Circadian pain assessment in neonates from a nurse's perspective.

OBJECTIVES: Circadian rhythms modulate the activity of pain pathways. But there appears to be a lack of understanding of how the perception of pain varies in neonates during the day. To assess circadian variations in pain perception we analyzed intensities of pain at 3 h intervals in neonates (within 24 hours). We also followed up the nursing interventions practiced for pain reduction and their effectivity within a day. METHODS: We have analyzed the clinical observations of pain in 16 neonates during 62 days of hospitalization (with Mean = 3.87, Min = 1, Max = 19). The pain was assessed by a

Assessing capacity to consent to sexual relations: a guide for nurses.

The right to marry and found a family is a fundamental freedom bestowed on adults under the European Convention of Human Rights (Council of Europe 1950). Engaging in sexual activity is an essential component of that right and should be free from state interference. Community nurses, however, often express concern over the possibility that adults with learning disabilities or other impairments of the mind cannot give a real consent to participate in sexual relations and so should be prevented from doing so in their own best interests. In this article Richard Griffith and Cassam Tengnah review t

Nurses must be more confident in assessing Gillick competence.

The recent measles outbreak in south west Wales saw mass immunisation of children who had not received both doses of the measles, mumps and rubella (MMR) vaccine. The most difficult target group to immunise proved to be children in the 14-17 year age group, due in primarily to the reliance on parental consent for the immunisation instead of the child's consent under the rule in Gillick. For future mass immunisation programmes with this age group to be successful, nurses must be more confident in assessing Gillick competence and allow the child to consent to the vaccination. In this article the

Standardised assessment of older patients by a nurse in an emergency department.

The primary aim of this study was to evaluate the ability of a nurse practitioner in geriatrics, working in the emergency department of a tertiary referral hospital, to assess high-risk elderly patients comprehensively. A secondary aim was to explore patient characteristics associated with referral to community aged care services. Of 469 patients assessed by the nurse, 327 (70%) were admitted to the hospital. A comprehensive set of data was obtained for 334 (71%) patients. For 142 patients not admitted, 163 new referrals were made, mostly to the Aged Care Assessment Team. Those referred were m

The role of community nurses in rash examination, assessment and management.

Community nurses play a significant role in the identification, assessment and treatment of rashes and associated skin conditions. It is important for them to be familiar with the location, pattern, appearance and feel of different types of rashes for timely diagnoses and escalation, if required. This article explores the processes of inspection, palpation, diagnostic tools and management strategies relevant to community nursing practice.

Assessing social determinants of health competencies among nurses.

BACKGROUND: Multiple national nursing organizations have advocated for improving health professionals' understanding of social determinants of health (SDOH) to enhance healthcare access, quality, and outcomes. PURPOSE: This project developed a self-assessment questionnaire to evaluate nursing professionals' self-perceptions of competencies in addressing SDOH, as few tools exist to measure this. METHODS: A total of 690 nurses completed the 23-item questionnaire. FINDINGS: In total, 690 nurses answered the SDOH self-assessment questionnaire. Cronbach's Alpha was 0.99. Nurses reported the high

Clinical skills: assessing and treating shock: a nursing perspective.

This article outlines the pathophysiology associated with hypovolaemic, cardiogenic and distributive shock, and discusses how each of these might present clinically in the patient. Nursing assessment of a patient in shock is explored, and the use of tools such as the pulse oximeter is examined. The evidence base for a variety of interprofessional interventions is analysed, including fluid therapies such as blood transfusion, the use of crystalloids and colloids, and drug therapies such as the use of inotropic and vasoactive agents. The nursing role in managing the patient in shock is considere

Assessing for abuse: self-report versus nurse interview.

Estimates are that 1.8 million women are physically abused each year by their husbands. To prevent abuse and promote physical safety, routine assessment is essential. Planned Parenthood of Houston and Southeast Texas added four self-reported abuse assessment questions to their standard intake form completed by all initial and annual-visit clients. After one month of self-report by 477 women, the prevalence of physical abuse was 7.3%. To compare self-report to nurse interview assessment, the same four questions were asked of 300 women coming to the same clinic for initial or annual visits. The

Assessing organizational social context in surgical domains: perspectives of nursing staff and first-line nurse managers and their associations with nurse-focused outcomes.

BACKGROUND: Nurse shortages underscore the need to understand how organizational culture and climate affect nurse-focused outcomes in surgical care. This study analyzed how nursing staff and first-line nurse managers perceive organizational culture and climate in surgical domains and explored the differences in their perceptions. Furthermore, it analyzed how these perceptions and discrepancies correlate with nurse-focused outcomes, such as nursing staff sickness absences and gross turnover. Additionally, the study investigated how unit sizes and care settings influence the relationship between

Instruments for assessing geriatric nursing competence among nursing personnel: protocol for a COSMIN-based systematic review.

INTRODUCTION: Accelerated population ageing has resulted in increasingly diverse and complex care needs among older people, posing a major challenge to healthcare systems worldwide. Nursing personnel represent a core workforce in the delivery of healthcare and nursing services for older people, and their geriatric nursing competence (GNC) is closely associated with the quality of care provided, care outcomes and older people's quality of life. Therefore, accurate assessment of GNC is essential for nursing education, workforce development and quality improvement. Although several instruments ha

Authentic and transformational leadership in nursing: A meta-analysis of their effects on nurse-assessed patient safety and quality of care.

BACKGROUND: Authentic leadership (AL) and transformational leadership (TL) styles contribute to enhancing nurse-assessed patient safety (PS) and improving quality of care (QoC). However, in the identified literature, meta-analytic evidence is limited to a single study regarding the effect of TL style on nurse-assessed QoC. PURPOSE: The study aimed to investigate the effects of AL and TL styles in nursing on nurse-assessed PS and QoC. METHODS: The mean effect size was calculated using the random effects model. Duval and Tweedie's trim-and-fill method was used to analyze publication bias. The

Substance Use Disorder Education: A Statewide Assessment of Core Content in Nursing Schools.

Of individuals who need treatment for substance use disorder (SUD), 12.6% do not receive it. One reason for this failure to treat is an inadequately prepared health care workforce, including nurses. To establish a state-wide baseline of SUD curricular content, we collected information about topics taught and barriers to teaching important topics using an anonymous survey sent to all 55 nursing programs in Indiana. Topics deemed important were taught more frequently, such as opioid withdrawal (60.6%). Lack of expertise was more commonly reported as a barrier than lack of time (25.3% vs. 7.1%).
