Early to midterm outcomes of anatomic shoulder arthroplasty performed on dysplastic glenoids.

BACKGROUND: Treatment of primary osteoarthritis with glenoid dysplasia or Walch type C glenoids remains controversial. There is scant literature available on patient outcomes after anatomic shoulder arthroplasty in patients with Walch type C glenoids. The purpose of this study was to evaluate the outcomes of total shoulder arthroplasty (TSA) for Walch type C dysplastic glenoids with standard (nonaugmented) glenoid components compared with TSA for glenoids with concentric wear and minimal erosion (Walch type A1). We hypothesized that TSA performed for Walch type C dysplastic glenoids with stand

Anterior glenohumeral instability: a pathology-based surgical treatment strategy.

The glenohumeral joint is the most frequently dislocated major joint, and most cases involve an anterior dislocation. Young male athletes competing in contact sports are at especially high risk of recurrent instability. Surgical timing and selection of surgical technique continue to be debated. Full characterization of the injury requires an accurate history and physical examination. Diagnostic imaging assists in identifying the underlying anatomic lesions, which range from no discernible lesion to significant bone loss of the glenoid or humeral head and/or capsulolabral stretching or avulsion

Revision of the loose glenoid component in anatomic total shoulder arthroplasty.

Loosening of the glenoid component is a frequent cause of failure of total shoulder arthroplasty (TSA). The etiology of glenoid component loosening is multifactorial and includes aseptic osteolysis, rotator cuff insufficiency, soft tissue instability, and infection. A loose glenoid component is frequently associated with a substantial loss of glenoid bone, which necessitates additional procedures to implant a new component. Several studies have shown that patients with a new glenoid component have better clinical outcomes, which makes successful glenoid reimplantation a priority. The reconstru

[Possibilities of magnetic resonance tomography in diagnostic imaging of the shoulder joint].

By virtue of its multiplanar representation, magnetic resonance imaging (MRI) allows clear visualization of the complex anatomical relationships of the shoulder joint. In addition to axial planes, slices perpendicular and parallel to the glenoid cavity are used to good advantage. In tears of the rotator cuff an increase in signal intensity within the cuff is recognized in T2- and proton-density-weighted images. Lesions of the glenoid labrum following luxations of the glenohumeral joint can be detected and classified using MRI. The diagnostic value of MRI as compared with other imaging modaliti

The Lego Block Technique: A Novel Method of Shaping Autogenous Bone Graft for Narrow Glenoids with Severe Bone Defects in Reverse Shoulder Arthroplasty.

In cases with complex glenoid bone defects, bony increased-offset reverse shoulder arthroplasty can restore glenoid bone stock and improve stability using autografts harvested from the humeral head. However, conventional grafting methods may be insufficient when glenoid defects or deformities are complex. We present a novel bracket-shaped bone grafting technique applied in a patient with advanced rheumatoid arthritis and significant glenoid erosion. The graft was sculpted from the resected humeral head to match the glenoid defect in a Lego block-like fashion, resulting in stable fixation and f

Editorial Commentary: Open Bankart Procedure for Anterior Shoulder Instability Is Recommended for Contact Athletes With Bony Bankart Fractures or Approximately 10% Glenoid Bone Loss.

Recurrent anterior shoulder instability with subcritical bone loss is a challenging clinical scenario. Many treatment options have been described, including arthroscopic Bankart repair, arthroscopic Bankart repair plus remplissage, open Bankart repair, and the Latarjet procedure. Arthroscopic Bankart repair alone has higher rates of failure in high-risk patient populations, especially at long-term follow-up. These high-risk populations include patients with significant glenoid bone loss (>20%), bipolar bone loss (including subcritical bone loss and off-track or "near-track" [8-10 mm] Hill-Sach

The Role of Inlay and Inset Glenoid Implants in Anatomic Total Shoulder Arthroplasty.

