Gingival enlargements and localized gingival overgrowths.

Gingival enlargements are a common clinical finding and most represent a reactive hyperplasia as a direct result of plaque related inflammatory gingival disease. These generally respond to conservative tissue management and attention to plaque control. However, a small group are distinct from these and whilst they also represent a reactive tissue response, this occurs at the level of the superficial fibres of the periodontal ligament. These epulides grow from under the free gingival margin and not as a result of a primary inflammatory gingival enlargement. This distinct aetiopathogenesis separ

A rare case of generalized pyogenic granuloma: a case report.

Gingival enlargement is defined as an overgrowth or increase in size of the gingiva. Enlargement can be of many types depending on etiologic factors like inflammation, drug-induced effects, neoplasm, hormonal imbalance, and systemic involvement (leukemia, etc). Drugs and hormonal imbalance are the most common causes of gingival enlargement. Nonspecific conditioned enlargement, or pyogenic granuloma, is considered an exaggerated conditioned response to minor trauma or chronic irritation. Pyogenic granuloma occurring in the oral cavity is a common phenomenon. However, simultaneously occurring ge

Aggressive pregnancy tumor mimicking a malignant neoplasm: a case report.

AIM: The aim of this report is to present the management of an aggressive, highly proliferative pregnancy tumor with clinical and radiographic characteristics highly suggestive of a malignant neoplasm. BACKGROUND: Pregnancy tumor is a benign hyperplastic gingival lesion occurring during pregnancy that is indistinguishable from a pyogenic granuloma arising in nonpregnant females, or in males. The lesion usually grows over a few months and tends to bleed. CASE DESCRIPTION: A 28-year-old woman at four months of gestation was referred for a massive gingival swelling (5.5 cm in greatest diameter)

Embryogenesis of the gingival cyst.

The embryogenesis of gingival cysts derived from the dental lamina was studied histologically in paraffin prepared sections of 10 pairs of human fetal jaws and in individual celloidin sections of human fetal skulls. Cystic degeneration in the dental lamina was noted as early as 10 weeks in utero prior to the separation of the developing tooth bud. Rapid proliferation and growth of such cysts was seen in 15-20 week old embryos during the morphodifferentiation stage of tooth development after fragmentation of the lamina had occurred. Islands, strands and nests of dental lamina epithelium were di

Peripheral odontogenic fibroma: A case report and review.

Odontogenic tumors mainly occur as intraosseous growths but sometimes may present in a peripheral location on the gingiva where they are referred to as peripheral odontogenic tumors (POTs) which are a rare entity, the most common of them being the peripheral odontogenic fibroma that is an otherwise uncommon, slowly growing, benign odontogenic neoplasm of the periodontal soft tissues. In fact, peripheral odontogenic fibroma is the only POT that is more frequent than its central counterpart. Although considered to be with a potential to recur after excision, the actual recurrence rate is not kno

Gingival swelling in a 13-year-old girl with multiple recurrences.

We present a challenging case of gingival swelling that recurred several times in 3.5 years in a 13-year-old girl. Histologic evaluation of the initial biopsy revealed groups of epithelioid cells surrounded by lymphocytes in the submucosal fibrous connective tissue, and it was diagnosed as noncaseating granulomatous inflammation. With the tissue specimens from subsequent multiple excisions and several immunohistochemical studies, the diagnoses evolved to benign cellular infiltrate of undetermined origin, epithelioid hemangioma, proliferating endothelial cell neoplasm of uncertain biologic pote

Aggressive type of central giant cell granuloma in a woman on hormone replacement therapy: a histopathological insight.

Quite a few lesions of the oral cavity specifically of the gingiva have a greater inclination towards women and mostly occur during the first four decades of life, the cause of which may be credited to the changing levels of sex hormones. Out of all such lesions, one lesion whose aetiology is still unclear and which originats from the periosteum or periodontal ligament is central giant cell granuloma. Repeatedly, it is has been described as a reactive lesion, the cause of which may be secondary to local irritants or trauma, specifically plaque or calculus, which is not considered to be a true

The incidence and pattern of oro-facial infection and swellings in Nigerian children.

