Neonatal cranial osteomyelitis: a complication of fetal monitoring.

Use of a scalp electrode facilitates and adds precision to electronic fetal monitoring. We report an instance of neonatal osteomyelitis due to infection of the electrode insertion site. Electrode-associated soft and bony tissue infections are most commonly mediated by cervicovaginal microflora and appear clinically and microbiologically distinct from non-electrode-associated cases. Risk assessment, antiseptic care, and observation of the electrode insertion site are recommended. This report emphasizes the potential morbidity of scalp electrode use and suggests means to prevent or mitigate this

Electronic fetal heart rate monitoring: why the dilemma?

As medical technology becomes more and more complex, both the benefits and risks increase. Perhaps we have misled ourselves and, unwittingly, our patients into expecting the perfect outcome by complication-free care. As we learn more about the pathophysiology of disease it becomes apparent that our expectations are unrealistic. In addition, as clinicians we have become more sophisticated about the utility of clinical testing. We have learned that the predictive value of a test is related to its sensitivity and specificity, and to the prevalence of the disease in the population being tested. We

[Complications of intra-uterine fetal monitoring (author's transl)].

The authors review the literature of the complications of intra-uterine fetal monitoring. Internal tocography, which is at the moment the ultimate method for evaluating uterine contractions, can give rise to infections. Placing the catheter into the uterus is not the only risk but that risk can be increased by the techniques used for the delivery, the social class of the patient, the time the membranes have been ruptured and the flora of the vagina before it was put in place. With a little in the way of precautions the complications of infection can be avoided. They, as far as the mother is co

Electronic fetal heart rate monitoring: early neonatal outcomes associated with normal rate, fetal stress, and fetal distress.

OBJECTIVE: The purpose of this study was to test the ability of a clearly defined classification system for electronic fetal heart rate monitoring to predict early neonatal outcome. STUDY DESIGN: All labors of women with singleton pregnancies > or = 32 weeks' gestation electronically monitored at 2 institutions were examined. Tracings in the final hour before delivery were defined as normal, fetal stress, or fetal distress. After delivery, Apgar scores, cord blood gas values, and admission to the neonatal intensive care unit were examined as measures of early neonatal outcome. RESULTS: Among

Review of optical methods for fetal monitoring in utero.

The current technology for monitoring fetal wellbeing during child birth is cardiotocography. However, CTG has high false positive rates that lead to unnecessary emergency Cesarean deliveries and false negatives that result in birth injuries. To curtail these issues, fetal pulse oximetery has been a topic of interest for many decades. Fetal pulse oximetry would yield the oxygen saturation of the fetus in utero and provide a more robust marker for clinicians to make decisions about performing emergency Cesarean deliveries. Here, we present a review of biomedical optical developments related to

Fetal home monitoring for the prenatal management of gastroschisis.

BACKGROUND: Fetal distress is a frequent complication of gastroschisis, and could be screened for by home monitoring, as many pregnant women expecting an affected child live far away from a specialized perinatal center. This study was undertaken to audit a policy of fetal home monitoring (FHM) to achieve early detection of fetal heart rate (FHR) abnormalities in gastroschisis. METHODS: Daily FHM was started at a median age of 30 weeks in 31 pregnant women referred following prenatal diagnosis of isolated gastroschisis. Monitoring was considered abnormal in cases with decelerations, tachycardi

Meningitis, ventriculitis, and hydrocephalus: a complication of fetal monitoring.

A case is presented of meningitis, ventriculitis, and hydrocephalus secondary to the use of fetal scalp monitoring. The most commonly reported fetal complication related to the application of the scalp electrode is the development of a scalp abscess; the incidence varies between 0.45 and 4.5%, but several other complications have also been reported and the overall incidence is not well established. Infants in whom a scalp electrode has been used should be carefully examined to detect potential serious complications.

Hydrothorax treated in utero and monitored by fetal echocardiography.

Primary fetal hydrothorax is a relatively rare disorder, occurring in 1 out of 15,000 pregnancies, but connected with high perinatal mortality. In the event of little or no progress of the effusion or stable clinical course, conservative management seems to be the most appropriate course of action. In nearly half of the cases, a placement of only one pleuroamniotic shunt enables a total regression of the primary fetal hydrothorax and, consequently, a favourable outcome. The remaining cases require the placement of yet another shunt. In the case of repeated shunt failure, the prognosis is very

Factors involved in the interpretation of fetal monitor tracings.

In order to study how physicians choose to use electronic fetal monitoring and interpret tracings, we administered a questionnaire to which 107 practicing obstetricians and 11 experts in electronic fetal monitoring responded. Sixty-one (57%) of the respondents monitored more than half of their deliveries (high users). In comparison to the less frequent users of electronic fetal monitoring (low users), they showed more positive attitudes toward electronic fetal monitoring and were nearly always more likely to perform cesarean sections on hypothetical patients described in the questionnaire. The

Experience with fetal monitoring in a university teaching hospital.

