Neural Vision Restoration in Ophthalmology.

Neural vision restoration is a rapidly advancing discipline at the intersection of neuroscience, bioengineering, and ophthalmology. This review synthesizes emerging strategies to restore visual perception through retinal prostheses, optic nerve and thalamic implants, cortical brain-computer interfaces (BCIs), optogenetics, and non-invasive stimulation. Although initial experiments have demonstrated primitive visual abilities such as light perception and motion detection, artificial vision remains cognitively demanding and fundamentally different  from natural vision. Advances in artificial int

Therapeutic advances in tremor.

Although tremor is a highly prevalent movement disorder, progress in this field is limited because of the poor understanding of many of the underlying conditions. This review summarizes recent treatment attempts since 2013. For essential tremor, recent innovations are phase I or II studies of Octanol, several clinically relevant refinements for deep brain stimulation, and the development of the new magnetic resonance imaging guided focused ultrasound technique. Further new invasive and noninvasive electrical and magnetic stimulation techniques have been tested for essential tremor and parkinso

Towards a closed loop retinal prosthesis: measuring electrically evoked retinal responses using large electrodes.

Objective.Sensory prostheses use arrays of electrodes to stimulate neural tissue and restore a sense of vision or hearing. At perceptible levels of stimulation, the current from each electrode spreads and causes overlapping regions of neural activation. This lack of specificity results in perceptual deficits. Methods to overcome this reduced specificity, such as a closed loop stimulation approach require measurement of the neural response to stimulation. This investigation tests the possibility of using the large stimulating electrodes such as those required by some subretinal or suprachoroida

Diagnostic features predicted by deep learning improve human object recognition in simulated prosthetic vision.

Objective. Cortical visual prostheses aim to partially restore vision by electrically stimulating V1. Yet, their effectiveness is constrained by the limited number of simultaneously elicitable phosphenes. Identifying sparse but informative visual features for object recognition is therefore crucial to enable functional prosthetic vision. In this study, we aim to target diagnostic features for object recognition.Approach. We combined a deep learning pipeline with bioplausible phosphene simulation to optimize phosphene placement for object recognition. The phosphene renderings were presented to

Endovascular neuromodulation: feasibility of endovascular stimulation near the cerebellum.

Objective.Electrical stimulation and neural recording underpin neural prostheses for restoring function and treating neurological disorders, but clinical adoption is limited by the invasiveness of implantation. The Endovascular Neural Interface offers an alternative by accessing intracranial targets through the cerebral vasculature. This work presents the first strength-duration characterization of cortical evoked potentials, elicited by endovascular stimulation, adjacent to the cerebellum.Approach.A polymer-based stent-electrode array was deployed into the left transverse sinus of an ovine mo

An experimental vestibular neural prosthesis: design and preliminary results with rhesus monkeys stimulated with modulated pulses.

A vestibular neural prosthesis was designed on the basis of a cochlear implant for treatment of Meniere's disease and other vestibular disorders. Computer control software was developed to generate patterned pulse stimuli for exploring optimal parameters to activate the vestibular nerve. Two rhesus monkeys were implanted with the prototype vestibular prosthesis and they were behaviorally evaluated post implantation surgery. Horizontal and vertical eye movement responses to patterned electrical pulse stimulations were collected on both monkeys. Pulse amplitude modulated (PAM) and pulse rate mod

Mathematical models and human disease.

Mathematical models of brain function are built from data covering anatomy, physiology, biophysics and behavior. In almost all cases, many possible models could fit the available data. Theoreticians make assumptions that allow them to constrain the number of possible model structures. However, a model that was more useful clinically would result if the constraints came from lesion studies in animals or clinical disorders. Here, we show a few examples of how clinical disorders have led to improvements in models. We also show a few examples of how models could lead to neural prostheses for patie

Liquid-metal-based three-dimensional microelectrode arrays integrated with implantable ultrathin retinal prosthesis for vision restoration.

Electronic retinal prostheses for stimulating retinal neurons are promising for vision restoration. However, the rigid electrodes of conventional retinal implants can inflict damage on the soft retina tissue. They also have limited selectivity due to their poor proximity to target cells in the degenerative retina. Here we present a soft artificial retina (thickness, 10 μm) where flexible ultrathin photosensitive transistors are integrated with three-dimensional stimulation electrodes of eutectic gallium-indium alloy. Platinum nanoclusters locally coated only on the tip of these three-dimension

Intraretinal Electrophysiology and Resistivity Profiles of WT and RCS Rat Retina.

Retinal prostheses have been utilized in the treatment of blindness resulting from retinal degeneration. However, they have not met patient expectations, leading to market withdrawals. As a result, research continues to focus on improving visual perception, such as by modeling retinal neural activation. The retina's electrical resistivity profile is key, as it influences the current spread during electrical stimulation. To advance efficient stimulation parameters, more data on the electrical properties of the retina in both its healthy and diseased state is needed. While this question has been

Seeing anew: Mechanisms, outcomes, and challenges in adapting to restored sight via visual prostheses.

Restoring vision through prosthetic devices represents one of the most ambitious frontiers in translational neuroscience. Despite decades of progress, functional outcomes remain highly variable, often falling short of patient expectations, as adapting to restored vision through visual prostheses is a complex process influenced by neural plasticity, perceptual learning, and psychological factors. We propose a five-stage framework that captures the hierarchically dependent yet fragile process of adaptation: Device Activation, Perceptual Calibration, Cognitive Integration, Behavioral Generalizati

Machine learning for hand pose classification from phasic and tonic EMG signals during bimanual activities in virtual reality.

