvement in variable proportion of three mechanisms: ventilatory limitation, muscle dysfunction and cardio-vascular involvement (inadequate intake of oxygen at tissue level). Ventilatory limitation is caused by the combination of increased demand and decreased ventilatory capacity Increased ventilatory demand is the result of exercise worsening of ventilation-perfusion imbalance, and decreased ventilatory capacity is the result of decreased elastic recoil and dynamic obstruction. The consequence is the expiratory flowlimita