Fetal monitoring during labour may be expected to decrease perinatal losses and the number of infants born with brain damage. In a prospective study of intrapartum fetal monitoring in selected high-risk pregnancies in a Winnipeg hospital the monitoring rate was 26.5% and the cesarean section rate in the monitored group was 22.0%. The fetal outcome in the monitored group was better than in the unmonitored group. The establishment of a fetal intensive care unit is believed to be strongly desirable in improving fetal surveillance during labour. Fetal monitors should be stationed in the delivery r

Clinical utility of inpatient monitoring for reduced fetal movements in the absence of fetal pathology.

BACKGROUND: Evidence on the follow-up of women with reduced fetal movements (RFM) and physiological fetal parameters at initial assessment is limited. We evaluated whether inpatient monitoring provides additional clinical value in this population. MATERIAL AND METHODS: In this retrospective study, women with a first episode of RFM and physiological fetal assessment (by means of cardiotocography, single deepest vertical pocket, estimated fetal weight, and umbilical artery Doppler) between January 2017 and December 2025 were identified and their clinical course and inpatient monitoring was anal

Clinical Role of the Noninvasive Abdominal Fetal ECG in the Detection and Monitoring of Fetal Tachycardia.

BACKGROUND: Fetal tachycardias can cause adverse fetal outcomes including ventricular dysfunction, hydrops, and fetal demise. Postnatally, ECG is the gold standard, but, in fetal practice, echocardiography is used most frequently to diagnose and monitor fetal arrhythmias. Noninvasive extraction of the fetal ECG (fECG) may provide additional information about the electrophysiological mechanism and monitoring of intermittent arrhythmias. Signal processing advances could provide improved data quality impacting clinical translation. The aim of this study was to assess the fetus with known or suspe

Effect of dialysis on fetal heart rate: is inpatient admission for fetal monitoring necessary?

OBJECTIVE: Pregnant patients receiving hemodialysis (HD) have long hospital stays for the purpose of electronic fetal monitoring (EFM) during HD, which allows for monitoring of fetal well-being. However, more frequent dialysis allows for smaller fluid shifts, preventing maternal hypotension. Our aim was to determine differences in rates of EFM abnormalities during HD versus non-stress testing (NST) off dialysis for gravid women with renal failure. METHODS: Retrospective cohort study over a 13-year period (2000-2013) identified five patients with renal failure in pregnancy. EFM tracings were r

Effectiveness of Remote Fetal Monitoring on Maternal-Fetal Outcomes: Systematic Review and Meta-Analysis.

BACKGROUND: To solve the disadvantages of traditional fetal monitoring such as time-consuming, cumbersome steps and low coverage, it is paramount to develop remote fetal monitoring. Remote fetal monitoring expands time and space, which is expected to popularize fetal monitoring in remote areas with the low availability of health services. Pregnant women can transmit fetal monitoring data from remote monitoring terminals to the central monitoring station so that doctors can interpret it remotely and detect fetal hypoxia in time. Fetal monitoring involving remote technology has also been carried

Intrapartum electronic fetal monitoring features associated with a clinical diagnosis of nonreassuring fetal status.

BACKGROUND: Nonreassuring fetal status detected by continuous electronic fetal monitoring accounts for almost 1 in 4 primary cesarean deliveries. However, given the subjective nature of the diagnosis, there is a need to identify the electronic fetal monitoring patterns that are clinically considered nonreassuring. OBJECTIVE: This study aimed to describe which electronic fetal monitoring features are most commonly associated with first-stage cesarean delivery for nonreassuring fetal status, and to evaluate the risk of neonatal acidemia following cesarean delivery for nonreassuring fetal status

Fetal heart rate and transcutaneous monitoring during experimentally induced hypoxia in the fetal dog.

A model of impaired oxygen delivery, using an acute surgical preparation in the fetal dog, is described. Fetal heart rate, transcutaneous pO2 and pCO2, and tissue pH were simultaneously recorded during hypoxic episodes produced by a series of successive occlusions of the maternal abdominal aorta. Corresponding values were also determined in the arterial blood of the fetus. The following pathophysiologic sequence of events was observed: 1) a latency period with no changes; 2) a drop in pO2 which stabilized later at a lower pressure; 3) late fetal heart rate deceleration, the pattern of which wa

Deep Time-to-Event Models for Intrapartum Fetal Monitoring.

Timely detection of fetal distress during labor is a central pursuit of modern obstetric care. The primary measure used in this regard is fetal heart rate and contraction monitoring (echocardiotocography or CTG), which provides a continuous signal during labor. The vast majority of machine learning applications to CTG assume access to a 30 to 60 minute window just prior to delivery, which makes an untenable assumption that the timing of delivery is known. In this work, we reframe the problem as a time-to-event prediction task. We develop deep time-to-event models to predict the joint probabili
