OBJECTIVES: There are at least 3 million North American Indians and Eskimos in North America. The epidemiology of rheumatic diseases in Native North Americans differs from that described for the remainder of the North American population. An enhanced understanding of rheumatic diseases in these indigenous people may provide valuable clues to the cause of these disorders and improve rheumatologic care.
METHODS: The world literature was searched for all reports of rheumatic diseases in North American Indians and Eskimos. The reports were reviewed and the findings summarized by disease process.

Culicoides sp. are the primary vectors of two hemorrhagic disease (HD) viruses, Epizootic hemorrhagic disease virus (EHDV) and bluetongue virus (BTV). In North America, Epizootic hemorrhagic disease (EHD) and Bluetongue disease (BT) cause significant morbidity and mortality in white-tailed deer (Odocoileus virginianus) populations and can cause morbidity in farmed animals, resulting in restricted agricultural trade. Originally, HD was limited to southeastern and southcentral North America but has expanded in distribution to the north and west, resulting in severe disease outbreaks in naïve wil

PURPOSE OF REVIEW: Arthropod-borne viruses (arboviruses) represent a group of pathogens with increasing global relevance, some of which cause neuroinvasive disease. Transmitted by arthropod vectors and maintained by a variety of primary and amplifying hosts, epidemics are dependent on numerous environmental and anthropogenic factors. This review serves to highlight several important neuroinvasive arboviruses relevant to North America and discuss the neurologic presentations, diagnosis, outcomes, and future trends.
RECENT FINDINGS: Recent shifts in the epidemiology and ecology of arboviruses i

RATIONALE: Podocyte infolding glomerulopathy (PIG) is a newly described condition with only 37 cases reported worldwide. Due to its rarity, the pathogenesis and evolution of this disease is unclear. This case report contributes to our collective knowledge about the clinical and histological progression of this disease.
PRESENTING CONCERNS OF THE PATIENT: Over the course of a year, a 52-year-old Malaysian woman with no known prior medical history developed progressively worsening edema and other findings consistent with nephrotic syndrome.
DIAGNOSIS: Unlike most patients with PIG, this patien

BACKGROUND: Improved understanding of Bartonella species seroepidemiology in dogs may aid clinical decision making and enhance current understanding of naturally occurring arthropod vector transmission of this pathogen.
OBJECTIVES: To identify demographic groups in which Bartonella exposure may be more likely, describe spatiotemporal variations in Bartonella seroreactivity, and examine co-exposures to other canine vector-borne diseases (CVBD).
ANIMALS: A total of 15,451 serology specimens from dogs in North America were submitted to the North Carolina State University, College of Veterinary

Alcoholic liver diseases comprise a spectrum of clinical disorders and changes in liver tissue that can be detected by pathology analysis. These range from steatosis to more severe signs and symptoms of liver disease associated with inflammation, such as those observed in patients with alcoholic hepatitis or cirrhosis. Although the relationship between alcohol consumption and liver disease is well established, severe alcohol-related morbidities develop in only a minority of people who consume alcohol in excess. Inter-individual differences in susceptibility to the toxic effects of alcohol have

Using quantitative PCR analysis and DNA sequencing, we provide evidence for the presence of rat lungworm (Angiostrongylus cantonensis) in Oklahoma, USA, and identified a potentially novel rat host (Sigmodon hispidus). Our results indicate a geographic range expansion for this medically and ecologically relevant parasite in North America.

