Knowledge of the anatomy of the mediastinal nerves is essential for the evaluation and surgical treatment of most thoracic neoplasms. Thorough knowledge of the normal anatomy of the mediastinal nerves and of their variants cannot be overestimated because nerve trauma during nerve anatomy is also important because mediastinal or lung tumors can locally infiltrate those nerves either directly or through nodal metastases, making them generally unresectable.

Solitary fibrous tumors are uncommon spindle cell neoplasms originally thought to be restricted to the pleura. We describe a 62-year-old woman who presented with stridor and an anterior mediastinal mass. At thoracotomy, a 10.5 x 6.5 x 5.5 cm, circumscribed, firm mediastinal mass demonstrated no direct cardiac or pulmonary involvement. The tumor consisted of spindle cells organized in a patternless pattern with collagenous stroma and hemangiopericytoma-like vessels. Spindle cells were immunoreactive for CD34, CD99, desmin, vimentin and bcl-2 protein and a diagnosis of mediastinal solitary fibro

Hilar and mediastinal lymphadenopathy are sometimes the only radiological manifestation of metastatic disease from extrathoracic neoplasms. This association has not been previously described with carcinoma of the cervix. We report two patients in whom hilar and mediastinal lymph node enlargement represented the first evidence of tumor recurrence.

A 40-year-old woman was seen with stridor and mediastinal widening secondary to tuberculous mediastinal lymphadenopathy mimicking neoplasm. Initially, stridor could only be controlled with high-dose corticosteroids, but following initiation of antituberculous chemotherapy corticosteroids were withdrawn successfully and the mediastinal lymphadenopathy resolved.

Mediastinal lesions occur in a wide variety of clinical conditions. Metastatic granulosa cell tumour (GCT) in the mediastinum is a rare occurrence. We report a case of a woman who had a metastatic (GCT) in her mediastinum 40 years after treatment of the initial neoplasm. Surgical resection of the mediastinal mass revealed a low-grade epithelioid neoplasm with coffee bean-shaped nuclei and immunohistochemical stains that were consistent with metastatic GCT.

The clinical records of 1,071 cases of extrathoracic malignant neoplasms seen over a 2 year period sere reviewed: 163 had abnormal chest films, and 25 of these showed evidence of mediastinal and/or hilar lymph node metastases. The primary malignancies which metastasized to intrathoracic lymph nodes included eight tumors of the head and neck, 12 genitourinary malignancies, three carcinomas of the breast, and two malignant melanomas. The chest films were analyzed to determine the distribution of lymph node groups involved. Unilateral lymph node enlargement occurred in eight. The most frequently

Intrathoracic neoplasms other than bronchogenic carcinoma comprise a disparate group of tumors that have in common an anatomic location within the thorax. For the purposes of this review, they are divided into three categories: pulmonary metastases from locations other than the thorax, mediastinal neoplasms, and unusual primary lung neoplasms. This article reviews the recent literature; discusses evolving strategies for imaging, diagnosis, and treatment; and presents promising research developments.

Anterior mediastinal neoplasms comprise a diverse group of tumors and occasionally manifest as a cystic lesion. We retrospectively reviewed computed tomography (CT) scans in 46 patients with pathologically proved cystic anterior mediastinal tumors. We categorize them into a pure cystic mass and a cystic tumor with a solid portion. In this pictorial essay, the radiologic appearances of cystic anterior mediastinal tumors are described with CT and illustrated with their gross and histopathological findings on resected specimen.

A detailed study is made of the anatomy and physiology of the mediastinum. Proceeding from the anatomo-physiologic peculiarities of the mediastinum, the clinical picture, pathoanatomy and diagnostic possibilities of mediastinal tumours and cysts are comprehensively discussed. Seventeen patients with primary true mediastinal tumours and cysts, of which 16 cases with benign neoplasms and one - with malignant blastoma are reported. The results of the surgical treatment of benign mediastinal tumours and cysts are estimated as very good.

Synovial sarcoma is a malignant mesenchymal neoplasm that accounts for approximately 10% of all soft tissue sarcomas. These tumors most commonly occur in the extremities of young adults but are not restricted to this site and can arise in virtually any organ system. Synovial sarcomas originating in the mediastinum are exceptionally rare and are often mistaken for other, more common neoplasms in this location, especially since there are no specific imaging characteristics or clinical manifestations. Contrary to synovial sarcomas of the extremities, mediastinal tumors more commonly affect male p

INTRODUCTION: Mediastinal diseases in HIV patients show complex diagnostic challenges due to opportunistic infections and rare neoplasms. While Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is a key technique, its effectiveness is limited as it only provides cytological samples. Endobronchial ultrasound-guided mediastinal cryobiopsy (CryoEBUS) emerges as a promising alternative, yielding larger and higher-quality samples. This study, the first in Mexico and the first worldwide in HIV patients, evaluates its diagnostic efficacy compared to EBUS-TBNA, highlighting

