Coagulation factor XI vaccination: an alternative strategy to prevent thrombosis.

UNLABELLED: Essentials Coagulation Factor (F) XI is a safe target for the development of antithrombotics. We designed an antigen comprising the human FXI catalytic domain and diphtheria toxin T domain. Antigen immunization reduced plasma FXI activity by 54% and prevented thrombosis in mice. FXI vaccination can serve as an effective strategy for thrombosis prevention. SUMMARY: Background Coagulation factor XI serves as a signal amplifier in the intrinsic coagulation pathway. Blockade of FXI by mAbs or small-molecule inhibitors inhibits thrombosis without causing severe bleeding, which is an in

Factor XI and contact activation as targets for antithrombotic therapy.

The most commonly used anticoagulants produce therapeutic antithrombotic effects either by inhibiting thrombin or factor Xa (FXa) or by lowering the plasma levels of the precursors of these key enzymes, prothrombin and FX. These drugs do not distinguish between thrombin generation contributing to thrombosis from thrombin generation required for hemostasis. Thus, anticoagulants increase bleeding risk, and many patients who would benefit from therapy go untreated because of comorbidities that place them at unacceptable risk for hemorrhage. Studies in animals demonstrate that components of the pl

Review: Factor XI deficiency: review and management in pregnant women.

Factor XI deficiency is a rare disease found predominantly in Ashkenazi Jews. There is a poor correlation between factor XI level and bleeding in patients with factor XI deficiency. Individuals with severe factor XI deficiency are usually at risk of excessive bleeding after surgery and injury, particularly when trauma involves tissues rich in fibrinolytic activity. Women with partial or severe deficiency are at risk of excessive uterine bleeding during labor. The unpredictable nature of factor XI deficiency complicates management during pregnancy and delivery. This review gives an overview of

[Factor XI inhibition in atrial fibrillation: where do we stand?].

Factor XI (FXI) has emerged as a promising target in the prevention and treatment of venous and arterial thromboembolism in several clinical settings. Its pharmacological inhibition, through small molecules, monoclonal antibodies, antisense oligonucleotides and small-interfering RNA, has been developed with the aim of achieving effective anticoagulation while reducing bleeding risk compared to conventional anticoagulants. Phase II studies have explored the role of FXI inhibitors in surgical thromboprophylaxis, in non-valvular atrial fibrillation (AF), and as an adjunctive strategy in acute ath

Total Laparoscopic Hysterectomy in Factor XI Deficiency: A Case Report.

Abnormal uterine bleeding (AUB) is a frequent reason for gynaecological consultation and can occasionally reflect rare coagulation disorders. The case study presented herein concerns a 42-year-old para 3 woman (gravida 3, para 3), who exhibited symptoms including irregular heavy menses and the presence of a large uterine myoma. The patient was referred for further evaluation due to symptomatic anaemia and a prolonged activated partial thromboplastin time (aPTT). Haematological evaluation confirmed severe factor XI (FXI) deficiency, with activity levels of 0.3% and 1.24% on repeat testing. As m

Targeting factor XI as a compromise between thrombosis and bleeding.

Thromboembolic diseases have long been a leading cause of morbidity and mortality, necessitating advances in anticoagulant drugs. Heparins, vitamin K inhibitors, and direct oral anticoagulants (DOACs) are well-established drug classes that help prevent thromboembolic complications. While effective, they pose significant risks during long-term therapy, including bleeding, osteoporosis, heparin-induced thrombocytopenia, and the need for frequent monitoring and dose adjustments. Factor XI (FXI) inhibitors represent an innovative approach in anticoagulation therapy, aiming to balance thromboemboli

Factor XI Deficiency in a Patient with Cervical Spondylotic Myelopathy.

STUDY DESIGN: Case report. OBJECTIVE: To summarize the clinical manifestations and treatment of Factor XI deficiency in a patient with cervical spondylotic myelopathy. SUMMARY OF BACKGROUND DATA: Factor XI deficiency is a rare genetic bleeding disorder caused by reduced levels and insufficient activity of a coagulation factor XI. It is claimed to be associated with prominent bleeding in case of trauma and surgery irrelevant to the FXI level. This is the first ever case of a patient with factor XI deficiency with cervical spondylotic myelopathy. METHODS: A case was investigated retrospective

Factor XI inhibition fulfilling the optimal expectations for ideal anticoagulation.

Thromboembolic diseases are leading cause of mortality accounting for an estimated 1 in 4 deaths all over the world. Anticoagulation remains the mainstay of prevention and treatment of venous thromboembolic disorders. Conventional anticoagulants have been efficiently used over the last decades, but their clinical use encounters safety and convenience issues. To overcome these limitations, research have focused on development of new targets for anticoagulation leading to a relatively new class of drugs, non-vitamin K antagonist oral anticoagulants, specifically targeting activated factor X and

Coronary artery stenting in a patient with factor XI deficiency.

