Investigation of 89 children born by drug-dependent mothers. I. Neonatal course.

Among 89 infants born by opiate- and methadone-addicted mothers 20% were preterm and 31% were light for gestational age. Mean gestational age and birth weight were lower in infants of mothers who had taken mainly opiates compared with infants of mothers who had taken mainly methadone. Preterm labor was more frequent among women who had been acutely withdrawn on methadone within the last month before birth than among women who were maintained on methadone at birth. 85% of the newborns had withdrawal symptoms and 12% had convulsions the severity of which was not correlated with the type of drug

Sauvons les Bébés: child health and U.S. humanitarian aid in the First World War era.

From 1917 to 1923, the American Red Cross organized an array of long-term child health projects in Europe as part of its larger wartime and post-war humanitarian efforts. Across the continent, the organization established child health clinics, better baby shows, playgrounds, fresh air camps, and courses for women on infant and child hygiene. Hundreds of U.S. doctors, nurses, and other child welfare professionals traveled to Europe to administer these programs. These activities call attention to American efforts to reform the health of European youth and, in so doing, to reshape European medici

Reducing infant mortality rates using the perinatal periods of risk model.

Despite decreases in the last 50 years, infant mortality rates in the United States remain higher than in other industrialized countries. Using overall infant mortality rates to determine the effectiveness of interventions does not help communities focus on particular underlying factors contributing to static, and sometimes increasing, community rates. This study was designed to determine and rank contributing factors to fetal-infant mortality in a specific community using the Perinatal Periods of Risk (PPOR) model. The PPOR model was used to map fetal-infant mortality for 1995 to 1998 in the

Evaluation and management of retinal hemorrhages in infants with and without abusive head trauma.

Extensive intraocular hemorrhage in young infants in the setting of acute brain injury and in the absence of a history of severe accidental trauma or underlying medical cause must be considered to be nonaccidental injury until otherwise proven. In the absence of any obvious explanation, the presence of any retinal hemorrhage should raise the possibility of abusive head trauma in the differential diagnosis and perhaps lead to such testing as skeletal radiography and neuroimaging, consultation with a child abuse specialist, as well as workup for other relevant systemic conditions. Mesenchymal hamartomas of the pediatric head and neck.

We describe the fifth published report of a mesenchymal hamartoma presenting as a cheek mass. A 5-month-old infant was brought to our institution for evaluation of an enlarging left-sided congenital cheek mass. Over time, the lesion had begun to cause significant facial asymmetry and oral incompetence. Radiologic imaging revealed an approximate 2.5 × 3.5-cm, noncystic lesion located in the left buccal space, separate from the mandible and surrounding the salivary glands. Magnetic resonance imaging (MRI) sequences demonstrated an isointense, T1-weighted lesion with avid gadolinium uptake, and i

Outbreak of herpangina associated with Coxsackievirus B3 infection.

An outbreak of acute febrile illness was observed in summer, 1987, in a welfare home in which 31 healthy infants were accommodated. Within a 5-day period 25 infants (81%) acquired a febrile illness. Coxsackievirus B3 was isolated from 16 (64%) of 25 throat swabs. In the patients in whom viral culture was negative or not performed, 6 were serologically identified as having a coxsackievirus B3 infection. Among 22 patients identified as having a coxsackievirus B3 infection 7 had typical herpangina and the others had pharyngitis with or without a few small vesicles. Serum alpha-interferon was dete

Antecedent renal disease and the outcome of pregnancy.

We assessed the effect of "healed" childhood renal disease on subsequent pregnancies by following-up a cohort of 224 children initially hospitalized with kidney disease. The pregnancy experience in this cohort was compared to two "control" cohorts comprising 81 female siblings and 191 age-matched female patients hospitalized contemporaneously for respiratory infection. The incidence of spontaneous abortion, stillbirth, and pregnancy-associated hypertension was not different among the cohorts; however, the incidence of infants with low birth weights was significantly greater in the renal and re

Infant removals: The need to address the over-representation of Aboriginal infants and community concerns of another 'stolen generation'.

OBJECTIVES: The removal of a child from their parents is traumatising, particularly in Aboriginal communities where a history of child removals has led to intergenerational trauma. This study will determine where disparities in child protection involvement exist among Aboriginal and non-Aboriginal children and characteristics associated with infant removals. Challenges faced by child protection and other agencies, and opportunities for overcoming these, are discussed. METHODS: Data from both the Australian Institute of Health and Welfare and linked Western Australian government data was used

Welfare impacts of the illegal wildlife trade in a cohort of confiscated greater slow lorises, Nycticebus coucang.

