The option of Linac-based radiosurgery in a Gamma Knife radiosurgery center.

OBJECTIVE: Due to the fundamental differences in treatment delivery, linear-accelerator-based radiosurgery can be complementary to Gamma Knife (GK) for intracranial lesions. We reviewed the effect of adding GK to an existing linear accelerator (Linac)-based radiosurgery practice and analyzed case selections for the two modalities. PATIENTS AND METHODS: UC Davis Medical Center installed a Leksell Gamma Knife Model C in October 2003 to supplement an established Linac-based radiosurgery program. Radiosurgery indications for the 15 months before and after installation were compared. RESULTS: Rad

Hepatitis C virus transmission in a pediatric oncology ward: analysis of an outbreak and review of the literature.

Hospital-related hepatitis C virus (HCV) infections continue to occur even after the introduction of blood donor screening. We report an outbreak of HCV in nine patients of a pediatric oncology ward in 1996/1997. Sequencing of the hypervariable genomic region 1 (HVR1) of the E2/NS1 region showed near identity between HCV isolates from these patients as evidence for infection with the same virus. Despite a detailed and careful investigation, the source of infection and the mode of virus transmission could not be established. Based on a review of the current literature about nosocomial HCV infec

The Risk Factors of Readmission in Postoperative Gynecologic Oncology Patients at a Single Institution.

INTRODUCTION: Hospital readmission rates are an important measure of quality care and have recently been tied to reimbursement. This study seeks to identify the risk factors for postoperative readmission in patients treated by a gynecologic oncology service. METHODS: A 7-year retrospective review (2007-2013) of all patients operated on by the University of Virginia gynecologic oncology service who were readmitted within 30 days of discharge was performed. Abstracted data included demographics, dates of surgery, operative details, cancer history, and relevant medical history. The readmitted pa

Establishment of Integrated Hospital-Based Supportive and Palliative Services: An Experience from Iran.

Integrating supportive and palliative care into the oncology program is considered essential for effective cancer management. Despite the challenges that low- and middle-income countries such as Iran face in providing comprehensive supportive and palliative care services, continuous efforts are directed towards improving services and infrastructure. In this regard, the Iranian Cancer Control Center (MACSA) plays a fundamental role in providing supportive and palliative care with special emphasis on expanding services and ensuring equitable access for cancer patients in Iran. Through joint effo

The Impact of an Oncology Hospital at Home Program on Health Care Costs.

INTRODUCTION: Hospital-at-home is an initiative to move health care services that have traditionally been provided in a hospital setting to a patient's home. The objective of this study was to assess the impact of the oncology Huntsman at Home (HH) hospital-at-home program on health care costs from the health care system's perspective. METHODS: Using a difference-in-difference approach, we compared health care costs between 169 oncology patients enrolled in HH and 198 similar patients who would have been eligible for HH but lived outside the HH service area. Costs were measured from the healt

A new palliative care consultation team at the oncology department of a university hospital: an assessment of initial efficiency and effectiveness.

PURPOSE: Palliative care consultation teams (PCCT) in acute hospitals have increased in number over recent years. To assess whether these teams are both efficient in their role within a palliative care centre and effective in the care that is provided for patients, we reviewed the initial activity of a new PCCT at the Oncology Department of Clínica Universidad de Navarra, a European Society for Medical Oncology-designated centre of integrated oncology and palliative care. METHODS: All patients evaluated by the PCCT over the first 3 years of its activity were included. Data about diagnosis, de

The European study on centralisation of childhood cancer treatment.

BACKGROUND: It is generally agreed to centralise treatment of childhood cancers (CCs). We analysed (1) the degree of centralisation of CCs in European countries and 2) the relations between centralisation and survival. PATIENTS AND METHODS: The analysis comprised 4415 CCs, diagnosed between 2000 and 2007 and followed up to the end of 2013, from Belgium, Bulgaria, Finland, Ireland, the Netherlands and Slovenia. All these countries had national population-based cancer registries and were able to provide information on diagnosis, treatment, treatment hospitals, and survival. Each case was then c

[Prevalence of psychiatric disorders and factors associated with delirium in patients referred to a psycho-oncology service].

The charts of 628 cancer patients, referred to the psycho-oncology service at the Hôtel-Dieu de Québec from 1994 to 1997, were reviewed in 1997 to determine the nature, the prevalence, and the characteristics of psychiatric diagnoses, particularly delirium. Results showed that among patients diagnosed with a psychiatric disorder, 54.5% had adjustment disorders, 13.1% suffered from delirium and 10.9% from major depression. Statistically significant associations were observed between delirium and the use of opioids (odds-ratio (OR): 3.87, 95% confidence interval (CI) = 2.28-6.57), corticosteroid

Transparency, Repricing, and Price Convergence in Cancer Care.

OBJECTIVES: Cancer imposes a significant economic burden on the US healthcare system and its patients. We quantified changes in price levels and variation for oncologic services after federal price transparency regulations and evaluated whether the prevalence and granularity of disclosures were associated with these changes. METHODS: This retrospective longitudinal study used nationwide hospital price transparency data from December 2021 to June 2024. The data set included billing codes across 4 oncology service categories (inpatient, chemotherapy administration, radiation, and surgery). A li

Capacity to care: Provider agency and socio-structural constraints in breast cancer care across public secondary and tertiary hospitals in Soweto.

Timely breast cancer care remains challenging in South Africa, where both health-system and patient-level barriers contribute to delayed diagnosis and treatment. This study explored healthcare providers' experiences and perspectives on breast cancer care at Chris Hani Baragwanath Academic Hospital in Soweto and its two referring secondary hospitals in Johannesburg. Using a cross-sectional qualitative design informed by phenomenology, we conducted in-depth interviews with 30 providers involved in breast cancer care. Thematic analysis identified intersecting structural and individual barriers, t

Study of completeness of the surveillance, epidemiology and end results (SEER) program case ascertainment by hospital size and casefinding source.

Factors that may enter into the completeness/incompleteness of reporting include: 1. The competence and diligence of staff. 2. Familiarity with the record systems used within each hospital department. This limited review suggested that hospitals employing their own staff for case ascertainment achieved somewhat higher completeness of reporting rates compared with institutions that relied on central registry circuit riders to pick up cases. 3. Where cases are identified through computer listings by diagnosis, errors in the coding of some cases may result in the exclusion of these cases. 4. The

Five-year-survey of invasive aspergillosis in a paediatric cancer centre. Epidemiology, management and long-term survival.

The epidemiology, management, and long-term survival of invasive aspergillosis was assessed in a prospective, 5-year observational study in 346 unselected paediatric cancer patients receiving dose-intensive chemotherapy for newly diagnosed or recurrent malignancies. Invasive aspergillosis occurred exclusively in the context of haematological malignancies, where it accounted for an incidence of 6.8% (n = 13 of 189). The lung was the primary site in 12 cases, and dissemination was present in three of those. Prior to diagnosis, the overwhelming majority of patients had been profoundly neutropenic
