Systematic review on biomechanical effects of high-velocity, low amplitude spinal manipulation.

BACKGROUND: Spinal manipulative therapy uses high-velocity low-amplitude (HVLA) thrusts which are clinically effective, but underlying mechanisms are still unknown. OBJECTIVE: To summarize the evidence for biomechanical effects of HVLA thrusts in asymptomatic and symptomatic humans as well as for a possible link between biomechanical effects and clinical effectiveness. STUDY DESIGN: Systematic review of randomized controlled trials. METHODS: An information specialist conducted systematic literature searches in six databases [Medline (OvidSP), Premedline (PubMed), CINAHL, EMBASE, Cochrane, a

The association between cervical artery dissection and spinal manipulation among US adults.

PURPOSE: Cervical artery dissection (CeAD), which includes both vertebral artery dissection (VAD) and carotid artery dissection (CAD), is the most serious safety concern associated with cervical spinal manipulation (CSM). We evaluated the association between CSM and CeAD among US adults. METHODS: Through analysis of health claims data, we employed a case-control study with matched controls, a case-control design in which controls were diagnosed with ischemic stroke, and a case-crossover design in which recent exposures were compared to exposures in the same case that occurred 6-7 months earli

Taming of the Testicular Pain Complicating Lumbar Disc Herniation With Spinal Manipulation.

Degenerative disease of the lumbar spine is often ignored as a potential cause of testicular pain because the exact link between the two remains uncertain. This article reports the case of a 60-year-old man with a 3-year history of low back pain and unexplained right testicular pain for 2 years. Painful symptoms were negatively affecting his social, physical, and sexual functions. After failure to achieve pain relief through multiple types of therapy, the patient sought chiropractic treatment for his condition. Lumbar spine magnetic resonance imaging (MRI) revealed disc protrusion at the L1/L2

Spinal manipulation and therapeutic exercises in treating post-surgical resurgent lumbar radiculopathy.

Lumbar radiculopathy due to disc herniation is one of the most common conditions seen in orthopedic practices worldwide. Surgical intervention is often necessary, and most patients improve without reoccurrences. The purpose of this case is to describe the successful outcome of post-surgical resurgent lumbar radiculopathy treated with spinal manipulation and therapeutic exercises. Here, we discuss the case of a 42-year-old male who received twelve treatments of spinal manipulation, in addition to therapeutic McKenzie methods exercises. By the end of the treatment plan, the patient was asymptoma

Full-endoscopic interlaminar removal of chronic lumbar epidural hematoma after spinal manipulation.

A patient with mild cervical myelopathy due to multilevel ossification of the posterior longitudinal ligament (OPLL) initially underwent a cervical C3-T1 laminectomy with C2-T2 fusion utilizing lateral mass screws. The patient's new postoperative right upper extremity paresis largely resolved within several postoperative months. However, approximately 6 months later, the patient developed increased paraparesis attributed to thoracic OPLL and Ossification of the yellow ligament (OYL) at the T2-T5 and T10-T11 levels. The patient underwent simultaneous minimally invasive (MIS) unilateral MetRx ap

Comparison of force development strategies of spinal manipulation used for thoracic pain.

INTRODUCTION: To date, there is a paucity of comparative analysis of manual therapeutic procedures for the treatment of pain in the same spinal region. This paper examines the cross correlation of force-time histories across three distinct strategies of force production for the same thoracic spine procedure. Secondary analysis includes examination of the characteristics that account for potential differences. METHODS: A homogeneous sample of 21 male volunteers and three clinicians were selected as a convenience sample. The force-time histories were recorded using a table mounted force plate (

Altered sensory behaviors in mice following manipulation of endogenous spinal adenosine neurotransmission.

