Arthroscopic Complete Synovectomy of the Proximal Interphalangeal Joint of the Finger.

Rheumatoid arthritis is characterized by hypertrophic synovitis destroying the involved joints. If medical treatment fails to control the synovitis, synovectomy is a worthwhile prophylactic procedure that might help to delay further distention of the joint capsule and ligament. Synovitis of the proximal interphalangeal joint is usually performed by an open approach. The major complication is scar development around the interphalangeal joint, leading to considerable joint contracture. Arthroscopy of the proximal interphalangeal joint is a minimally invasive technique that can eliminate the risk

[Differential therapeutic Approaches in the prosthetic Arthroplasty of Finger Joints].

Degenerative changes of the finger joints often lead to various surgical treatments such as arthrodesis or artificial joint replacement. There are a lot of surgical approaches for surgeons to choose from. The different approaches with their characteristic advantages and disadvantages during surgery and for the postoperative treatment should be selected and applied based on patient-, implant- and aftercare-specific indications. This article presents a short review of possible approaches and associated surgical techniques for finger joint replacement.

[Implant arthroplasty in the metacarpophalangeal and proximal interphalangeal finger joints].

BACKGROUND: Osteoarthritic changes in the finger joints are common, especially in the elderly population. Without adequate treatment, these changes can lead to pain, joint deformity, instability or impaired motion. Operative treatment options can be divided into prosthetic joint replacement, joint fusion and denervation. PATIENTS/MATERIAL AND METHOD: During the last decades, various prosthetic implant designs have appeared on the market. This article provides an overview of implant evolution, current implants, clinical results and promising technical novelties. RESULTS: Due to favourable cli

A Novel Underactuated Robotic Finger with Variable Stiffness Joints.

Existing robotic hands mostly consist of rigid finger mechanism with constant joint stiffness, leading to poor handling performance and even unexpected safety issues. This paper proposed a novel underactuated robotic finger with variable stiffness joints based on human finger anatomy and electrostatic adhesion(ESA) principle. The proposed finger is unique in the 3D printable one-piece body structure consisting of three similar joints, actuated by only one linear actuator to mimic the flexion/extension movement of the human finger. It is characterized by simple actuation, light weight, low cost

Simultaneous double interphalangeal joints dislocation in a finger in a teenager.

BACKGROUND: Simultaneous dislocation of the proximal and distal interphalangeal joints of the same finger is a rare event. In literature simultaneous double interphalangeal dislocation in the same finger has been reported only one case in a skeletally immature patient which was a fourteen year old male. Our case was twelve years old which is the youngest case in the literature that has been reported. CASE DESCRIPTION: A 12-year-old male injured his right ring finger while playing football. Examination revealed a stepladder deformity. There was no sign of neurovascular damage. Radiographs show

Successful manual reduction of locked metacarpophalangeal joints in fingers.

BACKGROUND: Many studies on the etiology and operative treatment of locked metacarpophalangeal joints in fingers have been reported, but there have been few investigations on manual reduction. The rate of success of manual reduction in previous reports has been low, and no consensus has been reached with regard to the best method of manual reduction. On the basis of our experience with operative treatment, we devised a safe method of manual reduction. METHODS: Between January 1987 and December 1995, we reduced a locked metacarpophalangeal joint in twelve female patients; every locked finger w

Entrapment of the metacarpophalangeal joint of the middle finger. Case report.

Locking of the second metacarpal joint is common, but locking of other metacarpal joints is unusual. We present a case, in which the joint of the third finger locked and the mechanism was an osteophyte catching of the ulnar accessory collateral ligament. In such a case, the joint should be explored through a palmar approach. Conservative treatment is not recommended.

[Rheumatoid arthritis: surgery of the metacarpophalangeal joints of the long fingers].

The author reviews the pathophysiological mechanisms of rheumatoid deformations of the long fingers' MP joint. He considers the different possibilities of surgical treatments; then focusing on arthroplasty, he compares and comments on technical points (incision, resection of the MP head, section of intrinsic muscles, size of the implant, joint capsule and ligament reconstruction, dressing, postoperative care) and results (mobility, implant fracture, ulnar deviation and palmar dislocation) achieved by various authors. He concludes that the Swanson implant remains at present the best therapeutic

[Septic arthritis of the finger joints. Treatment with immediate arthrodesis].

A consecutive series of 44 patients with septic arthritis of finger joints is presented. The patients were treated with antibiotics, incision, debridement and usually drainage. In 13 patients with destruction of the joint cartilage, an arthrodesis was made at the time of incision. The failure rate with conventional therapy was 42 per cent. The failure rate in the arthrodesis group, which represented all the severe cases, was 23 per cent. In septic arthritis of finger joints with destruction of the cartilage arthrodesis with Kirschner-wire and interosseous wiring is recommended at the time of p

Osteophytes and joint space narrowing are independently associated with pain in finger joints in hand osteoarthritis.

OBJECTIVE: To study the associations between structural abnormalities on ultrasound (US) or conventional x-rays (CR) and pain in hand osteoarthritis (HOA). METHODS: In 55 consecutive patients with HOA (mean age 61 years, 86% women) fulfilling the American College of Rheumatology criteria, pain in 30 separate hand joints was assessed upon palpation; osteophytes were assessed by US and CR and joint space narrowing (JSN) by CR. Associations between structural abnormalities and pain per joint were analysed using generalised estimated equations to account for patient effects and adjusted for age,

MR imaging of the metacarpophalangeal joints of the fingers: evaluation of 38 patients with chronic joint disability.

OBJECTIVE: To report the MR imaging findings of painful injured metacarpophalangeal (MCP) joints of the fingers. DESIGN AND PATIENTS: MR imaging of 39 injured MCP joints in 38 patients was performed after a mean delay of 8.8 months. The MR images were obtained with the fingers in extended and flexed positions using T2-weighted and T1-weighted sequences before and after intravenous injection of a gadolinium compound. Ten patients were treated surgically. Mean clinical follow-up was 1.8 years. RESULTS: Tears of the collateral ligaments were the most common lesion (30/39), most being radial in

Functional evaluation of optimal index finger proximal interphalangeal joint fusion angle using a simulated joint arthrodesis.

This study aims at qualitatively and quantitatively evaluating the effects of simulated index finger proximal interphalangeal (PIP) joint fusion angles on hand kinematic function and performance. Although arthrodesis of the index finger PIP joint is an effective medical procedure that produces a durable, pain-free, and stable joint, it permanently immobilizes the joint. Twenty healthy subjects performed basic functional hand activities with the index finger PIP joint unconstrained (UC) and constrained to selected angles under surveillance of a motion capture system. Our results indicate differ

Rotatory subluxation of the metacarpophalangeal index finger joint: a case report.

We report a case of rotatory subluxation of the metacarpophalangeal joint (MCPJ) of the finger. A 40-year-old man sustained an open injury to his index finger following an explosive injury. Radiographs showed rotatory subluxation of the index finger MCPJ. The index finger extensor digitorium was found interposed in the MCPJ, with a complete tear of the radial collateral ligament. Treatment was by open reduction and repair of the collateral ligament and the extensor tendon. A high level of clinical suspicion is needed to diagnose this entity.