Factor XI deficiency is a rare inherited coagulopathy first described in Ashkenazi Jews. In Japanese populations, factor XI deficiency is thought to be very rare. This disorder causes unique problems during percutaneous coronary intervention (PCI). During PCI, prevention of thrombosis is important and heparin is usually used for anticoagulation. However, care must also be taken to avoid serious complications of bleeding. These two situations are contradictory and anticoagulation with heparin might increase severe bleeding in patients with factor XI deficiency. An 84-year-old Japanese woman was

BACKGROUND: Mechanisms underlying unresolved left ventricular thrombus (LVT) developed afteracute myocardial infarction (MI) are unclear. AIMS: To investigate whether elevated factor (F) XI is associated with persistent LVT followingacute MI. METHODS: In consecutive 148 patients (aged 63.9 [6.9] years) with LVT detected during hospitalization for acute MI, all of whom received oral anticoagulation with dual antiplatelet therapy, we assessedFXI levels 3 months post-MI, while off anticoagulation, along with plasma fibrin clot permeability(Ks), clot lysis time, and plasminogen activator inhibit

Tissue factor pathway inhibitor is a potential modifier of bleeding risk in factor XI deficiency.

BACKGROUND: Factor (F) XI deficiency is associated with increased bleeding risk in some individuals. Neither FXI levels nor clinical clotting assays predict the bleeding risk. Compared with controls, FXI-deficient bleeders have reduced clot formation, decreased fibrin network density, and increased susceptibility to fibrinolysis. Tissue factor pathway inhibitor (TFPI) was recently implicated as a modifying factor in individuals with bleeding of unknown cause. OBJECTIVES: To determine the potential of TFPI in modifying the bleeding risk in FXI-deficient individuals. METHODS: The effects of TF

Management of Factor XI deficiency during coronary artery bypass graft surgery.

This is a man in his late 70s with a history of myocardial infarction and congenital factor XI (FXI) deficiency, who underwent urgent coronary artery bypass surgery for coronary artery disease. He demonstrated less than 2% factor XI activity on serological assay. Preoperatively, the patient was managed with only a non-therapeutic heparin drip, forgoing any preoperative factor resuscitation. Intraoperatively, the patient received fresh frozen plasma administration for post-bypass coagulopathy based on real-time assessment of bleeding. This resuscitation strategy advocates for the use of factor

The Discovery of a Novel Anticoagulant Mechanism: Factor XI Activation Inhibitors.

Despite advances in anticoagulant drug discovery to establish a new standard of care, represented by Factor Xa (FXa) inhibitors, additional therapies are needed to address the concomitant bleeding risk posed by these agents. Factor XI (FXI) plays a key role in the intrinsic coagulation cascade contributing to thrombin generation and fibril formation, and there is an increasing weight of evidence supporting a comparable efficacy and superior safety profile of FXI-based oral anticoagulants. A novel allosteric mechanism of action (MoA) for inhibiting FXI activation to Factor XIa (FXIa) was discov

Survival advantage of coagulation factor XI-deficient mice during peritoneal sepsis.

Anticoagulation is a rational approach to the treatment of sepsis-associated consumptive coagulopathy, but its application is limited because of the risk of excessive bleeding. Factor XI (FXI) contributes substantially to pathological blood coagulation (thrombosis), whereas it contributes only modestly to normal hemostasis. We found that FXI-deficient mice have reduced coagulopathy and increased survival relative to FXI-expressing wild-type mice during cecal ligation and puncture-induced acute peritonitis/sepsis. This finding suggests that FXI contributes to coagulopathy and/or inflammation du

Four novel FXI gene mutations in three factor XI- deficient patients.

Hereditary factor XI deficiency is a mild bleeding disorder, which is highly prevalent among Ashkenazi Jews, but has been reported in all populations. In Ashkenazi Jews, two factor XI gene mutations Glu 117X (type II) and Phe283Leu (type III) are particularly common. In other ethnic groups, factor XI deficiency is a rare bleeding disorder and is related to a variety of mutations throughout the factor XI gene. Three cases of quantitative factor XI deficiency in relation with four novel missense mutations are reported: a compound heterozygosity for two novel mutations (Ala 181 Val and Ala 412 Th

Plasma thromboplastin antecedent (PTA, factor XI): a specific and sensitive radioimmunoassay.

A specific, sensitive, and reproducible radioimmunoassay for human plasma thromboplastin antecedent (PTA, factor XI) has been developed with purified PTA and monospecific rabbit antiserum. Precise measurements of PTA antigen were possible for concentrations as low as 0.3% of that in normal pooled plasma. Normal plasma contained approximately 6 microgram PTA/ml. A good correlation (correlation coefficient 0.68) existed between the PTA procoagulant assays and radioimmunoassays among 50 normal adults (25 males and 25 females). PTA antigen was markedly reduced in plasma of 13 patients with congeni

A Case Report of a Hemodialysis Patient With Coagulation Factor XI and Factor XII Deficiencies.

Coagulation Factor XI (FXI) and Factor XII (FXII) deficiencies are rare. FXI deficiency is associated with a bleeding disorder, while FXII deficiency is not, but both can cause chronic prolongation of activated partial thromboplastin time and impair thrombus formation, posing great challenges for hemodialysis anticoagulation. Traditionally, heparin or low-molecular-weight heparins (LMWHs) are not considered a safe anticoagulation option for patients with increased bleeding risk. In this context, FXI and FXII have received substantial attention as targets for new anticoagulants. We present the
