In several reports, the functional results of mastoid surgery in children have been stated to be disappointing. In this study, a series of 49 child patients who underwent mastoid surgery for chronic suppurative otitis media were examined 10 years postoperatively. At surgery, a cholesteatoma was detected in 49% of the patients. During the observation period, 33% of the patients were subjected to a second operation. At the last follow-up examination, 57% of the operated ears showed hearing levels of 20 dB or better, but in 16% hearing levels were worse than 40 dB. In 16% of the patients, contral

I have reviewed a group of otologic patients who represent controversial problems in management. Decisions for revisional otologic surgery must be based on each patient's individual findings. Statistical reviews are of little help in deciding which patient will benefit. Physicians managing patients with poor results after surgery for hearing, infection, or vertigo must obtain accurate audiologic, electronystagmographic, and laboratory data before deciding on revisional surgery. The overall interests of the patient are sometimes best served by avoiding further surgical insults to the ear.

Introduction Tympanoplasty and mastoidectomy are common procedures performed by otolaryngologists that can result in complications for which patients may seek compensation. Medical malpractice case analyses may offer insight into how clinicians can avoid risk and improve patient satisfaction. We aimed to comprehensively characterize litigation after mastoidectomies and tympanoplasties in the United States. Methods The Westlaw Campus Research legal database was searched for all available court decisions associated with claims of medical malpractice after tympanoplasty and/or mastoidectomy in th

INTRODUCTION: A clear understanding of the difficulty levels of different aspects of a surgery is required to effectively guide and assess trainees. This study investigated which steps of cortical mastoidectomy are most challenging for novice and intermediate surgeons, and how these performances align with expert opinions.
METHODS: Thirty participants were divided into three groups: 10 novices (medical students), 10 intermediates (surgical trainees), and 10 experts (consulting ENT surgeons). Each performed cortical mastoidectomy on 8 anatomically different temporal bone specimens on a virtual

BACKGROUND AND PURPOSE OF THE STUDY: the role of cortical mastoidectomy in the surgical treatment of tubotympanic type of otitis media has remained controversial especially when there is no evidence of active infection. Though literature is replete with studies for and against the requirement of mastoidectomy, there is a paucity of prospective studies in this regard. A randomized controlled trial was conducted to assess the impact of mastoidectomy in the management of mucosal chronic otitis media.
METHODS: 62 patients with uncomplicated mucosal chronic otitis media were randomly allotted to t

Open mastoid surgery still remains a principal operative treatment for chronic discharging ear. The indications for this procedure still abound in the tropics although it is rarely done in the developed countries nowadays. This series re-emphasize the control of chronic otorrhoea and operative wound infection as benefiting from a combined open mastoidectomy and reconstruction with deep temporalis fascia flap in children. The operative technique is further discussed. It is our hope that repopularising this procedure will help in the control of highly prevalent chronic ear in the tropical enviro

Chronic suppurative otitis media (CSOM) without cholesteatoma, the surgical treatment of which is still controversial, is a common diagnosis in otologic practice. A retrospective analysis of 323 patients who underwent surgery for noncholesteatomatous chronic otitis media in the Gruppo Otologica, Piacenza, Italy, between April 1983 and December 1993 is presented. Cases were separated into three groups according to different surgical treatment modalities and conditions of the ears at the time of operation. Group I (n = 53) consisted of cases of CSOM treated by tympanoplasty without mastoidectomy

A retrospective analysis of patients who underwent surgery for chronic otitis media, cholesterol granuloma, and cholesteatoma between 1971 and 1979 was performed. The prevalence of postoperative cholesteatoma was higher than that previously reported in the literature. This emphasizes the need to re-explore patients who have had intact canal wall tympanoplasty with mastoidectomy performed for the removal of cholesteatoma. The elimination of cavity problems and better functional results must be balanced against the need for a second operation when an intact canal wall procedure for cholesteatoma

Endocranial complications, which are undiagnosed or diagnosed late, present the greatest danger in masked mastoiditis. Inadequate treatment will inevitably result in irreversible and progredient loss of conductive hearing. In children, deterioration of the general condition and disturbances in physical development are frequently the first symptoms indicating mastoid infection. Because its pathohistological specifity, a reliable interruption and elimination of the mastoid infection can only be achieved by radiacal mastoidectomy. Negative criticism of mastoidectomy is not supported by recent cli

BACKGROUND: Mastoid obliteration after canal wall down mastoidectomy (CWDM) is recommended to reduce both complications and recurrence. In this study, the bone allograft was used for mastoid obliteration in large cavities and reconstruction of the external auditory canal (EAC) after retrograde CWDM.
METHODS: This is a prospective, single-arm, open-label trial. Inclusion criteria were adult patients (>18 years old) who had undergone retro grade CWDM due to chronic otitis media with cholesteatoma. Bone allograft was used for mastoid obliteration in large cavities and for EAC reconstruction.

When surgically managing cholesteatoma, the Bondy modified radical mastoidectomy (MRM) and retrograde mastoidectomy can enhance visualization while minimizing the extent of surgery required for disease removal. The Bondy MRM can be used for disease limited to the atticoantral space, when the ossicles and middle ear space are otherwise healthy, and for canal cholesteatomas. The retrograde mastoidectomy offers a safe alternative for patients with unfavorable tegmen and sigmoid sinus positions and poorly pneumatized mastoids. Both approaches achieve favorable recidivism rates and postoperative he

Introduction Retraction pocket is a condition in which the eardrum lies deeper within the middle ear. Its management has no consensus in literature. Objective To assess the role of mastoidectomy in the management of retraction pockets added to a tympanoplasty. Methods Prospective study of patients with retraction pocket and referred to surgery. The patients were randomly assigned to two groups: one managed with tympanoplasty and mastoidectomy and the other group with tympanoplasty only. The minimum follow-up considered was 12 months. The outcomes were: integrity of eardrum, recurrence, and

Mastoidectomy is a common surgical procedure within otology. Despite being inherently well suited for implementation of robotic assistance, there are no commercially available robotic systems that have demonstrated utility in aiding with this procedure. This article describes a robotic technique for image-guided mastoidectomy with an experimental cooperatively controlled robotic system developed for use within otolaryngology-head and neck surgery. It has the ability to facilitate enhanced operative precision with dampening of tremor in simulated surgical tasks. Its kinematic design is such tha

Osteomyelitis of the temporomandibular joint (TMJ) is a rare disease that is characterized by osteitis of the mandibular condyle or glenoid fossa. Although this condition is usually associated with trauma, dental disease, or extension of a local infection, it may also occur as a complication of otologic infection. However, osteomyelitis of the TMJ as a complication of otologic surgery has not yet been reported. I present the case of a 54-year-old woman who developed a polyp on the superior portion of the tympanic membrane 9 weeks after canal wall down mastoidectomy. This is an extremely rare c

INTRODUCTION: Etiopathogenetically, there are two types of chollesteatomas: congenital, and acquired. Numerous theories in the literature try to explain the nature of the disease, however, the question about cholesteatomas remain still unanswered. The aim of the study was to present a case of external ear canal cholesteatoma (EEC) developed following microsurgery (ventilation tube insertion and mastoidectomy), as well as to point ant possible mechanisms if its development.
CASE REPORT: A 16-year-old boy presented a 4-month sense of fullness in the ear and otalgia on the left side. A year befo
