A Single-Center Review of Renal Replacement Therapy in Patients With Acute Traumatic Brain Injury.

BACKGROUND: Traumatic brain injury (TBI) is a major cause of morbidity, and concomitant acute kidney injury presents substantial management challenges. Continuous renal replacement therapy (CRRT) is thought to cause less hemodynamic instability than intermittent hemodialysis (IHD) after TBI, but factors associated with modality selection in TBI remain unclear. We sought to describe the clinical characteristics and outcomes of patients with TBI requiring renal replacement therapy and to test whether injury severity, Glasgow Coma Scale (GCS), and comorbidity burden were associated with the recei

Fluid removal intensity in intermittent hemodialysis for acute kidney injury: net ultrafiltration rate characterization and outcomes.

In patients receiving continuous renal replacement therapy (CRRT) for acute kidney injury (AKI), excessive net ultrafiltration (NUF) rates >1.75 mL/kg/h have been associated with increased mortality, delayed AKI recovery and complications of hemodynamic instability. There is limited information on fluid removal practices and NUF rates in AKI patients treated with intermittent hemodialysis (IHD). We conducted a retrospective study of AKI patients who underwent IHD at our center between 2020 and 2023. The primary outcome was NUF rate, assessed as body weight-scaled, body surfaces area-scaled and

Net ultrafiltration prescription survey in Europe.

BACKGROUND: Fluid overload is common in patients in the intensive care unit (ICU) and ultrafiltration (UF) is frequently required. There is lack of guidance on optimal UF practice. We aimed to explore patterns of UF practice, barriers to achieving UF targets, and concerns related to UF practice among practitioners working in Europe. METHODS: This was a sub-study of an international open survey with focus on adult intensivists and nephrologists, advanced practice providers, and ICU and dialysis nurses working in Europe. RESULTS: Four hundred eighty-five practitioners (75% intensivists) from 3

Continuous renal replacement therapy in dogs and cats.

Continuous renal replacement therapy (CRRT) is a relatively new extracorporeal blood purification modality that is rapidly gaining acceptance for the treatment of acute kidney injury in cats and dogs. The author has used CRRT for the treatment of leptospirosis, tumor lysis syndrome, heatstroke, pre- and postsurgical support of ureteral obstructions, as well as aminoglycoside and melamine toxicities. As the name implies, CRRT is a slow gradual process, and once treatment begins, patients continue therapy until their renal function returns, they are transitioned to intermittent hemodialysis (IHD

Nutritional therapy in critically ill patients with acute kidney injury on renal replacement therapy: An evidence review.

Protein-energy wasting is common in critically ill patients with acute kidney injury. Renal replacement therapy (RRT) has negative effects on nutrient balance and can further deteriorate nutritional status. The prescribed dialysis modality is a determinant of micronutrient and amino acid losses, with continuous RRT (CRRT) leading to the greatest losses. It is essential to assess and monitor the nutritional status in any RRT modality, implementing conventional nutritional assessment tools (SGA) and technologies for monitoring hydration and body composition (BIA, US). Nutritional therapy should

Management of renal dysfunction in patients with liver cirrhosis: role of pretransplantation hemodialysis and outcomes after liver transplantation.

Patients with end-stage liver disease (ESLD) who develop hepatorenal syndrome (HRS) have very high mortality rates. For patients with HRS type I, median survival without specific therapy is only 2 weeks. Due to worsening clinical condition in such patients secondary to uremia and hepatic disease, some form of renal replacement therapy (RRT), either intermittent hemodialysis IHD or continuous veno-venous hemodialysis (CVVHD), must be instituted. However, the literature regarding the survival benefits of the hemodialysis for the worsening renal failure in liver cirrhotic patients remains limited

[Fomepizole, ethanol or dialysis in the case of life-threatening ethylenglycol intoxication?].