Adenosine or adenosine analogs injected intrathecally (i.t.) induce significant antinociception. Recent studies support the existence of an endogenous spinal system that can modulate nociceptive input by releasing adenosine. Inhibition of adenosine metabolism by administration of an adenosine kinase inhibitor, in the present study, decreased behavior induced by putative pain neurotransmitters providing additional support for an endogenous purinergic system. Conversely, administration of high doses of methylxanthines (i.t.), adenosine receptor antagonists, induced behavior similar to that induc

Spinal Manipulation for Subacute and Chronic Lumbar Radiculopathy: A Randomized Controlled Trial.

OBJECTIVE: We evaluated the efficacy of spinal manipulation for the management of nonacute lumbar radiculopathy. METHODS: In a university hospital we performed a randomized controlled trial with 2 parallel arms. Patients (n = 44) with unilateral radicular low back pain lasting more than 4 weeks were randomly allocated to manipulation and control groups. The primary outcome was the intensity of the low back pain on a visual analog scale. The secondary outcome was the Oswestry Disability Questionnaire score. We also measured spinal ranges of motion. The assessments were carried out at the basel

The quality of reports on cervical arterial dissection following cervical spinal manipulation.

BACKGROUND: Cervical artery dissection (CAD) and stroke are serious harms that are sometimes associated with cervical spinal manipulation therapy (cSMT). Because of the relative rarity of these adverse events, studying them prospectively is challenging. As a result, systematic review of reports describing these events offers an important opportunity to better understand the relation between adverse events and cSMT. Of note, the quality of the case report literature in this area has not yet been assessed. PURPOSE: 1) To systematically collect and synthesize available reports of CAD that have b

Effects of thoracic spinal manipulation in long COVID patients: A randomised controlled trial.

INTRODUCTION: Long COVID patients manifest dyspnoea, chest heaviness, and symptoms, ascertaining deteriorated lung function. As thoracic spinal manipulation in such patients may lead to spinal misalignment, the chest wall compliance may also get affected. This study aimed to determine the role of thoracic spinal manipulation on pulmonary function, thoracic spine mobility, and chest expansion in patients with Long COVID. METHODS: A Randomised Controlled trial was done in Dr. D.Y. Patil College of Physiotherapy, Pune, Maharashtra on 42 individuals who fulfilled the inclusion criteria. They were

Monocular visual loss after closed head trauma: immediate resolution associated with spinal manipulation.

OBJECTIVE: To discuss the case of a patient who demonstrated that spinal injuries may cause both cortical and ocular visual loss that was ameliorated by manipulative care. CLINICAL FEATURES: The patient suffered separate incidents of binocular and monocular loss of vision. A female child, aged 9 yr, presented with bilateral concentric narrowing of the visual fields that returned to normal immediately after spinal treatment. Approximately 1 yr later, she returned with monocular loss of vision after she was struck on the head by a ball. INTERVENTION AND OUTCOME: The child was treated by spinal

Remission of Dowager's hump by manipulative correction of spinal alignment: a case report.

Dowager's hump is described as excessive kyphotic curvature in the thoracic spine with a Cobb angle of more than 40 degrees. This case report presents a 61 years old female office clerk who experienced headaches and neck pain for 3 years that extended into her right shoulder and upper chest. She consulted her primary care physician two months before seeing the chiropractor when the neck pain worsened. A diagnosis of cervicalgia related to osteoarthritis was made based on cervical and thoracic X-ray findings. The patient received non-steroid anti-inflammatory drugs (celecoxib and etoricoxib) an

Physicians using spinal manipulative treatment in The Netherlands: a description of their characteristics and their patients.

BACKGROUND: Various health care professionals apply Spinal Manipulative Treatment (SMT) in daily practice. While the characteristics of chiropractors and manual therapists and the characteristics of their patient populations are well described, there is little research about physicians who use SMT techniques. A distinct group of physicians in The Netherlands has been trained in musculoskeletal (MSK) medicine, which includes the use of SMT. Our objective was to describe the characteristics of these physicians and their patient population. METHODS: All registered MSK physicians were approached
