Absorbed dose and deterministic effects to patients from interventional neuroradiology.

Following the presentation of radiation-induced skin effects by three patients who had undergone glue embolisation of intracranial arteriovenous malformation (AVM), measurements were made of absorbed dose to the skin of patients undergoing other interventional neuroradiological procedures that involve long fluoroscopy times. The maximum absorbed dose to the skin measured by thermoluminescent dosemeters during these procedures was 4 Gy. From these measurements and from records of fluoroscopy time and the number of digital runs acquired, estimates of the maximum absorbed skin dose were made for

Imaging findings in hypophysitis: a review.

Hypophysitis (HP) is a rare acute or chronic inflammatory condition of the pituitary gland. The greatest challenge in the management of HP is establishing a diagnosis through clinical criteria and non-invasive methods and predicting the patients' clinical outcome. The aim of this review is to describe the neuroradiological findings of this rare disease, providing some information regarding the possible differential diagnosis in order to avoid unnecessary surgery. Gadolinium-enhanced pituitary magnetic resonance imaging (MRI) is considered the neuroradiological investigation of choice. The feat

Neuroradiologic review of intracranial infection.

The detection of intracranial infection continues to be a common reason for neuroradiologic examination. This review covers the neuroradiologic findings of the variety of common infections. It includes meningitis and its complications, as well as bacterial, viral, tuberculous, fungal, and parasitic diseases. Finally, a review of the common infections associated with AIDS is presented. With an increased ability to recognize such infections, radiologists should be able to supply their clinical colleagues with more specific diagnoses.

Imaging of Nontraumatic Neuroradiology Emergencies.

Imaging of acute neurologic disease in the emergency department can be challenging because of the wide range of possible causes and the overlapping imaging appearance of many of these entities on nonenhanced computed tomography (CT). The key to formulating a succinct, pertinent differential diagnosis includes characterizing the pattern of abnormalities on CT and identifying key features that suggest a particular diagnosis. This article divides neurologic emergencies into 5 scenarios based on the CT findings, including subarachnoid hemorrhage, intraparenchymal hemorrhage, vasogenic edema withou

Inter- and intraobserver agreement in CT characterization of nonaneurysmal perimesencephalic subarachnoid hemorrhage.

BACKGROUND AND PURPOSE: The perimesencephalic pattern of SAH as seen on unenhanced CT is associated with significantly better outcomes when compared to an aneurysmal pattern of SAH. The aim of this study was to determine the degree of inter- and intraoberver agreement for characterization of the NAPH as seen on unenhanced CT. MATERIALS AND METHODS: We retrospectively reviewed the CT scans of 37 patients with spontaneous SAH, all of whom had undergone CT within 24 hours of onset of headache symptoms. All patients had undergone conventional cerebral angiography to confirm or exclude aneurysms o

Pituitary height and neuroradiological alterations in patients with Prader-Labhart-Willi syndrome.

Prader-Willi syndrome (PWS), a genetic disorder due to an alteration in the paternally derived long arm of chromosome 15, is characterized by a complex clinical picture (short stature, obesity, hypogonadism) that seems to be referable to as a central hypothalamic/pituitary dysfunction. To determine whether there is any diminution in the anterior pituitary gland or other neuroradiological alterations, we retrospectively analysed 91 patients with PWS (42 females, 49 males; age range: 0.7-16.8 years) by cerebral magnetic resonance imaging (MRI). Of these 91 patients, MRI analysis showed a reducti

Transcranial Doppler sonography. Part 2. Evaluation of intracranial and extracranial abnormalities and procedural monitoring.

Transcranial Doppler sonography can be used to evaluate a spectrum of intracranial and extracranial vascular abnormalities. It is of proved value in the detection and follow-up of vasoconstriction caused by subarachnoid hemorrhage and can be used to demonstrate significant stenosis or occlusion of basal intracranial arteries and coexisting routes of collateral circulation. Transcranial Doppler sonography can play an important role in the determination of brain death and can be used to identify the nidus of an arteriovenous malformation, along with its major routes of supply and drainage. [Neuroradiologic emergencies in infectious pathology].

The main goal of urgent imaging evaluation of patients with suspected CNS infection is to differentiate infectious from tumoral or vascular lesions in order to provide appropriate management. MR imaging, including diffusion weighted imaging and spectroscopy, is superior to CT imaging to characterize lesion location and etiology. The CT and MRI features of the more frequent bacterial, viral and parasitic CNS infections will be described.

Intrathecal gadolinium-enhanced MR cisternography in the evaluation of clinically suspected cerebrospinal fluid rhinorrhea in humans: early experience.

In this prospective multicenter study, the authors evaluated the utility of magnetic resonance (MR) cisternography after intrathecal administration of gadopentetate dimeglumine in 15 patients clinically suspected of having cerebrospinal fluid (CSF) rhinorrhea. By means of lumbar puncture, a single dose of 0.5 mL of gadopentetate dimeglumine was injected into the lumbar subarachnoid space. Thirteen patients showed leakage of contrast material through the cribriform plate into the ethmoid or sphenoid air cells. No leakage was observed in two patients. The study results show the relative safety a

Clinical diagnostic reference levels in neuroradiology based on clinical indication.

This study focuses on patient radiation exposure in interventional neuroradiology (INR) procedures, a field that has advanced significantly since its inception in the 1980s. INR employs minimally invasive techniques to treat complex cerebrovascular diseases in the head, neck, and spine. The study establishes diagnostic reference levels (DRLs) for three clinical indications (CIs): stroke (S), brain aneurysms (ANs), and brain arteriovenous malformation (AVM). Data from 209 adult patients were analyzed, and DRLs were determined in terms of various dosimetric and technical quantities. For stroke,

The role of interventional neuroradiology in the management of skull base tumours and related surgical complications.

The management of hypervascular skull base tumours is complex and requires a multidisciplinary approach. Skull base surgery may be challenging because of the risk of serious intra-operative bleeding and of potential injuries to lower cranial nerves and/or large cervical vessels. Over the last four decades, advances in neuro-interventional procedures have produced a range of adjunctive endovascular techniques in addition to conventional surgery. Digital subtraction angiography (DSA) allows for a better understanding of tumour vascularisation and its relationship with Surrounding vessels. Tumour

Establishment of portuguese diagnostic reference levels and trigger levels for radiation-induced tissue reactions in interventional neuroradiology.

PURPOSE: To establish national diagnostic reference levels (DRLs) for interventional neuroradiology (INR) procedures in Portugal. METHODS: A multicentre retrospective study was performed in six reference centres, including the six most common INR procedures: cerebral thrombectomy, cerebral arteriography, cerebral aneurysm embolisation, arteriovenous malformation (AVM) and arteriovenous fistula (AVF) embolisation, cerebral vasospasm treatment, and carotid artery stenting. Additionally, retinoblastoma embolisation, performed in a single Portuguese centre, was included. DRLs were defined as the
