Regular use of Thai kratom has been linked to reduced blood triglyceride levels and body mass index (BMI) in healthy individuals. We analyzed Green Thai Kratom (GTK) and Red Thai Kratom (RTK) to investigate their effects on pancreatic digestive enzymes. The ethanol extracts of GTK and RTK inhibited lipase activity more strongly than alpha-glucosidase activity, suggesting the presence of lipase inhibitors. Mitragynine, the major compound in GTK, showed potent lipase inhibition and moderate alpha-glucosidase inhibition. Quercetin, found in both extracts, strongly inhibited alpha-glucosidase but

The leaves of Mitragyna speciosa (commonly known as kratom), a tree endogenous to parts of Southeast Asia, have been used traditionally for their stimulant, mood-elevating, and analgesic effects and have recently attracted significant attention due to increased use in Western cultures as an alternative medicine. The plant's active alkaloid constituents, mitragynine and 7-hydroxymitragynine, have been shown to modulate opioid receptors, acting as partial agonists at mu-opioid receptors and competitive antagonists at kappa- and delta-opioid receptors. Furthermore, both alkaloids are G protein-bi

The term kratom is commonly used for both Mitragyna speciosa and herbal products prepared mainly from leaves. Kratom is well known as a drug that can serve as a less toxic and less-addictive pain-relieving substitute for opium, as well as a therapy for hypertension, cough, and diarrhea. Its major alkaloid, mitragynine, also deserves concern. However, most people use kratom as a psychological stimulant, which carries a risk of addiction associated with negative social and health impacts. This paper reviews basic facts about kratom and its potential use in pharmacology, pharmacokinetics, and pha

BACKGROUND: Drug-induced hyperpigmentation accounts for 10%-20% of acquired pigmentary disorders and can be misdiagnosed for other causes such as melasma, post inflammatory changes, or heavy metal deposition. Kratom (Mitragyna speciosa), a Southeast Asian plant with an opioid-like profile, has been increasingly used in the United States for several conditions such as pain, mood disorders, and opioid withdrawal. While neuropsychiatric and liver toxicity adverse effects are documented, hyperpigmentation, and skin changes remain poorly recognized.
CASE PRESENTATION: We describe a 38-year-old fem

Alcohol use disorder (AUD) accounts for nearly 4.7% of all deaths and imposes a huge economic burden on society. Despite the magnitude of the problem, only a few Food and Drug Administration (FDA)/European Medicines Agency (EMA)-approved drugs are currently available for AUD treatment. Despite being efficacious, these drugs are not without problems, adverse effects being a major issue. That combined with medication adherence and compliance problems, the discovery of new drugs is imperative. Kratom (Mitragyna speciosa) alkaloids and some of their semisynthetic derivatives reduce alcohol intake

The chemical composition of total alkaloids from leaves and roots of Guiera senegalensis was investigated. Three beta-carboline alkaloids were purified: in addition to harman and tetrahydroharman, known in roots and leaves, harmalan (dihydroharman) was isolated for the first time from roots of Guiera senegalensis. Guieranone A, a naphthyl butenone, was also purified from leaves and roots. The in vitro antiplasmodial activity and the cytotoxicity of extracts and pure compounds were evaluated. Each total alkaloid extract and beta-carboline alkaloids presented an interesting antiplasmodial activi

Kratom (Mitragyna speciosa) is a commonly used over-the-counter supplement with opioid-like and dopaminergic activity. Cutaneous side effects have been incompletely characterized; only limited case reports describe its associated hyperpigmentation. We describe two cases of kratom-induced photodistributed brown-gray patches with biopsies that show circular, orange-brown granules in the dermis that stain positive with Fontana-Masson and negative for iron. In conjunction with existing literature, our cases suggest a possible distinct clinicopathologic pattern for which kratom use can be suspected

Here we present a case of a coincidence of addiction to "Kratom" (botanically known as Mitragyna speciosa Korth) and developed severe primary hypothyroidism. We are discussing a possibility that high dose of indole alkaloid mitragynine (the major alkaloid identified from "Kratom") might reduce the normal response of the thyroid gland to thyroid-stimulating hormone resulting in primary hypothyroidism. Further experimental investigations of mitragynine as a possible suppressor of thyroid gland function would be a matter of interest.

Kratom use is increasingly encountered in clinical practice, yet products marketed under this name encompass markedly different substances with distinct pharmacologic and risk profiles. Kratom leaf (Mitragyna speciosa) contains a complex mixture of alkaloids, primarily mitragynine, with serving size dependent stimulant and sedative effects, whereas many modern products labeled as kratom contain concentrated, semi-synthetic, or synthetic alkaloids, particularly 7-hydroxymitragynine (7-OH), with substantially greater opioid-like potency. This review summarizes the pharmacology, clinical effects,

A structure-activity relationship study of active molecules against chloroquine-resistant Plasmodium falciparum K1 strain is reported. Structurally simplified analogues of antiplasmodial active alkaloids presented similar levels of activity as their corresponding natural products extracted from Guiera senegalensis and Mitragyna inermis with IC50 values on chloroquine-resistant P. falciparum K1 strain of up to 10.6 μM for spirooxindoles and 13.8 μM for β-carbolines. The identification of such simpler and cheaper structural analogues is crucial to efficiently study these natural products' action

BACKGROUND: Within the past year, the online sale of semi-synthetic products containing chemicals derived from mitragynine, kratom's primary alkaloid: 7-hydroxymitragynine (7-OH) and mitragynine pseudoindoxyl (MP) has emerged. Both are highly selective mu opioid receptor agonists undetectable or not present in fresh kratom leaves and may, as constituents of novel product formulations, pose public health risks.
METHOD: Between September 2024-February 2025 we abstracted from websites of vendors selling 7-OH and MP products information about these products, including formulation types, dose/serv
