[Cerebral malaria after a two-week charter tour to Gambia].

Cerebral malaria is a potentially life-threatening complication of Plasmodium falciparum malaria. We describe a case of severe cerebral malaria in a previously healthy 63-year-old Danish female who had returned from a two-week tourist tour to Gambia. The severe course of the infection described in this case draws attention to two major aspects regarding imported cases of malaria in non-endemic countries: the need for thorough counselling, including chemoprophylaxis, and continuous focus on potential malaria infection after travelling to malaria-endemic areas.

A Belgian traveler who acquired yellow fever in the Gambia.

A 47-year-old Belgian woman acquired yellow fever during a 1-week vacation in The Gambia; she had never been vaccinated against yellow fever. She died of massive gastrointestinal bleeding 7 days after the onset of the first symptoms. This dramatic case demonstrates that it is important for persons to be vaccinated against yellow fever before they travel to countries where yellow fever is endemic, even if the country, like The Gambia, does not require travelers to be vaccinated.

Trial of the Trachoma Rapid Assessment methodology in The Gambia.

PURPOSE: To test the validity and repeatability of a newly designed Trachoma Rapid Assessment (TRA) methodology. METHOD: Results from an initial TRA in 20 villages (TRA1) are compared with those of a second validation TRA (TRA2) after 6 weeks in 13 villages, randomly selected out of the original 20, conducted by different field staff. Findings of TRA1 and TRA2 are compared with those of a door-to-door survey of the entire population of all 13 villages. RESULTS: There is a considerable amount of variation between the results of TRA1 and TRA2 and between the results of both TRA's and the door-

Seasonal variation of paediatric diseases in The Gambia, west Africa.

In a 3-year prospective study of 9584 consecutive paediatric admissions to the Royal Victoria Hospital in Banjul, The Gambia, we examined the impact of seasonal variations in childhood diseases. The four principal diseases (falciparum malaria, pneumonia, gastro-enteritis and malnutrition) all peaked in September to October following the rainy season. The mortality rate was also higher in the rainy season than in the dry season. Of the 1525 children with cerebral malaria, 83% were admitted during the extended rainy season from July to December. With the emergence of chloroquine-resistant malari

Experimental Transmission of Plasmodium malariae to Anopheles gambiae.

Our current knowledge of the clinical burden, biology, and transmission of Plasmodium malariae is extremely scarce. To start addressing some of those questions, we experimentally infected Anopheles gambiae mosquitoes with fresh P. malariae isolates obtained from asymptomatic individuals in Lambaréné, Gabon. The proportion of mosquitoes infected via direct membrane feeding assay with either P. malariae monoinfections (16% [19 of 121]) or coinfections (28% [31 of 112]) was higher after serum replacement than in parallel groups without serum replacement (4% [4 of 102] and 4% [2 of 45], respective

Viral paratransgenesis in the malaria vector Anopheles gambiae.

Paratransgenesis, the genetic manipulation of insect symbiotic microorganisms, is being considered as a potential method to control vector-borne diseases such as malaria. The feasibility of paratransgenic malaria control has been hampered by the lack of candidate symbiotic microorganisms for the major vector Anopheles gambiae. In other systems, densonucleosis viruses (DNVs) are attractive agents for viral paratransgenesis because they infect important vector insects, can be genetically manipulated and are transmitted to subsequent generations. However, An. gambiae has been shown to be refracto

Reproductive physiology of Anopheles gambiae and Anopheles atroparvus.

When exposed to a human host, Anopheles gambiae started probing 4 h post-eclosion, but 95% successfully blood-fed by 16-20 h with maximal blood volumes of 5- 10 microl per female. When fed sugar, the 95% feeding was not observed until 36-40 h post-eclosion; sugar meals appeared to interfere with blood meals. Similarly in An. atroparvus, maximum volumes were 10 microl when starved but only 6 microl when fed sugar. This species did not bite before 2 d, and 95% biting was by 4 d. Given single blood meals to water-kept An. gambiae, a threshold body size for oogenesis was detected. With wing length

Prevalence of anaplasmosis and babesiosis in N'Dama cattle of the Gambia.

Sera from 184 N'Dama cattle randomly selected and averaging 2-7 years of age were tested for the presence of specific antibodies to Anaplasma marginale, Babesia bovis and B. bigemina, using one or more serological tests including complement fixation, rapid card agglutination and indirect fluorescent antibody (IFA). Tests for A. marginale and B. bovis were essentially negative. Utilising the IFA test 65% of the sera tested were positive for B. bigemina. Three randomly selected two-year-old N'Dama bulls were splenectomised. All three showed an acute recurrence of a B. bigemina parasitaemia. Two

Paediatric cardiac care in low resource settings evidence from the Gambia.

