The arterial blood supply of the arm is unique, as the collateral circulation is especially good. Because of this anatomic perculiarity it is possible to deal with lack of peripheral pulse without critical ischemia occurring in fractures in the elbow region. We present two children with supracondylar fractures of the humerus with damage to the brachial artery and the different management results. It is necessary to carry out the immediate diagnostic and therapeutic procedures in order to prevent late complications. We conclude that Volkmann's ischemic contracture can always be prevented.

We report the case of a young athlete who sustained an inversion injury to his ankle and presented several hours later with leg pain out of proportion to the injury. Pressure measurements suggested a lateral compartment syndrome and he underwent an emergency fasciotomy. The case is described and recommendations for diagnosis and treatment are given. Angiotensin II (A-II) is known to potentiate ischemic dysfunction during ischemia, but the mechanisms involved are not completely established. We examined the effects of A-II on intracellular calcium concentration ([Ca++]i) and cell contracture caused by metabolic inhibition in isolated adult rabbit ventricular myocytes. [Ca++]i was assessed by flow cytometry, using the Ca(++)-sensitive fluorescent probe, fluo-3. After 90 min of exposure to 2 mM cyanide (CN) and 0 glucose, there was a significant increase in myocyte [Ca++]i. This increase was slightly augmented in the presence of 100 nM A-II.

Ischemic necrosis of the intrinsic muscles of the hand has been known about for decades, as has the resulting local ischemic contracture in the hand.
