Levels of HCG in subjects using the Progestasert(TM) intrauterine progesterone systems.

Twelve subjects using the intrauterine progesterone system were followed during 4 consecutive menstrual cycles for FSH, LH, hCG and progesterone levels. They were all multiparous, with documented ovulation and sexually active even during the ovulatory period. With the exception of one case which became pregnant, there was no evidence for the presence of hCG during the period of observation. This data, even if preliminary, suggest that the Progestasert(TM) exerts a contraceptive action which differs from that of other devices, whether medicated or not, and that, such action occurs at a stage ea

Fixation techniques for the Levonorgestrel-Releasing Intrauterine system in adenomyosis: a narrative review.

BACKGROUND: Adenomyosis causes severe dysmenorrhoea and menorrhagia. While the Levonorgestrel-Releasing Intrauterine System (LNG-IUS) is a highly effective conservative treatment, its success is frequently compromised by high expulsion rates (15%-37.5%) caused by uterine enlargement and cavity distortion. This review evaluates the principles, clinical efficacy, and safety of emerging intrauterine fixation techniques designed to prevent LNG-IUS downward displacement and expulsion. METHODS: A comprehensive literature search was conducted across PubMed, Web of Science, and Google Scholar for art

Health-related quality of life and satisfaction associated with 3-year use of a levonorgestrel 13.5 mg intrauterine device vs Nova T copper 380 mm2 intrauterine device: Results of a phase 4 randomized controlled trial.

OBJECTIVES: This study aimed to investigate the impact of levonorgestrel 13.5 mg and Nova T copper 380 mm2 intrauterine devices (LNG13.5-IUD and Cu380-IUD, respectively) on health-related quality of life (HRQoL) and the satisfaction with the method throughout 3 years of use. STUDY DESIGN: We conducted a single-center, evaluator-masked, randomized controlled trial to compare the bleeding profile of LNG13.5-IUD and Cu380-IUD users. Secondary objectives included HRQoL and satisfaction throughout the study. We used the validated questionnaire of the Spanish Society of Contraception (SEC-QoL), whi

Use of Intrauterine Devices and Risk of Human Immunodeficiency Virus Acquisition Among Insured Women in the United States.

Concerns have been raised about progestin-containing contraceptives and the risk of human immunodeficiency virus (HIV) acquisition. Based on health insurance data from women in the United States with intrauterine device (IUD) insertions during 2011-2018, there was no increased risk of incident HIV diagnosis for levonorgestrel-releasing IUDs versus copper IUDs.

The intrauterine contraceptive device: an often-forgotten and maligned method of contraception.

Although 90% of women at risk for unintended pregnancy in the United States use contraception, <1% of these women use the intrauterine contraceptive device. The mechanism of action of intrauterine contraceptive devices has been controversial, but several studies suggest that interference with sperm migration or function and with fertilization may be the most likely mechanisms. More important, there is lack of compelling evidence that the intrauterine contraceptive device acts as an abortifacient. The risk for pelvic inflammatory disease among users now appears to be extremely low, primarily as

A comparative study of the effect of the Progestasert TM and Gravigard IUDs on dysmenorrhoea.

Data obtained from questionnaires on the occurrence of dysmenorrhoea in women using IUDs inserted randomly (146 of them Progestasert and 149 Copper 7) are analyzed. In both groups a reduction in the intensity of menstrual cramps was observed over 12 months of use. Although these reductions are significant in each group, they are slightly higher in Progestasert users. However, on comparing the reduction tendencies of both groups, the difference between them is not significant. No significant differences become apparent either when comparing premenstrual and intermenstrual camps. The usefulness

Mechanism of action of intrauterine devices: biochemical changes.

The mechanism of action of intrauterine devices varies from one type of device to another, although the principle of action is the same, namely, to interfere with the physiology of reproduction at the endometrial level. The biochemical changes of the endometrial tissue and the composition of the uterine fluid have been reviewed in relation to the use of inert, copper-medicated and steroid-releasing IUDs. All IUDs, whether inert or medicated, provoke a significant increase in the number of neutrophils, mononuclear cells and plasma cells, the presence of which is unrelated to the tissue infiltra

Laparoscopic retrieval of a levonorgestrel intrauterine device in a patient presenting with abdominal pain.

Uterine perforation is a potentially serious uncommon complication of intrauterine device (IUD) use. It has an incidence of one in 1000 insertions. It can manifest with abnormal uterine bleeding and pain. The most common site of implantation is the omentum. Risk factors include breastfeeding, <6 months postpartum, lack of experience of the healthcare professional, congenital uterine anomalies and nulliparity. Transvaginal ultrasound scan (USS) is accurate in localisation of the Mirena IUD. If the IUD is not evident within the uterus, an abdominal radiograph should be performed. Laparoscopic su

Background parenchymal enhancement in contrast-enhanced MR imaging suggests systemic effects of intrauterine contraceptive devices.

OBJECTIVES: Levonorgestrel-releasing intrauterine contraceptive devices (LNG-IUDs) are designed to exhibit only local hormonal effects. There is an ongoing debate on whether LNG-IUDs can have side effects similar to systemic hormonal medication. Benign background parenchymal enhancement (BPE) in dynamic contrast-enhanced (DCE) MRI has been established as a sensitive marker of hormonal stimulation of the breast. We investigated the association between LNG-IUD use and BPE in breast MRI to further explore possible systemic effects of LNG-IUDs. METHODS: Our hospital database was searched to ident

[Intravascular migration of a contraceptive implant].

BACKGROUND: The Implanon NXT is a commonly used contraceptive. Incorrect localization of the implant can cause complications. CASE DESCRIPTION: A 41-year-old woman is seen in the gynaecology outpatient clinic with a request to remove a recently placed Implanon NXT because of worsening mood symptoms. The implant can't be found on physical and ultrasound examination. Duringsurgicalexplorationthe implant is not found at theinsertion site' By means of X-ray scanning the implant becomes visible around the humeral head. The implant appears to be located in the cephalic vein and is subsequently remo

Ex-vivo forces associated with intrauterine device placement and perforation: a biomechanical evaluation of hysterectomy specimens.

BACKGROUND: This biomechanical analysis of hysterectomy specimens assesses the forces associated with intrauterine device placement. These include compressive forces required to cause uterine perforation with two commonly available commercial intrauterine device placement instruments and a metal uterine sound. METHODS: We obtained hysterectomy specimens at a single academic center. All specimens resulted from excision for benign conditions in premenopausal women by any operative method. Within one hour of excision, we stabilized uterine specimens in an apparatus specifically designed for this

Transmural migration and perforation of a levonorgestrel intrauterine system: a case report and review of the literature.

INTRODUCTION: Uterine perforation is an uncommon yet well-known complication of copper intrauterine devices and the levonorgestrel intrauterine system (IUS). While initial extrauterine placement at the time of insertion is felt to be the cause of perforation in most cases, some hypothesize that delayed transmural migration and subsequent perforation can occur with slightly malpositioned or even properly placed devices. CASE: A 46-year-old female had a 52-mg levonorgestrel IUS inserted for menstrual management and contraception. We arranged a follow-up ultrasound as the uterus was enlarged on
