Positive predictive value of fecal occult blood testing in persons taking warfarin.

BACKGROUND: In clinical practice, some physicians discontinue warfarin prior to fecal occult blood testing (FOBT). Although anticoagulant use is associated with an increased risk of overt gastrointestinal bleeding, the impact of warfarin on the positive predictive value of FOBT is unknown. METHODS: During a 5-yr period, we prospectively studied all patients taking warfarin who were referred for the evaluation of a positive FOBT. For each patient taking warfarin, we enrolled one age- and gender-matched control subject with a positive FOBT who was not taking anticoagulants. A detailed clinical

Transmissibility of hepatitis B virus (HBV) infection through blood transfusion from blood donors with occult HBV infection.

BACKGROUND: Studies of the transmissibility of hepatitis B virus (HBV) in occult hepatitis B (OHB) through blood transfusion are scarce. We aimed to determine the transmissibility of HBV in blood donors with OHB through transfusion in animal and human studies. METHODS: Among 217,595 blood donors, 67 donors with OHB were identified. Four chimeric mice populated with human hepatocytes were inoculated with 2 donor serum samples. Serial serum and liver HBV DNA levels were measured. Forty-nine recipients of blood transfusions traced from 10 donors with OHB (9 of whom were positive for antibody to

Diagnostic Indications and Endoscopic Correlates of Fecal Occult Blood Testing in Hospitalized Patients.

Background: FOBT is widely used for colorectal cancer screening in asymptomatic populations but is frequently applied in hospitalized patients with anemia or suspected gastrointestinal bleeding despite limited supporting evidence. Objectives: To evaluate the clinical utility of fecal occult blood testing (FOBT) in hospitalized patients, focusing on its indications and its association with endoscopic findings while accounting for differential endoscopic verification. Methods: We conducted a retrospective study of 160 hospitalized adults who underwent FOBT at a medium-complexity referral center.

Digital rectal examination versus spontaneous passage of stool for fecal occult blood testing.

BACKGROUND: The diagnostic value of a positive fecal occult blood test (FOBT) at the time of digital rectal examination (DRE) is disputed despite being used commonly by a significant number of physicians. A meta-analysis was conducted to evaluate FOBT by DRE for detecting neoplasia versus FOBT on stool passed spontaneously (SPS) in asymptomatic patients undergoing colorectal cancer screening. METHODS: MEDLINE, the Cochrane Central Register of Controlled Trials and the Cochrane database of systematic reviews, CINAHL, PubMed, and recent abstracts from major conferences were searched in August 2

Transient occult hepatitis B virus infection in a blood donor with high viremia.

BACKGROUND: Screening of blood donors for viral nucleic acids has recently been introduced in several countries. With the use of transcription-mediated amplification, a blood donor was detected who had 90,000 copies of hepatitis B virus (HBV) DNA/mL but no hepatitis B surface antigen (HBsAg) or antibody to hepatitis B core antigen (anti-HBc). One month later, anti-HBc and hepatitis B surface antibody (anti-HBs) appeared; HBV DNA disappeared after 2 months. This study asked why HBsAg was undetectable in this rare case of transient occult HBV infection. STUDY DESIGN AND METHODS: The HBV DNA in

Lower specificity of occult-blood test on stool collected by digital rectal examination.

BACKGROUND/AIMS: To compare the diagnostic accuracy of an immunochemical fecal occult-blood test for colorectal cancer between the stool specimens obtained during the routine screening and those during the digital rectal examination. METHODOLOGY: One hundred and fourteen patients with colorectal cancer and 228 healthy controls served as subjects of the study. Fecal occult-blood was tested by both of two methods; by the routine screening and by the digital rectal examination, and the sensitivity and specificity of an immunochemical fecal occult-blood test were determined in these two methods.

Distribution of large-bowel cancers detected by occult blood test in asymptomatic patients.

A randomized, controlled study on occult blood in stool was conducted in 48,000 asymptomatic participants, ages 50 to 80 years. Those with positive hemoccult test results underwent a complete colonic workup including colonoscopy and, in some cases, an air-contrast barium-enema examination when total colonoscopy could not be accomplished. From April 1976 to December 1980, 113 patients (62 men and 51 women), with an average age of 66 years, were found to have invasive carcinomas of the large bowel: right colon--25 per cent, transverse colon (included both flexures)--9 per cent, descending colon-

Occult Blood and Perianal Examination: Value Added in Pediatric Inflammatory Bowel Disease Screening.

OBJECTIVES: Pediatric inflammatory bowel disease (IBD) often presents insidiously and standard blood tests are normal in 20% of patients. We hypothesize that fecal occult blood testing (FOBT) and the perianal examination in addition to blood tests provide important information during the screening process for IBD. The aim of the present study was to measure the diagnostic value of adding FOBT and perianal examination to standard screening laboratories in evaluating children and adolescents for IBD. METHODS: The medical records of consecutive patients undergoing ileocolonoscopy for IBD were re

Immunochemical fecal occult blood tests predict dual antiplatelet therapy discontinuation after coronary stenting.

OBJECTIVE: The discontinuation of dual antiplatelet therapy (DAPT) increases the risk of stent thrombosis after coronary stenting. Some patients must discontinue DAPT due to gastrointestinal (GI) tract disease; however, the type of examination that is most useful for detecting GI tract diseases has not been fully evaluated. The purpose of this study was to clarify whether the immunochemical fecal occult blood test (iFOBT) can be used to predict GI tract disease-related DAPT discontinuation following stent implantation in patients with coronary artery disease. METHODS: A total of 181 consecuti

Molecular characteristics of occult hepatitis B virus from blood donors in southeast China.

The characteristics of 30 carriers with occult hepatitis B virus (HBV) infection (OBI) were compared with those of 30 individuals diagnosed as being HBV carriers at the time of blood donation, 60 asymptomatic carriers, and 60 chronic hepatitis patients. The prevalence of genotype C was significantly higher in carriers with OBIs than in any other HBsAg-positive (HBsAg(+)) group (P < 0.001). Specific amino acid substitutions in the regions from amino acids 117 to 121 and amino acids 144 to 147 located in the major hydrophilic region of the S gene were associated with carriers with OBIs (P < 0.01

Screening colonoscopy in asymptomatic average-risk persons with negative fecal occult blood tests.

Colonoscopy was performed on 210 asymptomatic average-risk persons, aged 50-75 years, who had negative fecal occult blood test results. Colonoscopy was complete to the cecum in 209 subjects. Fifty-three subjects (25%) had adenomas and two had cancer. All of the adenomas greater than or equal to 1 cm in size and both cancers occurred in subjects aged greater than or equal to 60 years. Fifty-one percent of subjects with adenomas and one with cancer had no neoplasms distal to the sigmoid-descending colon junction. One subject had a major postpolypectomy hemorrhage that stopped spontaneously. Scre
