A 58-year-old man, received polypectomy to evaluate the polyp of sigmoid colon. Pathological examination of the specimen revealed non-invasive adenocarcinoma. Systemic work up including abdominal computed tomography (CT) demonstrated a bladder tumor 11 x 18 mm in diameter associated with left obturator lymph node swelling (16 x 14 mm). In the preoperative staging, this nodule was suspected to be lymph node metastasis, even though both cancers were diagnosed to be early stage. Transurethral resection of bladder tumor (TUR-Bt) and lymph node open biopsy were performed. The final diagnosis of bla

Compression of a peripheral nerve or nerve trunk can occur during pregnancy and delivery. The injury may be caused by the fetal head, the application of forceps, trauma or hematoma due to cesarean section, or improper positioning in leg holders. Often, no cause of the injury is found. The most common nerve compression syndromes during pregnancy and delivery are carpal tunnel syndrome, femoral neuropathy, and post partal foot drop. Obturator neuropathy, meralgia paraesthetica, tarsal tunnel syndrome, and syndrome of the rectus abdominis muscle occur less frequently. Symptoms, such as paraesthes

BACKGROUND: This report presents a case of bilateral lumbosacral trunk (LST) variation observed during cadaveric dissection and to analyze its possible developmental mechanism and clinical significance.
MATERIALS AND METHODS: During the dissection of a Chinese adult male specimen, bilateral variations of the LST were observed, with communicating branches joining the femoral nerve and/or obturator nerve.
RESULTS: The right LST was formed solely by the ventral ramus of the fifth lumbar nerve (L5), without contribution from the fourth lumbar nerve (L4), and gave off one communicating branch (1.

The purpose of this study was to determine if various lithotomy positions increase strain on the obturator and lateral femoral cutaneous nerves in fresh adult cadavers. A static load cell was used to record strain changes of the obturator and lateral femoral cutaneous nerves in the pelvis and anterior thigh when the lower limbs were placed in three sets of positions of varying hip abduction and flexion. The means of the strain measurements, which were measured in grams in all positioning angles were compared to the baseline 0 degrees measurements. Analysis of variance was calculated for the di

Neurolytic blockade is one of the therapeutic possibilities to treat spasticity of various muscles. In patients with spasticity of the adductor thigh muscles, a percutaneous approach to the obturator nerve is often difficult. We describe a new approach to the obturator nerve and we examine its feasibility. The second objective was to assess the efficacy of obturator neurolysis for the management of adductor thigh muscle pain and spasticity associated with hemiplegia or paraplegia. Nerve blocks were performed via a combined approach using fluoroscopy and nerve stimulation to identify the obtura

Pain and restriction of movements in the hip joint due to coxarthrosis are common causes of patients' visits to their doctors. During the past few years, endoprosthetic surgery has been the most common surgical method for the treatment of this disease. Endoprosthetic operation is contraindicated for some patients in view of their excessive body weight or the presence of concurrent somatic diseases. In order to eliminate pain due to coxarthrosis, we have developed a new operation--percutaneous radiofrequency destruction of the obturator nerve--carried out at the point of its exit from the small

BACKGROUND Neurilemmomas are rare tumors derived from the Schwann cells that comprise the peripheral nerve sheaths. They have a slow growth and rarely display malignancy. Early diagnosis is rare, and the treatment consists by surgical resection. Although robotic-assisted surgery is commonly used for treating retroperitoneal diseases, there are few reports of resection of retroperitoneal and pelvic schwannoma through robotic-assisted surgery. In the present study, we reported a case of complete excision of a benign retroperitoneal schwannoma of the obturator nerve by robotic-assisted surgery. C

BACKGROUND: Cases of obturator nerve impingement (ONI) caused by osteophytes resulting from bone hyperplasia on the sacroiliac articular surface have never been reported. This paper presents such a case in a patient in whom severe lower limb pain was caused by osteophyte compression of the sacroiliac joint on the obturator nerve.
CASE SUMMARY: A 65-year-old Asian man presented with severe pain and numbness in his left lower limb, which became aggravated during walking and showed intermittent claudication. The physical examination revealed that the muscle strength of the left lower limb had de

BACKGROUND: Intraoperative injury of the obturator nerve has rarely been reported in patients with gynecological malignancies undergoing extensive radical surgeries. Irreversible damage of this nerve causes thigh paresthesia and claudication. Intraoperative repair may be done by end-to-end anastomosis or grafting when achieving tension-free anastomosis is not possible.
CASE PRESENTATION: A 28-year-old woman with stage IB cervical cancer underwent fertility-sparing surgery, including conization and bilateral pelvic lymphadenectomy. The left obturator nerve was damaged intraoperatively during p

A four-year-old female spayed Labrador Retriever, which had undergone bilateral triple pelvic osteotomy (TPO) at the age of eight months, was presented with severe progressive shifting pelvic limb lameness for a duration of three months prior to presentation. The dog had multiple episodes of showing signs of excruciating pain, as well as an inability to rise or ambulate, inappetance, and lethargy. Orthopaedic examination revealed severe bilateral pelvic limb muscular atrophy, and signs of severe pain on abduction of the pelvic limbs, on rectal palpation ventrally, and on palpation of the regio

PURPOSE: We developed a transurethral resection system comprising a uniquely designed resectoscope and high frequency wave generator. The obturator nerve is free of electrical stimulation during transurethral resection since high frequency current is delivered via a resection loop to the sheath of the resectoscope in saline irrigation. Preclinical verification of the system was performed in an animal model to ensure its efficacy and operational safety.
MATERIALS AND METHODS: The swine bladder wall was transurethrally resected using the system in a saline environment. Results were subsequently

We report successful restoration of quadriceps function following contralateral obturator to lateral branch of the femoral nerve transfer (with intervening autograft) in two patients with multilevel lumbosacral root avulsions, resulting in severe unilateral motor and sensory deficits. Three years postoperatively, patient 1 had regained Medical Research Council grade 3 knee extension with the ability to extend against 10 pounds of resistance. At 28 months postoperatively, patient 2 had regained Medical Research Council grade 2 knee extension. Treatment options for severe lumbosacral plexus inju

Intraoperative injury of the obturator nerve may occur in gynecologic oncologic procedures when extensive pelvic side wall dissection is performed. In this case, we report an immediate repair of an incompletely transected obturator nerve during robotic-assisted pelvic lymphadenectomy. A 62-year-old gravida 3, para 3 woman was admitted to our clinic for postmenopausal bleeding. The result of an endometrial biopsy was complex endometrial hyperplasia with atypia, and a robotic-assisted laparoscopic hysterectomy was performed. A frozen section of the specimen revealed grade 1 endometrioid adenocar

During transurethral electroresection in the posterolateral bladder neck, trigone and posterior urethra, unintentional contractions of the thigh-adductor muscles may occur due to irritation of the obturator nerve. The sudden displacement of the bladder wall against the cutting loop may cause a perforation of the bladder. The authors describe the topographic relation of the bladder wall to the passage of the obturator nerve in the minor pelvis. The technique of obturator nerve block by local anaesthesia is described and its efficacy is demonstrated in 21 patients.

Nerve transfer is a valid surgical procedure for restoring lower-extremity function after lumbosacral plexus nerve root avulsion. We determined the anatomical feasibility of transferring the obturator and genitofemoral nerves for this purpose. The obturator, genitofemoral and femoral nerves, and the S1 and S2 nerve roots on both sides were exposed in 10 cadaver specimens. We traced all nerves to their origins. The lengths of the obturator and genitofemoral nerves were measured from their origins to their exits from the abdominal cavity. The transverse and longitudinal diameters of all nerves w
