Bone remodeling following mandibular reconstruction using fibula free flap.

To investigate bone remodelling responses to mandibulectomy, a joint external and internal remodelling algorithm is developed here by incorporating patient-specific longitudinal data. The primary aim of this study is to simulate bone remodelling activity in the conjunction region with a fibula free flap (FFF) reconstruction by correlating with a 28-month clinical follow-up. The secondary goal of this study is to compare the long-term outcomes of different designs of fixation plate with specific screw positioning. The results indicated that the overall bone density decreased over time, except f

Malocclusion Management Following Mandibular Reconstruction With Free Fibula Flaps.

STUDY DESIGN: Free fibula flaps are nowadays the gold standard for the surgical reconstruction on large mandibular defects. Malocclusion is an important complication of this type of reconstruction and many of these patients end up requiring subsequent orthognathic corrective surgery. This is a descriptive retrospective case series study. OBJECTIVE: To describe the demographic data, operative techniques, corrective methods and postoperative results in the management of malocclusion following mandibular reconstruction with free fibula flap. METHODS: This case series study included patients who

Use of porous space maintainers in staged mandibular reconstruction.

The success of mandibular reconstructions depends not only on restoring the form and function of lost bone but also on the preservation of the overlying soft tissue layer. In this case study, 5 porous polymethylmethacrylate space maintainers fabricated via patient-specific molds were implanted initially to maintain the vitality of the overlying oral mucosa during staged mandibular reconstructions. Three of the 5 patients healed well; the other 2 patients developed dehiscences, likely due to a thin layer of soft tissue overlying the implant. The results presented provide evidence that a larger

Bilateral bifid mandibular condyles diagnosed with three-dimensional reconstruction.

Bifid mandibular condyles (BMCs) are rare anomalies. The overwhelming majority of prior reports described their predominantly unilateral occurrence diagnosed by panoramic radiography. We present an even rarer case of bilateral BMC initially identified by panoramic radiography and confirmed with colour-enhanced three-dimensional CT. These images substantiate the theory that the secondary condyles arise from the neck of the mandible (Lopez-Lopez et al. Bifid condyle: review of the literature of the last 10 years and report of two cases. Cianio 2010; 28: 136-140).

Nonvascularized bone grafting for mandibular reconstruction: myth or reality?

This retrospective study analyzes the long-term results and complications of mandibular reconstruction with nonvascularized bone grafts (NVBGs) and suggests refinements. A total of 166 patients underwent mandibular ablation and primary reconstruction between June 1990 and August 2009, of which 101 were NVBGs, 27 were vascularized bone grafts, and 38 were with functional plate reconstruction. Eighty-seven of 101 patients undergoing NVBG, having all records and followed up for a minimum of 12 months, were included in this study. The analysis criteria included site of defect, surgical approach, m

Reconstruction of the mandibular condyle using transport distraction osteogenesis.

Transport distraction osteogenesis was used to reconstruct the articulation of the temporomandibular joint in two patients. Patient 1 underwent a gap arthroplasty for release of a bony ankylosis. Patient 2 had degenerative joint disease after tumor resection. Both patients had painful, limited mouth opening and facial asymmetry as a result of condylar shortening. A reverse-L osteotomy was performed from the sigmoid notch to the posterior border of the mandible. An external transport distraction device was applied, and the segment was advanced superiorly, 1.0 mm per day, until contact was made

The serratus anterior/rib composite flap in mandibular reconstruction.

A flap comprising the serratus anterior muscle and the underlying rib, based on the serratus branches of the thoracodorsal artery has been developed. In addition to the rib the potential exists to include a cartilaginous growth centre, a functional muscle and/or overlying skin. A latissimus dorsi musculocutaneous unit can be included on a common pedicle. This paper describes its use as both a free and a pedicled composite unit in mandibular reconstruction. The surgical anatomy of the flap is reviewed and cases are shown to illustrate its use in the treatment of congenital, malignant and post-t

Comparison of reconstruction plates and miniplates in mandibular defect reconstruction with free iliac flap.

OBJECTIVE: Given the increasing demand for precise and reliable reconstructive techniques in maxillofacial surgery, we try to offer valuable insights for clinicians in selecting optimal fixation methods. MATERIALS AND METHODS: Patients were categorized into miniplate and reconstruction plate groups for accuracy and bone healing comparison. We measured gonial angle, intercondylar, intergonial and anterior-posterior distance for general accuracy and distance of segmental endpoint to the sagittal plane for partial accuracy. The bone healing rate of the two groups was compared with CT images at 3

Mandibular reconstruction outcomes for in-house patient-specific solutions.

BACKGROUND: Segmental mandibulectomy and mandibular reconstruction are often performed for various benign and malignant head and neck conditions. Standard of care reconstruction involves titanium plate fixation with tissue transfer. The advent of computer-aided design and manufacturing (CAD/CAM) has enhanced aesthetic and functional outcomes in mandibular reconstruction by enabling patient-specific solutions like 3D-printed anatomic models. At an increasing number of institutions, these solutions can be produced in-house via point-of-care manufacturing. Since little has been published on the a

Biomechanical assessment in virtual surgical planning for mandibular reconstruction.

The fibula free flap has been widely used for reconstructing large segmental bone defects in the maxillofacial region. While mandibular reconstruction requires a high degree of precision, the impact of surgical deviations on clinical outcomes remains poorly understood. This study addressed geometric inaccuracies during surgery and examined the effects of loading patterns on the biomechanical behavior of reconstructed mandibles. Patient-specific finite element (FE) models were developed using CT data from three representative cases - two with Brown Class II defects and one with a Class III defe

Double Arterialized Scapular Tip Free Flap for Mandibular Reconstruction.

INTRODUCTION: Scapular tip free flap (STFF) has become today one of the workhorse flaps for maxillary reconstruction; recently, the possibility of extending the vascular supply by adding to the angular branch of the circumflex pedicle up to its periosteal entrance in the lateral border of the scapula has been proposed as a reliable technique to improve the length of perfused bone when STFF is used for mandibular reconstruction. The purpose of this study was to evaluate the patients who had received microvascular reconstruction of the mandible with STFF vascularized by both the circumflex scapu

Manometric assessment of pharyngeal swallowing pressure after mandibular reconstruction.

Oral cancer patients who undergo mandibular bone partial resection often exhibit swallowing disturbance after surgery. We examined the usefulness of manometry in obtaining quantitative data on swallowing function in patients after mandibular bone partial resection. Postsurgical swallowing function was investigated, using a combination of videofluorography (VF) and manometry, in five patients with oral cancer, who underwent mandibular bone partial resection. Three patients underwent reconstruction of soft tissue defects only, while the remaining two patients underwent reconstruction of both sof

Mandibular reconstruction using free bone flap after preoperative chemoradiation.

When treating head and neck cancer of an advanced stage, additional therapy modalities are often combined with surgery. This sets new challenges for the reconstructive surgery, especially after segmental mandibulectomy. There is continuous discussion considering the optimal timing of the surgery with relation to other treatment methods such as radiation therapy and chemotherapy. In this work, we have analyzed a series of 10 patients treated with segmental mandibulectomy and preoperative irradiation or chemoradiation in our institute between 1999 and 2006. Surgery was scheduled within 5 weeks f