Tick-borne infections are seen throughout the United States, with varying geographical locations. Many of these infections are also increasingly seen in Europe. Certain ticks (eg Ixodes) can transmit more than one infection. Diagnosis, particularly in early infection, can be challenging and therefore knowledge of the distinguishing clinical features and epidemiology of these diseases is important. Testing for Lyme disease often causes confusion for patients and medical providers, as serological tests may be negative in early infection and conversely may be positive for years after infection. N

Plague, caused by the bacterium Yersinia pestis, is established across western North America, and yet little is known of what determines the broad-scale dimensions of its overall range. We tested whether its North American distribution represents a composite of individual host-plague associations (the "Host Niche Hypothesis"), or whether mammal hosts become infected only at sites overlapping ecological conditions appropriate for plague transmission and maintenance (the "Plague Niche Hypothesis"). We took advantage of a novel data set summarizing plague records in wild mammals newly digitized f

We report on three cases of meningococcal disease caused by ciprofloxacin-resistant Neisseria meningitidis, one in North Dakota and two in Minnesota. The cases were caused by the same serogroup B strain. To assess local carriage of resistant N. meningitidis, we conducted a pharyngeal-carriage survey and isolated the resistant strain from one asymptomatic carrier. Sequencing of the gene encoding subunit A of DNA gyrase (gyrA) revealed a mutation associated with fluoroquinolone resistance and suggests that the resistance was acquired by means of horizontal gene transfer with the commensal N. lac

Extended-spectrum beta-lactamases (ESBLs) derived from the TEM-1 beta-lactamase were first identified in the USA in outbreak strains of Klebsiella pneumoniae in the middle to late 1980s, together with the SHV-5 ESBL. The TEM-10, TEM-12 and TEM-26 enzymes have remained in US hospitals, but have been joined by other ESBLs that are variants of the SHV-1 broad-spectrum beta-lactamase. In the most recent surveys from hospitals in the eastern part of the USA, the most prominent ESBLs have been the SHV-7 and SHV-12 enzymes. In Canada, a wider variety of ESBLs has been identified, with multiple member

The U.S. Clean Air Act identifies 188 hazardous air pollutants (HAPs), or "air toxics," associated with adverse human health effects. Of these air toxics, 18 were targeted as the most important in a 10-City Pilot Study conducted in 2001 and 2002 as part of the National Air Toxics Trend Sites Program. In the present analysis, measurements available from monitoring networks in North America were used to estimate boundary layer background concentrations and trends of these 18 HAPs. The background concentrations reported in this study are as much as 85% lower than those reported in recent studies

The etiologic agents of human dirofilariasis in the Old World are Dirofilaria immitis, which cause pulmonary and subcutaneous nodules, and Dirofilaria repens, which cause ocular lesions. Although reports of new cases of dirofilariasis are sporadic in other parts of the world, a considerable amount of information is generated in Europe regarding human dirofilariasis. Most cases have been detected in the Mediterranean countries, Ukraine, and Russia; however, isolated or short series of cases have been reported in the Balkan Republics and central and northern European countries. The legal risks associated with health research involving human subjects have been highlighted recently by a number of lawsuits launched against those involved in conducting and evaluating the research. Some of these cases have been fully addressed by the legal system, resulting in judgments that provide some guidance. The vast majority of cases have either settled before going to trial, or have not yet been addressed by the courts, leaving us to wonder what might have been and what guidance future cases may bring. What is striking about the lawsuits that have been commenced is the broad range

The aim of this qualitative case study was to investigate how organ donation is promoted and managed in North America aimed specifically at patients and relatives. A 4-week study to Boston and Baltimore in America was undertaken to observe and learn how organ donation is promoted and managed within a critical care environment. The study tour was undertaken specifically to examine the policy and procedures that enhance the procurement of organ donation in specific centres in the USA. The study tour wanted to examine and gain a better understanding of the issues, policies and procedures used by

Coenurosis is a zoonotic disease of humans caused by the larval stage of Taenia (Multiceps) species. In North America, the adult tapeworm of Taenia (Multiceps) serialis is found in canids. The cystic larval forms (coenuri) are found in hares, rabbits, squirrels, and, rarely, in humans. We review in clinical detail the fifth case reported from North America, involving a child with extensive central nervous system involvement. We also report a sixth case, involving an adult with an intramuscular coenurus. The latter case was diagnosed by needle aspiration of the cyst. Although praziquantel admin