Between the 2 of january and the 31 of december 1985 there was done a study of 215 children in ages between 0-15 years in the Department of Oral Diagnosis of the Ibadan University Hospital, where they were external patients with oral infections/swelling. The male-female proportion was 1.3:1. 42.33% were from 0 to 5. 70.24% belonged to the lowest social group of the community, with a high risk of oral-facial infections. The situation of their oral hygiene didn't reflect their social and economical state, and meant no predisposition to infection. The common infections were alveolar abscess, acut

Odontogenic myxofibroma of gingiva in a pediatric patient with tuberous sclerosis: A rare case report.

Tuberous sclerosis complex (TSC) is a rare multisystem genetic disease, with an estimated incidence of 1 in 6000-1 in 10,000. TSC is an autosomal dominant syndrome involving heart, kidneys, lungs, and skin. The classic triad of TSC is seizures, mental retardation, and angiofibromas; this triad occurs in only 29% of patients. The clinical diagnostic guidelines on TSC are prepared based on clinical features, radiographic findings. The most common oral manifestations of TSC are fibroma, gingival hyperplasia, and enamel hypoplasia. Odontogenic myxofibroma represents a rare slow-growing benign neop

Diagnostic and therapeutic challenges of myeloid sarcoma in the oral cavity.

Myeloid sarcoma (MS) is a rare neoplasm characterized by the proliferation of immature myeloid precursor cells outside the bone marrow. The pathogenesis of MS is complex and not completely understood. Moreover, it develops in any extramedullary site of the body. In this editorial, we discuss the article published by Li et al, which presents a clinical case involving a 32-year-old man who exhibited gingival inflammation in the maxillary region. It was initially diagnosed as periodontal disease. However, clinical evaluation revealed a firm, grayish-white mass which underscored the need for compr

Erythematous and Vascular Oral Mucosal Lesions: A Clinicopathologic Review of Red Entities.

Erythematous lesions of the oral mucosa are common and can reflect a variety of conditions, ranging from benign reactive or immunologically-mediated disorders to malignant disease. Together with vascular abnormalities, which can vary from reddish to bluish-purple in color, the differential diagnosis for erythematous oral mucosal change is quite diverse. This review focuses on salient clinical features and histopathologic findings of selected conditions which clinically present as red or vascular-like oral mucosal alterations, including oral vascular malformations and neoplasms, pyogenic granul

A Case of Hepatocellular Carcinoma Presenting as a Gingival Mass.

Oral metastatic tumor, which is uncommon and represents less than 1% of malignant oral neoplasms, usually arises from a primary mucosal or cutaneous cancer located in the head and neck regions. Metastasis of hepatocellular carcinoma (HCC) to the oral cavity, especially to gingiva, is extremely rare. A 50-year-old man, who was a chronic alcoholic and hepatitis B virus carrier, presented with abdominal distension and weight loss for the past 3 months. Three-phased contrast-enhanced abdominal CT revealed numerous conglomerated masses in the liver, suggesting huge HCCs arising in the background of

Renal cell carcinoma presenting as a cutaneous horn and nodules on the gingiva and scalp.

A 63-year-old man presented with a pulsatile cutaneous horn on the nose and multiple angiomatous nodules on the gingiva and scalp, which appeared over 2 months. He had severe hypercalcaemia, lytic lesions in multiple bones and acute kidney injury. Excision biopsy from the gingival nodule showed a clear cell neoplasm. The bone marrow showed atypical cells with similar morphology. Imaging showed a 7 cmx7.5 cm mass at the upper pole of the left kidney with metastases to the bones, liver and lung. Immunohistochemistry was consistent with metastatic renal cell carcinoma. Renal cell carcinoma presen

Metastatic renal cell carcinoma in the gingiva mimicking a hyperplastic lesion. Case report.

An unusual case of metastatic cell carcinoma in the gingiva, without roentgenographic evidence of underlying bone involvement, is described. The tumor had the clinical appearance of a pyogenic granuloma and peripheral giant cell granuloma. Histologic sections and the pertinent history revealed the tumor to be a metastatic renal cell carcinoma. Review of the literature reveals that metastatic lesions to the gingiva are rare, but when they occur they tend to mimic clinically the common localized hyperplastic lesions of the gingiva.