Lesions of the anterior mediastinum typically include neoplasms of the thymus or thyroid, hemangioma, germ cell neoplasms, lymphoma and others. Renal cell carcinoma often presents with flank pain, hematuria, abdominal pain and/or fever. Chest cavity involvement by renal cell carcinoma typically manifests as pulmonary parenchymal disease with or without hilar lymph node involvement. We report an unusual presentation of renal cell carcinoma in a patient who presented with symptoms secondary to a large anterior mediastinal mass. A 64-year-old woman came to the hospital complaining of malaise, cou

The clinical and autopsy findings in a case of histologically benign teratoma of the mediastinum in a 19-year-old man with Klinefelter's syndrome are presented. A rough statistical analysis based on a review of the literature revealed that the incidence rate of Klinefelter's syndrome among the patients with mediastinal germ cell tumor is 30 to 40 times that of Klinefelter's syndrome among the general control population. The pathogenesis of this neoplasm appears to be related to the genetic abnormality in Klinefelter's syndrome, and this seems to predispose to the development of extragonadal me

Mammalian ontogenesis and postnatal histogenesis involves the dynamic and appropriate interaction of two growth related phenomena: progression and regression. The thymus gland is the organ of the mammalian body that exhibits the most profound involution during normal postnatal histogenesis. Involution of the thymus can be compared to similarly regressive processes during the ontogeny of holometabolic insects, as well as to the spontaneous regression of neoplasms. It can be expected that in the future a better understanding of neoplastic regression will result from the comparison of ontogenetic

Primary mediastinal germ-cell tumors are rare neoplasms that usually occur in young men. Computed tomographic (CT) scans in four patients demonstrated large anterior mediastinal masses obliterating fat planes and extending inferiorly along the left mediastinal border. Nonseminomatous tumors contained areas of near-water density (cyst formation and/or necrosis) as well as soft-tissue density. Although CT features may suggest the diagnosis before histologic confirmation, the most important roles of CT are evaluation of extent of disease and monitoring response to therapy.

BACKGROUND: Mediastinal mass management may represent a real challenge for all the physicians who deal with it. Mediastinum, in fact, contains different vital structures which are often involved by growing neoplasms with increasing severity. Therefore, up to 60% of the patients with mediastinal mass are symptomatic, according to the structure involved. Different neoplasms may arise essentially from all mediastinal organs, whereas thymic epithelial tumors and lymphomas represent more than 90% of mediastinal tumors.
CASE DESCRIPTION: We report the first case described of a giant asymptomatic me

Of the 178 consecutive mediastinoscopies performed at the Istituto Nazionale per lo Studio e la Cura dei Tumori of Milan from 1967 to 1974 145 were performed for diagnostic purposes and 33 for the pre-operative evaluation of resectability in cases of proven lung carcinoma. Mediastinoscopy is a very useful procedure for obtaining a definite pathological diagnsis in indeterminate mediastinal and pulmonary lesions. In succases, mediastinoscopy should precede any attempt at therapy, as 1/3 primary mediastinal lymph node lesions are benign and about 30% of suspected mediastinal tumors or lymphomas

Thymic neoplasms are a common cause of an anterior mediastinal mass and may be benign or malignant. Thymic cysts are congenital or acquired and may be associated with a thymic malignancy. True thymic hyperplasia and thymic lymphoid hyperplasia may enlarge the thymus and simulate a neoplasm. Thymoma and thymic carcinoma are epithelial malignancies with distinct clinicopathologic features. Thymic carcinoid is a rare aggressive neuroendocrine malignancy associated with multiple endocrine neoplasia 1. Thymolipoma is a benign neoplasm. Hodgkin and non-Hodgkin lymphoma may primarily or secondarily i

The literature on pediatric sudden unexpected death (SUD) due to unrecognized mediastinal neoplasms is limited to a small number of case reports with several cases confirmed to be secondary to T-cell lymphoblastic lymphoma (T-cell LBL). Mediastinal T-cell LBL can be rapidly progressive and potentially fatal due to the compression and obstruction of the airway and/or the great vessels. The clinical presentation is nonspecific with a predominance of respiratory symptoms that are more apparent when the patient is supine. We presented three cases of pediatric SUD attributed to forensic autopsy-dia

Neoplasms of the left upper lobe may spread directly to the anterior mediastinal group of nodes without involving the inferior tracheobronchial, superior tracheobronchial, or paratracheal nodal chain. Routine cervical mediastinoscopy does not sample the anterior mediastinal node group. Parasternal anterior mediastinotomy was performed in 28 patients with left upper lobe carcinoma and normal findings from cervical mediastinoscopy. Despite the normal findings at cervical mediastinoscopy, 10 of the 28 patients were deemed to have inoperable disease because of spread of the neoplasm to the anterio