Paediatric congenital heart disease remains an important contributor to childhood morbidity in The Gambia. Evidence from 2008 to 2022 shows improved detection and reduced in-hospital mortality due to strengthened diagnostics and clinical care. However, late presentation and limited access to definitive cardiac interventions continue to drive adverse outcomes, highlighting the need for enhanced early detection, regional cardiac services, and integrated preventive strategies.

Health economic evaluation of hip fracture care pathways in The Gambia.

Hip fractures cause substantial morbidity, mortality and healthcare costs among older adults. While advances in surgical and postoperative care have improved outcomes in high-income countries, these interventions are often inaccessible in low-income and middle-income countries due to cost constraints. This study assessed the costs and outcomes of surgical versus non-surgical management of hip fractures to inform resource allocation in a resource-limited setting. Data from a prospective cohort study of patients with acute hip fractures over a 12-month postfracture follow-up period were used. Ou

Prevalence and risk factors associated with corneal opacity in The Gambia.

BACKGROUND/AIMS: Corneal opacity (CO) is a significant cause of vision impairment and blindness worldwide. The first National Eye Health Survey in The Gambia (1986) and follow-up (1996) showed a decline in the crude prevalence of blindness but an increase in blindness from non-trachomatous CO. This study aimed to provide an update on the prevalence of CO and identify associated risk factors. METHODS: Multi-stage stratified cluster random sampling was used to select a nationally representative sample of 10 800 adults aged ≥35 years. Distance best-corrected visual acuity was assessed with Peek

Hepatitis D virus infection, cirrhosis and hepatocellular carcinoma in The Gambia.

Hepatocellular carcinoma (HCC) incidence is high in The Gambia, and hepatitis B virus (HBV) infection is the main cause. People coinfected with HBV and hepatitis D virus (HDV) have an even greater risk of HCC and cirrhosis. Using a new HDV quantitative microarray antibody capture (Q-MAC) assay, we evaluated the association between HDV infection and HCC or cirrhosis among participants in The Gambia Liver Cancer Study. In this case-control study, cases had HCC (n = 312) or cirrhosis (n = 119). Controls (n = 470) had no clinical evidence of liver disease and normal serum alpha-foetoprotein. Parti

Bacterial infection and trachoma in the gambia: a case control study.

PURPOSE: Trachoma is the leading infectious cause of blindness worldwide. Conjunctival scarring is initiated by recurrent Chlamydia trachomatis infection. However, disease progression to trichiasis occurs even in regions where chlamydial prevalence is currently low, which suggests that other factors, for example other bacterial infection, may also drive inflammation and scarring, particularly in the late stages of trachoma. This study was undertaken to investigate whether trachomatous trichiasis or conjunctival scarring are associated with increased prevalence of bacterial infection. METHODS:

Prospective analysis for community participation in trypanosomosis control in The Gambia.

The shift towards community participation in the eradication of trypanosomosis calls for investigation the underlying incentive structure for individuals in the community to cooperate in the provision of various control methods. Survey data were used to assess patterns of the community's demand for insecticide pour-ons and trypanocidal drugs and factors affecting individual demand in The Gambia. The results show that insecticide pour-on is strongly preferred. Similarly, farmers revealed a preference for community-based provision scheme. Factors affecting an individual farmer's decision to inve

Non-trachomatous corneal opacities in the Gambia--aetiology and visual burden.

AIMS: National blindness surveys conducted in the Gambia in 1986 and 1996 showed an increase in blindness and visual impairment from non-trachomatous opacity. This study aimed to investigate the aetiology of these opacities and to assess the resulting visual burden. METHODS: A population-based, randomised blindness survey was conducted in the Gambia in 1996. Patients with visual impairment or blindness were examined by an ophthalmologist with a slit lamp. Causes of corneal opacity were determined as accurately as possible by clinical history and examination. RESULTS: A total of 154 patients

Epidemiology of human hookworm and Ascaris lumbricoides infestations in rural Gambia.

In a cross-sectional study of helminth infections in a rural village in The Gambia, West Africa, hookworm, probably Necator americanus and Ascaris lumbricoides were found to be the most prevalent helminths present at prevalence levels of 30% and 25% respectively. Other parasites present were Trichuris trichiura (2.4%) and Schistosoma mansoni (1.5%). The mean egg counts of N. americanus and A. lumbricoides in all age groups and in the total population were low. Egg counts between age and sex groups were not statistically significant. Haptoglobin polymorphism and its relationship to malaria infections in The Gambia.

The haptoglobin (Hp) status of the population of Keneba, The Gambia was investigated. Of the 825 persons examined 22.9% were ahaptoglobinaemic (HpO). The incidence of HpO was higher in the 209 persons showing malaria parasites in their blood (38.8%) than in non-parasitaemic individuals (17.5%). Children less than two years old had low incidences of HpO except in the first two months of life. After two years the incidence rose with little tendency to change in older age groups. No correlations were seen between HpO and sex, anaemia or sickle cell trait. However, a positive correlation was found
