Self-assessment of a dialysis unit can provide an effective means of improving or assuring the level of quality the unit provides in delivering safe and effective care. A tool for use by facility personnel was developed and is being used in 12 dialysis units within one dialysis company. This tool has been useful in identifying areas of risk and in assisting units in developing and maintaining survey readiness. This article presents the facility tool and describes how it may be used to assist other dialysis facilities in risk management and quality assurance activities.

Flexible endoscopes used in gastrointestinal procedures possess long, narrow channels that complicate decontamination and high-level disinfection. Infections may occur even with proper execution of all reprocessing stages, particularly as the endoscope ages or in the presence of biofilm. Effective reprocessing improves when the nurses and associates handling endoscopes are knowledgeable and well-trained, have sufficient time to complete all steps correctly, and work in a clean environment equipped with the right tools. Audits evaluate endoscope reprocessing to confirm the presence of necessary

BACKGROUND: At Walsall Healthcare NHS Trust, high referral volumes were overwhelming the community palliative care team, leading to delays in triage and patient contact alongside existing workloads. To improve this, a triage nurse role was introduced to assess and allocate new referrals, supported by a triage tool. This streamlined the review of clinical information and enabled timely telephone contact and appropriate clinical nurse specialist visits.
AIMS: To enhance patient care by systematically evaluating clinical performance and attitudes towards the newly implemented triage tool, as wel

Nursing staff play a crucial role in managing surgical patients in the postoperative period. However, with an increasing risk of subspecialty wards facing closure as a result of financial pressures within the NHS, the knowledge base and expertise of ward-based nursing staff of surgical subspecialties is becoming increasingly limited. Using patients undergoing thyroidectomy as an example, a quality improvement multidisciplinary audit is presented, which shows how simple targeted interventions through a multidisciplinary approach can help maintain high standards of patient care in a secondary ca

Over the last two decades there has been an increasing demand on health care services. This has led to increased waiting lists and waiting times to access public hospital services. In ophthalmic practice an aging population and technological advances have been major contributors to this situation. The challenge for health care providers is how to manage waiting lists within the resources provided and targets set by Elective Services, a department of the Ministry of Health (MoH). The Ministerial Taskforce on Nursing (1998) highlighted that nurses are an underutilised resource. Subsequent to the

BACKGROUND: Almost 90 percent of nursing home residents have some type of mobility limitation. Many spend most of their waking hours lying in bed or sitting. Such inactivity can negatively affect residents' health and general well-being. This pilot study aimed to assess (1) the effect of the sit-to-stand activity on mobility outcomes of nursing home residents, (2) the effect of an audit-and-feedback intervention on uptake of the sit-to-stand activity by healthcare aides, and (3) the contextual factors influencing uptake of the sit-to-stand activity by healthcare aides.

BACKGROUND: Point-of-care testing in the emergency department decreases wait times and supports evidence-based patient care. However, hurdles to successful implementation include management of interdisciplinary work flows and establishment of an effective quality control program. As COVID-19 testing is now integrated into screening protocols in emergency and urgent care settings, hospital systems must maintain flexible and adaptable respiratory virus testing to adapt to regional trends in transmission. In response to this challenge, our hospital system established a point-of-care respiratory v

INTRODUCTION: Health professionals may be susceptible to misusing alcohol due to stress and burnout. This is especially true in states with high alcohol consumption. Health care organizations can implement evidence-based policies, programs, and solutions that identify, address, and help prevent adverse outcomes and burnout for health workers. Recommendations for health care professionals are learning to recognize signs of distress, mental challenges, and burnout. Staying connected and reaching out for help is a start to getting back to basic good health habits instead of developing unhealthy h

Nurses have the potential to provide care for patients with bronchiectasis by assessing needs and co-ordinating input from other health professionals. Bronchiectasis is a disease that is poorly understood by the public and non-specialists. However, by educating both colleagues and patients, nurses can greatly improve care. This article highlights nursing care delivery to people with bronchiectasis and explores the need for further specialist nurse development. It describes the findings of a UK audit undertaken in December 2024, completed by 72 nurses who provide care and treat patients with br

LOCAL PROBLEM: Variations in nursing practice were observed across our hospital, a 520-bed acute care teaching facility in the northeast United States, regarding the timing and frequency of insulin administration in adult patients with diabetes. Chart audits noted that RNs administered insulin more than one hour after blood glucose results were obtained 97% of the time. In addition, insulin was given at bedtime only 37% of the time.
PURPOSE: The purpose of this quality improvement (QI) project was to improve the care of inpatients requiring insulin by implementing protocols and adjusting prac