Anatomic total shoulder arthroplasty is an established treatment for degenerative conditions involving the shoulder. One well-described complication of total shoulder arthroplasty is glenoid loosening, especially in younger and more active patients. As a result, several surgeons recommend against activities that may put patients at risk of implant failure after shoulder arthroplasty. Newer inlay and inset glenoid implants may have a role in treating arthritis in younger patients and may allow them to return to sports and high-impact activities, including weightlifting. These glenoid implants m

Anatomic measurement of osseous parameters of the glenoid.

The angle and position of the scapular glenoid are important in shoulder mechanics, the interpretation of diseases, and planning shoulder replacement surgery. In total shoulder replacement, understanding the bony parameters of the glenoid is also of considerable guiding significance for designing implant size and improving material adaptability. To compare glenoid parameters measured from skeletal scapula specimens with those measured by 3D modeling of CT scanning images, analyze correlations between these data, and draw conclusions to guide clinical treatment of shoulder joint injury and tota

When to Abandon the Arthroscopic Bankart Repair: A Systematic Review.

CONTEXT: Bone loss is a major factor in determining surgical choice in patients with anterior glenohumeral instability. Although bone loss has been described, there is no consensus on glenoid, humeral head, and bipolar bone loss limits for which arthroscopic-only management with Bankart repair can be performed. OBJECTIVE: To provide guidelines for selecting a more complex repair or reconstruction (in lieu of arthroscopic-only Bankart repair) in the setting of glenohumeral instability based on available literature. DATA SOURCES: An electronic search of the literature for the period from 2000

The Latarjet-Patte procedure for recurrent anterior shoulder instability in contact athletes.

Recurrent anterior shoulder instability is common in contact athletes and the high-energy injuries seen in this group make them more prone to bone loss. Athletes with recurrent instability and associated bone loss have high failure rates when treated with a soft tissue reconstruction procedure. Therefore it is preferred to manage recurrent instability in contact athletes with the Latarjet-Patte procedure. In this article, the authors describe their technique. They have found this procedure to be safe and effective, with very low recurrence and early return to sport. A meticulous surgical techn

Anatomic study of the superior glenoid labrum.

The purpose of the study was to describe the normal anatomy of the glenoid labrum to help identification of pathology and guide surgical repair. Twenty dry bone scapulae and 19 cadaveric shoulders were examined. Light microscopy was performed on 12 radial slices through the glenoid. An external capsular circumferential ridge, 7-8 mm medial to the glenoid rim marks the attachment of the capsule. A separate internal labral circumferential ridge 4 mm central to the glenoid rim marks the interface between the labrum and articular cartilage. A superior-posterior facet was found consistently on the

Analysis and 3D correction of glenoid dysplasia with metal hemi-wedge base plate augment: short-term radiographic outcomes.

BACKGROUND: Glenoid defects can be addressed traditionally by asymmetric reaming or by bone-preserving correction to a more lateral joint line by bone or metal augmented baseplates in reverse shoulder arthroplasties. While there is more evidence in literature regarding the outcome and complications of Bony Increased Offset Reversed Shoulder Arthroplasty (BIO-RSA), there is minimal reported experience with the outcome after metal glenoid augments. The aim of this study was to determine whether a metal augment can correct the glenoid deformity in an anatomic manner. METHODS: Glenoid morphology

Managing Glenoid Deformity in Shoulder Arthroplasty: Role of New Technology (Computer-Assisted Navigation and Patient-Specific Instrumentation).

The glenoid is considered a weak link in total shoulder arthroplasty because failure on the glenoid side is one of the most common reasons for revision of total shoulder arthroplasty. Glenoid wear is commonly seen in glenohumeral arthritis and compromises glenoid bone stock and also alters the native version and inclination of the glenoid. It is critical to recognize glenoid wear and correct it intraoperatively to avoid component malposition, which can negatively affect the survivorship of the glenoid implant. The end point of correction for the glenoid wear in shoulder arthroplasty is controv
