Shoulder hemiarthroplasty to manage haemophilic arthropathy: two case studies.

Bleeds within the shoulder joint can lead to significant joint destruction and may have associated pain, decreased range of movement (ROM), and impaired function. Conservative management should involve prompt administration of Factor VIII and physiotherapy to address all surrounding structures so as to minimize further damage. If conservative management fails to relieve severe, unremitting shoulder pain in the presence of underlying arthropathy, then arthroplasty may be considered. Outcomes of arthroplasties performed for osteoarthritis and rheumatoid arthritis appear favourable. Few articles,

Stabilizing the Unstable: Cone Hemiarthroplasty in Geriatric Intertrochanteric Fractures.

BACKGROUND: Unstable intertrochanteric fractures in the elderly pose a significant treatment challenge due to poor bone quality, comminution, and associated comorbidities. Bipolar hemiarthroplasty offers the advantage of early mobilization and reduced fixation-related complications. This study evaluates short-term outcomes of bipolar hemiarthroplasty using an uncemented titanium fluted, tapered cone femoral prosthesis in such fractures. METHODS: A retrospective analysis was conducted on 43 consecutive elderly patients treated with uncemented bipolar hemiarthroplasty using a tapered, fluted co

Four-Year Outcomes of Midcarpal Hemiarthroplasty for Wrist Arthritis.

PURPOSE: The purpose of this study was to review the average 4-year outcomes of a cohort of patients with wrist arthritis, treated by a single surgeon with a novel prosthetic hemiarthroplasty of the midcarpal joint. We hypothesized that midcarpal hemiarthroplasty would improve range of motion and grip strength of patients with wrist arthritis, with a complication profile comparable with that of alternative solutions for wrist arthritis. METHODS: We reviewed a series of 20 patients treated with a midcarpal hemiarthroplasty at an average of 4 years following surgery. Patients were evaluated obj

Contact mechanics of reverse engineered distal humeral hemiarthroplasty implants.

Erosion of articular cartilage is a concern following distal humeral hemiarthroplasty, because native cartilage surfaces are placed in contact with stiff metallic implant components, which causes decreases in contact area and increases in contact stresses. Recently, reverse engineered implants have been proposed which are intended to promote more natural contact mechanics by reproducing the native bone or cartilage shape. In this study, finite element modeling is used in order to calculate changes in cartilage contact areas and stresses following distal humeral hemiarthroplasty with commercial

Periprosthetic fractures around hip hemiarthroplasty performed for hip fracture.

UNLABELLED: Hip fracture is associated with considerable morbidity and mortality and occurs in an elderly and infirm group of patients. Periprosthetic fracture after hip hemiarthroplasty is a serious complication. In this study, we have reviewed our experience of this injury. The outcome measures used were fracture union, mortality, infection and requirement for further surgery. METHOD: We identified a cohort of 79 patients who sustained periprosthetic fractures after hip hemiarthroplasty from a prospective hip fracture database of 8354 patients (3611 were treated with hemiarthroplasty). Seve

Midcarpal hemiarthroplasty for wrist arthritis: rationale and early results.

Midcarpal hemiarthroplasty is a novel motion-preserving treatment for radiocarpal arthritis and is an alternative to current procedures that provide pain relief at the expense of wrist biomechanics and natural motion. It is indicated primarily in active patients with a well-preserved distal row and debilitating arthritic symptoms. By resurfacing the proximal carpal row, midcarpal arthroplasty relieves pain while preserving the midcarpal articulation and the anatomic center of wrist rotation. This technique has theoretical advantages when compared with current treatment options (i.e., arthrodes

Cementless bipolar hemiarthroplasty in femoral neck fractures in elderly.

BACKGROUND: Cemented hip arthroplasty is an established treatment for femoral neck fracture in the mobile elderly. Cement pressurization raises intramedullary pressure and may lead to fat embolization, resulting in fatal bone cement implantation syndrome, particularly in patients with multiple comorbidities. The cementless stem technique may reduce this mortality risk but it is technically demanding and needs precise planning and execution. We report the perioperative mortality and morbidity of cementless bipolar hemiarthroplasty in a series of mobile elderly patients (age >70 years) with femo

Functional outcome after proximal humeral fractures treated with hemiarthroplasty.

