Clinical features of cutaneous T-cell lymphoma.

The distinctive clinical features and natural history of mycosis fungoides, an epidermotropic variant of cutaneous T-cell lymphoma, are presented. These findings are compared with certain non-mycosis fungoides T-cell lymphomas that occasionally occur in the skin. Clinical staging and evaluation of patients with cutaneous lesions including plaques, tumors, or erythroderma, with or without nodal involvement and disseminated disease, are considered. Associated disorders such as follicular mucinosis, actinic reticuloid, lymphomatoid papulosis, and secondary primary malignancies are also presented.

Second malignancies in cutaneous T cell lymphoma.

Among sixty-three cutaneous T cell lymphoma (CTCL) patients seen over a 15-year period at the Duke University Medical Center (DUMC), 15.9% had a second malignancy. This is a higher frequency of second malignancies than has been documented previously in any series of CTCL patients. We evaluated the incidence of subsequent primary malignancies in these CTCL patients by comparing our series of patients to a general population. The overall cancer incidence rate in the CTCL patients was 2.4 times, and in white male patients 3.3 times, greater than expected. We also compared various characteristics

Clinical staging for cutaneous T-cell lymphoma.

The Mycosis Fungoides Cooperative Group has been following patients with cutaneous T-cell lymphoma, including mycosis fungoides and the Sézary syndrome variant. Previous analyses identified the extent of skin involvement and the number of sites of clinically enlarged lymph nodes as important prognostic variables. These two variables were used to classify 340 patients into four clinical stages. Repeat analysis based on additional followup data shows the usefulness of this clinical staging system for identifying patients with differing survival experience. An alternative grouping suggested by fi

Decreased T-cell receptor excision circles in cutaneous T-cell lymphoma.

PURPOSE: The T cell repertoire in patients with advanced cutaneous T cell lymphoma (CTCL) is significantly contracted despite the presence of relatively normal absolute numbers of T cells. We propose that many normal T cells were being lost in patients with CTCL, with the remaining normal T cells expanding clonally to fill the T cell compartment. T-cell receptor excision circles (TREC) form as a result of the initial gene rearrangement in naïve T cells. Although they are stable, they do not replicate and are subsequently diluted with the expansion of a population of T cells. Their concentratio

[What's new about cutaneous T-cell lymphoma?].

Cutaneous T-cell lymphomas (CTCL) are rare skin-predominant lymphomas, with Mycosis fungoides being the most common subtype. In Europe, the incidence is about 0.3 per 100 000 patient years. The majority of patients are suffering from limited disease with a 5-year survival of 90 %. Clinically, skin lesions can mimic eczema or psoriasis, leading to delayed diagnosis. CTCL diagnosis requires a combination of clinical examination, skin biopsy with histopathological analysis and blood tests to assess systemic involvement. CTCL treatment is stage-dependent. Depending on the stage of the disease, loc

Vitamin D in Cutaneous T-Cell Lymphoma.

Cutaneous T-cell lymphoma (CTCL) is characterized by the proliferation of malignant T cells in inflamed skin lesions. Mycosis fungoides (MF)-the most common variant of CTCL-often presents with skin lesions around the abdomen and buttocks ("bathing suit" distribution), i.e., in skin areas devoid of sun-induced vitamin D. For decades, sunlight and vitamin D have been connected to CTCL. Thus, vitamin D induces apoptosis and inhibits the expression of cytokines in malignant T cells. Furthermore, CTCL patients often display vitamin D deficiency, whereas phototherapy induces vitamin D and has benefi

Hemophagocytic Lymphohistiocytosis in Cutaneous T-Cell Lymphoma.

IMPORTANCE: Hemophagocytic lymphohistiocytosis (HLH) has been reported as a serious complication of cutaneous T-cell lymphoma (CTCL). Despite available diagnostic guidelines, it remains a diagnostic and therapeutic challenge in this patient population. OBJECTIVES: To examine the characteristics of CTCL associated with HLH and analyze the presenting signs and symptoms, therapeutic options, and outcome. DESIGN, SETTING, AND PARTICIPANTS: In this case series, patients diagnosed with CTCL and HLH who were treated at a single institution from January 1, 2014, through December 31, 2017, were studi

Oral involvement of cutaneous T-cell lymphoma.

Methylisothiazolinone (MI) is commonly used as a preservative in personal care products and is a frequent cause of allergic contact dermatitis. We present a patient with allergic contact dermatitis caused by MI in hair care products and discuss this allergen to bring attention to this common cause of contact dermatitis, and to highlight its frequent use in hair care products. If allergy to MI is suspected, testing should be performed to this individual preservative, as testing solely for the combination preservative, methylisothiazolinone/methylchloroisothiazolinone (Kathon CG®), may miss many

Primary Cutaneous Acral CD8+ T-Cell Lymphoma.