Illegal harvesting and trade are major forces behind population declines of wild slow lorises (genus Nycticebus). The impacts of the wildlife trade on individual slow lorises have not been as well described. In this article, we describe quantitatively the consequences of the wildlife trade for 77 greater slow lorises, N. coucang, who were confiscated en masse and brought to Cikananga Wildlife Center in Indonesia. Medical records indicated that in total, 28.6% of the slow lorises died within the first 6 months, mostly due to traumatic injury, and all the infants died. The greatest sources of mo

[Detection and monitoring of high-risk children].

The authors first describe problems related to the lowering of infant mortality and deal with the terms "high-risk factor" and "high-risk baby". As a result of the dangers presented by prenatal and perinatal noxae the need arises for low-risk births and optimal care of new-born babies. The public health system must be suitably organized so that babies with case history details can be supervised and others with cerebral motor disturbances without pathological data can be registered in the case history. In the registration of the latter group child-welfare centers have an important role to play.

Polyhydramnios: a predictor of severe growth impairment in achondroplasia.

The phenotypical characteristics in 20 children with achondroplasia were examined in relation to the occurrence of polyhydramnios. A history of polyhydramnios (9 of 20 cases) was associated with a lower height (SD) (P <.05), more severe shortening of the lower segment and upper limbs (P =.0001), and higher frequency of left-handedness (P =.0081).

The use of continuous surveys to generate and continuously report high quality timely maternal and newborn health data at the district level in Tanzania and Uganda.

BACKGROUND: The lack of high quality timely data for evidence-informed decision making at the district level presents a challenge to improving maternal and newborn survival in low income settings. To address this problem, the EQUIP project (Expanded Quality Management using Information Power) implemented a continuous household and health facility survey for continuous feedback of data in two districts each in Tanzania and Uganda as part of a quality improvement innovation for mothers and newborns. METHODS: Within EQUIP, continuous survey data were used for quality improvement (intervention di

Arterial ligation for pediatric epistaxis: developmental anatomy.

BACKGROUND: Anatomic studies of adult skulls have aided in the design of operations for the surgical ligation of nasal feeding vessels in the treatment of severe epistaxis. Lack of appropriate specimens has prevented similar studies in children. We performed an anthropometric study of archeological specimens to learn the effects of growth on key anatomic relationships. METHODS: We studied the skulls of children who died between 200 and 8000 years ago, recovered from archeological digs around the world. Assessing the Impact of Prenatal Medication for Opioid Use Disorder on Discharge Home With Parents Among Infants With Neonatal Opioid Withdrawal Syndrome.

OBJECTIVES: The number of women with opioid-related diagnoses in the United States has significantly increased in recent decades, resulting in concomitantly higher rates of infants born with neonatal opioid withdrawal syndrome (NOWS). Addressing prenatal opioid exposure is a priority for Alaska health systems. The objectives of this study were to: (1) identify maternal and neonatal factors associated with receipt of Medication for opioid use disorder (MOUD) and (2) determine the impact of prenatal MOUD on discharge to parents among infants with NOWS in 3 Alaska hospitals. METHODS: A retrospec

[Pulmonary infections in children. III. Pneumonia due to Chlamydia trachomatis].

The authors show a scientific literary review on Ch. trachomatis pneumonia. A distinctive syndrome pneumonia has been reported in infants infected by Chlamydia (at 2-3 months of age) in genital-urinary infected mothers. The infection may be preceded by conjunctivitis, in apyrexia followed by attacks of coughing pertussis-like. Blood eosinophilia is present. Although favorable prognosis in infancy, erythromycin is the drug of choice shortening the clinical course erythromycin is also recommended in woman infected prevention.

Clinician views on reporting pregnant and birthing patients who use alcohol and/or drugs to child welfare.

BACKGROUND: Multiple health professional associations have expressed concern with policies that require clinician reporting of pregnant people's substance use to child welfare, including that reporting negatively affects patient outcomes and the patient-provider relationship. However, research has shown that clinicians continue to report pregnant and birthing patients at high rates. OBJECTIVE: This study aimed to explore clinician views on reporting pregnant and birthing patients who use alcohol or drugs during pregnancy to child welfare and whether there are patterns in the types of decision