The EXTRIP (EXtracorporeal Treatments In Poisoning) workgroup is a collaborative international effort of pharmacologists, toxicologists, critical care physicians and nephrologists reviewing all available evidence in extracorporeal procedures for the treatment of intoxications in a standardized way to distill treatment recommendations for the physician at the bedside. The second round of guidelines will include recommendations for ethylenglycol intoxication. The case reported here is of a 60-year old man with a body weight of 65 kg who ingested approximately half a bottle (500 mL) of Aral Antif

Dark Urine After Nonfatal Drowning: Rhabdomyolysis-Associated Acute Kidney Injury and Freshwater Aspiration Pneumonia.

Nonfatal drowning is managed primarily as a respiratory emergency, but clinically important kidney injury may be overlooked, particularly with delayed presentation. A previously healthy 53-y-old man fell into a freshwater pond that may have been contaminated by wastewater, struggled for ∼30 to 60 min, aspirated water, and presented 2 to 3 d later with fever, dyspnea, oliguria, and dark brown urine. Respiratory testing identified Aeromonas hydrophila complex and Escherichia coli, supporting drowning-associated pneumonia after freshwater aspiration. Serum creatinine was 986 μmol·L-1 (11.2 mg·dL-

Utilization of Renal Replacement Therapy and Its Impact on the Emergency Department Length of Stay in South Korean Emergency Medical Centers.

Background: The utilization of renal replacement therapy (RRT) is crucial for the management of patients with acute kidney injury (AKI) in emergency departments (EDs). The prompt initiation of RRT, encompassing both intermittent hemodialysis (HD) and continuous renal replacement therapy (CRRT), is acknowledged as beneficial for critically ill patients. The purpose of this study is to investigate the implementation of RRT within EDs and its impact on ED length of stay (EDLOS) in South Korea. Methods: This retrospective study utilized data from the National Emergency Department Information Syste

Long-term followup of cadaveric renal transplantation in patients with spina bifida.

PURPOSE: Access related problems in hemodialysis and peritoneal dialysis are increased in cases of spina bifida due to vascular and body habitus limitations. Reports of renal transplantation in this patient group are exceedingly rare. We report long-term followup data on cadaveric renal transplantation for end stage renal failure in cases of spina bifida. MATERIALS AND METHODS: Between February 1986 and April 2000, 17 cadaveric renal transplants were performed in 11 females and 5 males with spina bifida. Mean age at transplantation was 20.2 years (range 10 to 35). Of the patients 11 were whee

Association of Hepatorenal Syndrome-Acute Kidney Injury with Mortality in Patients with Cirrhosis Requiring Renal Replacement Therapy: Results from the HRS-HARMONY Consortium.

KEY POINTS: In patients with cirrhosis and AKI requiring renal replacement therapy (RRT), hepatorenal syndrome-AKI was not associated with an increased 90-day mortality when compared with other AKI etiologies. Etiology of AKI may not be a critical factor regarding decisions to trial RRT in acutely ill patients with cirrhosis and AKI. Although elevated, mortality rates in this study are comparable with those reported in general hospitalized patients with AKI requiring RRT. BACKGROUND: While AKI requiring renal replacement therapy (AKI-RRT) is associated with increased mortality in heterogeneou

The impact of continuous renal replacement therapy on drug therapy.

Critically ill patients with multisystem organ failure often require daily administration of large volumes of fluid to provide electrolyte and nutrition support, medications, and blood products. This often results in fluid overload, which has historically been managed with intermittent hemodialysis (IHD). Unfortunately, IHD entails a high rate of fluid and solute removal that often exacerbates hemodynamic instability. Accordingly, continuous renal replacement therapy (CRRT), involving slow and continuous removal of water and solutes from the plasma, is currently preferred for managing these pa

Metformin-associated lactic acidosis (MALA): Is it an underestimated entity? A retrospective, single-center case series.