Primary cutaneous acral CD8+ T-cell lymphoma is a new provisional entity in the 2016 revision of the World Health Organization classification of lymphoid neoplasms. This is a challenging diagnosis because of its rarity, as well as its morphologic and immunophenotypic overlap with other CD8+ cytotoxic lymphoid proliferations. Appropriate classification of this entity is crucial because of its indolent clinical behavior compared with other CD8+ T-cell lymphomas. Knowledge of the clinical setting, sites of involvement, and morphologic features can aid in correct diagnosis. Here, we review the cli

[CUTANEOUS T CELL LYMPHOMA: UNILESIONAL MYCOSIS FUNGOIDES].

Unilesional mycosis fungoides is a rare cutaneous T cell lymphoma that warrants either radiation therapy or surgical excision. Benign characteristics result in misdiagnosis, delayed tissue biopsy and subsequently delayed provision of adequate treatment. A young patient presented with a history of 18 months of eczematous benign - appearing single lesion restricted to her index finger. Local electron-beam radiation following tissue diagnosis resulted in full recovery.

Primary cutaneous follicular helper T-cell lymphoma.

Follicular helper T-cells (TFH) represent a specific subset of CD4-positive helper T-cells that help B-cells to differentiate into long-lived antibody-secreting plasma cells or memory B-cells. The expression of TFH markers in neoplastic T-cells, traditionally related to the angioimmunoblastic (AITL) subgroup of peripheral T-cell lymphomas, is nowadays well-known to be more widespread than previously thought. We report hereby a case of cutaneous T-cell lymphoma in a 75-year-old woman, whose morphological and immunophenotypical features raises the differential diagnosis between cutaneous involve

Primary cutaneous CD20-positive T-cell lymphoma.

CD20 is a transmembrane protein that is expressed by B cells during their development and is, therefore, commonly used to label cells of B lineage in lymphoid infiltrates. CD20-positive T-cell lymphoma is infrequent but well recognized. Cases reported in the literature show a variety of clinical and histoimmunochemical profiles. Primary cutaneous CD20-positive T-cell lymphoma is vanishingly rare; only eight cases have been previously reported. We present two new cases of this entity and describe their clinical, histological and immunohistochemical features. CD20 is a highly specific B-cell mar

Evaluation of cutaneous angioimmunoblastic T-cell lymphoma.

BACKGROUND: Angioimmunoblastic T-cell lymphoma (AITL) accounts for 18% of peripheral T-cell lymphomas worldwide. Skin involvement occurs in up to 50% of patients but poses a diagnostic dilemma because of the limited number of reported cases and subsequent lack of established diagnostic criteria. OBJECTIVE: The purpose of this review is to examine common clinical, histologic, and molecular findings in cases of AITL with the hope of improving the diagnostic accuracy of this challenging condition. METHODS: We present a case of AITL and conducted a review of the literature. RESULTS: The common

Cutaneous involvement in angioimmunoblastic T-cell lymphoma.

Angioimmunoblastic T-cell lymphoma (AITL) is an aggressive non-Hodgkin's nodal peripheral T-cell lymphoma characterized by general lymphadenopathy, night sweats, fever, hepatosplenomegaly, polyclonal hypergammaglobulinemia, and cutaneous involvement. We present a rare case of AITL cutaneous involvement mimicking toxic erythema recurring with AITL relapse and suggesting a precursor of disease progression.

Emerging drugs in cutaneous T cell lymphoma.

BACKGROUND: Mycosis fungoides (MF) represents the most common type of primary cutaneous T cell-lymphomas (CTCL), which are characterized by a clonally proliferation of malignant CD4+ lymphocytes in the skin. OBJECTIVE: Skin-directed treatment regimens, like phototherapy and corticosteroids, are commonly used in early stages; systemic treatments and chemotherapies are used in advanced stages. Because conventional treatments usually end in a transient remission without curative results, there is a high need for new therapeutic strategies with acceptable side effects. METHODS: Literature and re

Treatment planning in cutaneous T-cell lymphoma.

Effective long-term management of cutaneous T-cell lymphoma (CTCL) requires administration of skin-directed therapies such as topically applied nitrogen mustard or photochemotherapy to achieve a complete response in clinically early disease (patch and thin-plaque-phase mycosis fungoides, MF) and often the concomitant administration of well-tolerated drugs with systemic effects such as interferon alfa, bexarotene, methotrexate or extracorporeal photopheresis in more advanced, but not highly aggressive/nontransformed disease (thick plaque or tumor phase MF or erythrodermic CTCL).