OBJECTIVE: To evaluate functional outcome after hemiarthroplasty for displaced proximal humeral fractures and to review whether prosthesis type, intraoperative technique or previous ipsilateral shoulder surgery could affect the outcome. METHODS: We reviewed the medical records and radiographs of patients who had undergone hemiarthroplasty for proximal humeral fractures between 1992 and 2000. We identified 45 patients, 39 with acute fractures and 6 with fracture-related complications. One surgeon performed 17 surgeries (38%), and the rest were carried out by 11 other orthopedic surgeons. Dislocation after revision of hemiarthroplasty to total hip replacement.

Over a 3-year period, nine hemiarthroplasties were revised to total hip arthroplasty. The hemiarthroplasties were all performed for an original diagnosis of subcapital fracture, and the revisions were for a variety of indications including instability, loosening and acetabular erosion. Four of the revised prostheses subsequently dislocated, and one has recurrent subluxation. This dislocation rate of 50% is higher than previously reported. Factors contributing to this are discussed. These include: previous strokes, advanced age, difficulty complying with instructions, and a hip capsule not cont

Unicompartmental tibial hemiarthroplasty: early results of the UniSpacer knee.

An intra-articular metallic spacer is evaluated for the treatment of isolated medial compartment OA. It is proposed that this implant can provide axial correction for a varus deformity, eliminate the pseudolaxity of the MCL, improve knee function, maintain ROM, and provide pain relief. One-and 2-year data are presented on 71 UniSpacer Knee System implants that were implanted in 67 patients (four had bilateral surgeries). The mean age and weight of these patients was 54 years and 94 kg (207 lb), respectively. The heaviest patient in the group weighed 168 kg (370 lb). All knees were evaluated us

Hemiarthroplasty for glenohumeral arthritis with massive rotator cuff tears.

Rotator cuff deficiency with associated glenohumeral arthritis poses a formidable clinical challenge. Humeral head replacement with maintenance of the coracoacromial arch is the current treatment of choice. Properly performed arthroplasty can provide pain relief and modest gains in motion.

Primary Hemiarthroplasty for the Treatment of Basicervical Femoral Neck Fractures.

Purpose Basicervical femoral neck fractures are uncommon injuries that occur at the extracapsular base of the femoral neck at its transition with the intertrochanteric line. Controversy remains in the orthopedic literature as to the optimal method of treatment for this fracture type given the inherent instability and greater rate of implant failure with traditional fixation constructs. The purpose of this study is to quantify the incidence and preferred treatment methods of basicervical hip fractures at a single, regional, Level 1 trauma center and to identify differences in postoperative comp

Proximal Phalanx Hemiarthroplasty for the Treatment of Advanced Hallux Rigidus.

Multiple treatment options exist for the management of late-stage hallux rigidus. The goals of treatment are pain reduction and restoration of function. Arthrodesis remains the treatment of choice, but recent advances support the use of first metatarsophalangeal hemiarthroplasty as a viable and successful option in properly selected patients in whom preservation of motion and function are desirable.

Cemented or uncemented hemiarthroplasty for the treatment of femoral neck fractures?

BACKGROUND AND PURPOSE: Cemented hemiarthroplasty is preferred in treating displaced fractures of the femoral neck in the elderly. The cementing process may cause a fat embolism, leading to serious complications or death. In this study, we wanted to determine whether use of uncemented hemiarthroplasty (HA) would lead to reduced mortality and whether there are differences in the complications associated with these different types of arthroplasty. PATIENTS AND METHODS: From the PERFECT database, which combines information from various treatment registries, we identified 25,174 patients who were

Hemiarthroplasty of the hip with and without cement: a randomized clinical trial.

BACKGROUND: Controversy exists regarding the use of cement for hemiarthroplasty to treat a displaced subcapital femoral neck fracture in elderly patients. The primary hypothesis of this study was that use of cement would provide better visual analog pain scores following this procedure in an elderly patient population. METHODS: Elderly patients (at least seventy years of age) without severe cardiopulmonary compromise who presented to one institution with a displaced subcapital femoral neck fracture were offered inclusion in the study. One hundred and sixty patients (mean age, eighty-five year

Surface replacement hemiarthroplasty for the treatment of osteonecrosis of the femoral head.

We reviewed the results of thirty-three femoral resurfacing procedures in twenty-five patients who had stage-III or early stage-IV osteonecrosis of the femoral head according to the classification system of Ficat and Arlet. There were no perioperative complications. Thirty hip prostheses (91 percent) survived for a minimum of five years. At a mean of 10.5 years (range, four to fourteen years) postoperatively, sixteen (62 percent) of the twenty-six hips with stage-III disease had a good or excellent Harris hip score. Four of the seven hips with stage-IV disease did not have or need a total hip