INTRODUCTION: Metformin is widely considered a first-line antiglycemic agent due to its cost-effectiveness and favorable adverse effect profile. However, its use is prohibited in patients with an estimated glomerular filtration rate <30 mL/min/1.73 m2 , due to the risk of potentially lethal metformin-associated lactic acidosis (MALA). We sought to evaluate MALA cases and outcomes at our institution. METHODS: In this observational, retrospective case series, we reviewed our EMR for all patients who had a metformin level drawn between January 2013 and May 2022 to identify individuals who met th

Vasodilator use in precapillary pulmonary hypertension with end stage kidney disease: A single center experience.

BACKGROUND AND OBJECTIVES: Pulmonary hypertension is commonly seen in end stage kidney disease and is most commonly due to elevated left heart pressures. There is limited data about vasodilator use during the management of Group 1 pulmonary arterial hypertension in the context of those who also have or later developed end stage kidney disease. The objective of this study was to determine safety and efficacy of vasodilator therapy in precapillary pulmonary hypertension requiring renal replacement therapy. DESIGN: This was a single-center retrospective case series. Patients were identified from

Aminoglycoside impregnated cement spacer precipitating acute kidney injury requiring hemodialysis.

The current standard of care for prosthetic joint infection includes two-stage arthroplasty, with antibiotic-impregnated cement spacers (ACS) utilized between the stages. We report a 75-year-old woman with previously normal renal function, who developed acute kidney injury (AKI) secondary to biopsy-proven acute tubular necrosis and acute interstitial nephritis after ACS placement containing tobramycin and vancomycin. Peak tobramycin level measured 25.3 mcg/mL, the highest value reported in the literature after ACS placement. Intermittent hemodialysis was initiated with subsequent full recovery

Kidney Replacement Therapies and Ultrafiltration in Cardiorenal Syndrome.

BACKGROUND: Some patients with cardiorenal syndrome 1 and congestion exhibit resistance to diuretics. This scenario complicates management and is associated with a worse prognosis. In some cases, rescue treatment may be considered by starting kidney replacement therapies or ultrafiltration. This decision is complex and necessitates a profound understanding of these techniques and the pathophysiology of this syndrome. These modalities are classified into continuous, intermittent, and ultrafiltration therapies, each with its own advantages and disadvantages that are pertinent in selecting the op

Transatrial repair of a giant left ventricular pseudoaneurysm and ischaemic mitral regurgitation after myocardial infarction: case report.

BACKGROUND: Left ventricular pseudoaneurysm (LVPA) is an infrequent but highly lethal complication of myocardial infarction. Early surgical repair with a resection of pseudoaneurysm is often performed, given that medical therapy alone is associated with a high risk of mortality. This report describes a case of a giant LVPA on the lateral wall post-infarction and mitral valve regurgitation that was successfully treated by surgical transatrial closure and mitral valve replacement. CASE SUMMARY: A 77-year-old man with chronic kidney disease and a history of percutaneous coronary interventions fo

Successful treatment of metformin and rodenticide intoxication with prolonged hemodialysis and methylene blue: A case report.

Metformin is a first-line treatment for diabetes mellitus. In cases of suicidal overdose or chronic accumulation, lactic acidosis may develop and, if untreated, can be fatal. Renal replacement therapy plays a key role in reversing the condition. Refractory hypotension may be encountered both in the presentation of intoxication cases and as a complication of renal replacement therapy. Methylene blue is recommended to improve blood pressure in such cases. A 25-year-old male presented to the emergency department after ingesting a high dose of metformin and around 20 g of rodenticide in a suicide

White Tongue Because of Uremic Stomatitis as a Sign of Advanced Kidney Disease.

We present a case of a previously healthy adolescent male who initially presented to his primary care physician with the chief complaint of a "large and white tongue," who subsequently was diagnosed with end-stage kidney disease (ESKD) and associated uremic stomatitis. This patient required admission to a PICU for acute renal replacement therapy with intermittent hemodialysis, and his hospital course was complicated by generalized tonic-clonic seizures. ESKD is difficult to diagnose in the pediatric population because these patients are often asymptomatic in the early stages given the insidiou