Myoelectric prostheses have recently shown significant promise for restoring hand function in individuals with upper limb loss or deficiencies, driven by advances in machine learning and increasingly accessible bioelectrical signal acquisition devices. Here, we first introduce and validate a novel experimental paradigm using a virtual reality headset equipped with hand-tracking capabilities to facilitate the recordings of synchronized EMG signals and hand pose estimation. Using both the phasic and tonic EMG components of data acquired through the proposed paradigm, we compare hand gesture clas

Cervical arthroplasty: material properties.

Discectomy, decompression, and fusion are traditionally used to manage cervical disc disease accompanied by neural element compression that is refractory to conservative management. Concerns regarding stress at levels adjacent to fusion and possible adjacent-level degeneration as well as a desire to maintain a more normal biomechanical environment have led to investigation of cervical disc replacement as an alternative to fusion procedures. Cervical disc prostheses currently under investigation are constructed of predominantly metal-on-polyethylene or metal-on-metal bearing surfaces, and use r

Electrochemical and Electrophysiological Performance of Platinum Electrodes Within the Ninety-Nine-Electrode Stimulating Nerve Cuff.

The trend in neural prostheses using selective nerve stimulation for electrical stimulation therapies is headed toward single-part systems having a large number of working electrodes (WEs), each of which selectively stimulate neural tissue or record neural response (NR). The present article reviews the electrochemical and electrophysiological performance of platinum WE within a ninety-nine-electrode spiral cuff for selective nerve stimulation and recording of peripheral nerves, with a focus on the vagus nerve (VN). The electrochemical properties of the WE were studied in vitro using the electr

Cognitive signals for brain-machine interfaces in posterior parietal cortex include continuous 3D trajectory commands.

Cortical neural prosthetics extract command signals from the brain with the goal to restore function in paralyzed or amputated patients. Continuous control signals can be extracted from the motor cortical areas, whereas neural activity from posterior parietal cortex (PPC) can be used to decode cognitive variables related to the goals of movement. Because typical activities of daily living comprise both continuous control tasks such as reaching, and tasks benefiting from discrete control such as typing on a keyboard, availability of both signals simultaneously would promise significant increase

Leg Prosthesis With Somatosensory Feedback Reduces Phantom Limb Pain and Increases Functionality.

Phantom limb pain (PLP) develops in most patients with lower limb amputation. Changes in the peripheral and central nervous system (CNS) are hypothesized to contribute to PLP. Based on ideas to modify neural reorganization within the CNS, the aim of the study was to test, whether prostheses with somatosensory feedback might help to reduce PLP, and increase the functionality of movement with a prosthesis. We therefore equipped the prostheses of 14 lower leg amputees with a simple to use feedback system that provides electrocutaneous feedback to patients' thigh whenever the foot and toes of the

Applying Common Spatial Pattern and Convolutional Neural Network to Classify Movements via EEG Signals.

Developing an electroencephalography (EEG)-based brain-computer interface (BCI) system is crucial to enhancing the control of external prostheses by accurately distinguishing various movements through brain signals. This innovation can provide comfortable circumstances for the populace who have movement disabilities. This study combined the most prospering methods used in BCI systems, including one-versus-rest common spatial pattern (OVR-CSP) and convolutional neural network (CNN), to automatically extract features and classify eight different movements of the shoulder, wrist, and elbow via EE

Fuzzing the brain: automated stress testing for the safety of ML-driven neurostimulation.

Objective.Machine learning (ML) models are increasingly used to generate electrical stimulation patterns in neuroprosthetic devices such as visual prostheses. While these models promise precise and personalized control, they also introduce new safety risks when model outputs are delivered directly to neural tissue. We propose a systematic, quantitative approach to detect and characterize unsafe stimulation patterns in ML-driven neurostimulation systems.Approach.We adapt an automated software testing technique known as coverage-guided fuzzing to the domain of neural stimulation. Brain implants for substituting lost motor function: state of the art and potential impact on the lives of motor-impaired seniors.

Recent scientific achievements bring the concept of neural prosthetics for reinstating lost motor function closer to medical application. Current research involves severely paralyzed people under the age of 65, but implications for seniors with stroke or trauma-induced impairments are clearly on the horizon. Demographic changes will lead to a shortage of personnel to care for an increasing population of senior citizens, threatening maintenance of an acceptable level of care and urging ways for people to live longer at their home independent from personal assistance. This is particularly challe

Applying intuitive EEG-controlled grasp neuroprostheses in individuals with spinal cord injury: Preliminary results from the MoreGrasp clinical feasibility study.

The aim of the MoreGrasp project is to develop a non-invasive, multimodal user interface including a brain-computer interface (BCI) for control of a grasp neuroprostheses in individuals with high spinal cord injury (SCI). The first results of the ongoing MoreGrasp clinical feasibility study involving end users with SCI are presented. This includes BCI screening sessions, in which we investigate the electroencephalography (EEG) patterns associated with single, natural movements of the upper limb. These patterns will later be used to control the neuroprosthesis. Additionally, the MoreGrasp grasp

The LION procedure to the pelvic nerves for treatment of urinary and faecal disorders.

Bladder dysfunctions have been treated for decades through medical treatments or surgical procedures, especially in the context of prolapse conditions and stress incontinence. Over the last decade, sacral nerve modulation (SNM) has been introduced as a further option in the treatment of some urinary and fecal symptoms. Current techniques of percutaneous implantation are limited to superficial extrapelvic nerves that expose patients to lead migration and dislocation or infections, complications that cannot be ignored. Access to endopelvic nerves and plexuses will be mandatory in the future, esp
