BACKGROUND: The retroperitoneal approach preserves the peritoneal cavity and its envelope. The benefits of laparoscopic retroperitoneal approach to the kidney and the adrenal gland are well known. The pancreas may represent another potential target. Whereas NOTES transperitoneal distal pancreatectomy has been reported, the retroperitoneal approach, which may combine the advantages of peritoneal preservation with those of no scar surgery, has never been explored. We report the feasibility of NOTES transvaginal retroperitoneal pancreatectomy in a porcine model.

Central pancreatectomy (CP) is a segmental pancreatic resection indicated to remove benign or low-grade malignant tumors of the isthmus and proximal part of the body of the pancreas. The main advantage of this operation compared with major resections is that it permits to spare normal pancreatic parenchyma; moreover, spleen and upper digestive and biliary tracts are saved. The description of the complete operation was reported for the first time by Dagradi and Serio in 1984 and subsequently spread worldwide by Iacono and Serio. In our opinion, it should be called the Dagradi-Serio-Iacono opera

BACKGROUND/AIMS: Laparoscopic pancreatic surgery has now developed as a clinical practice. In urological surgery, direct extraperitoneal surgery has become popular in laparoscopic adrenalectomy and nephrectomy. As the pancreas is also an organ located in the retroperitoneal cavity, we evaluate the efficacy of retroperitoneoscopic laparoscopic distal pancreatectomy.
METHODOLOGY: Specific-pathogen-free Large Yorkshire pigs were studied. No major bleeding episodes were encountered. After carbon dioxide insufflation, the abdominal viscera were inspected with a laparoscope. Below the left costal c

BACKGROUND: Limited middle segment pancreatectomy, or central pancreatectomy, has been described for sparing normal pancreatic tissue during resection of benign neoplasms of the pancreatic neck. Anatomic reconstruction after central pancreatectomy has been reported in other series with creation of a Roux-en-Y loop of jejunum for a mucosa-to-mucosa pancreaticojejunostomy.
STUDY DESIGN: Hospital charts and outpatient records were reviewed for 12 consecutive patients undergoing central pancreatectomy from August 1999 to November 2002.
RESULTS: We performed central pancreatectomy with pancreatic

The incidence of pancreatic fistula in a series of 178 pancreatic resections was 25 per cent and was influenced by the type of pancreatectomy, the management of the remaining pancreas and the size of the pancreatic duct. Thus, fistulas appeared in 52 per cent of patients undergoing pancreatoduodenectomy as compared to only 7 per cent of those undergoing distal pancreatectomy (P less than 0.001). Ligation of the pancreatic, duct in pancreatoduodenectomy led to a 70 per cent incidence; the incidence was 33 per cent if the pancreas was anastomosed to the jejunum instead (P less than 0.001). Fistu

BACKGROUNDS: Distal pancreatectomy fistula risk score (D-FRS) and DISPAIR-FRS has not been widely validated for predicting postoperative pancreatic fistula (POPF) after distal pancreatectomy (DP).
METHODS: We retrospectively analysed 104 patients undergoing DP. The predictive value of the D-FRS and DISPAIR-FRS were compared. Risk factors associated with POPF were investigated by multivariate analysis.
RESULTS: Of the 104 patients, 23 (22.1%) were categorized into the POPF group (all grade B). The areas under the ROC (AUCs) of the D-FRS (preoperative), D-FRS (intraoperative), and DISPAIR-FRS

Recently, the number of case reports detailing cancer recurrence in the pancreatic remnants, following surgical resection of intraductal papillary-mucinous carcinoma (IPMC) of the pancreas has increased. We report the case of a 74-year-old woman who underwent pancreatic resection twice in a 3-year period for primary IPMC and remnant pancreatic ductal carcinoma. We first performed distal pancreatectomy for branched IPMC in the pancreatic tail. Histopathological examination revealed invasive IPMC and the negative margin of the pancreatic duct. The expression of tumor markers gradually increased

OBJECTIVE: To analyze the most well-known studies devoted to completion pancreatectomy (CP) for postoperative complications after pancreatoduodenectomy.
MATERIAL AND METHODS: We analyzed original articles and reviews between 1992 and 2023 (number of patients ≥5 (5-120)).
RESULTS: Mean blood loss in CP ranged from 500 to 2180 ml, surgery time - from 144 to 340 min. Along with standard postoperative complications (fluid collection, abscesses, bile leak and bleeding), patients had high risk of venous gastric congestion. The incidence of relaparotomies ranged from 7% to 63% with postoperative mo

BACKGROUND: Several factors affect the level of difficulty of laparoscopic distal pancreatectomy (LDP). The purpose of this study was to develop a difficulty scoring (DS) system to quantify the degree of difficulty in LDP.
METHODS: We collected clinical data for 80 patients who underwent LDP. A 10-level difficulty index was developed and subcategorized into a three-level difficulty index; 1-3 as low, 4-6 as intermediate, and 7-10 as high index. The automatic linear modeling (LINEAR) statistical tool was used to identify factors that significantly increase level of difficulty in LDP.
RESULTS:

Total remnant pancreatectomy after pancreaticoduodenectomy (PD) is a difficult procedure. Recently, distal pancreatectomy and PD have been performed laparoscopically. Herein, we present the first case report of a laparoscopic total remnant pancreatectomy. A 72-year-old woman underwent a totally laparoscopic pylorus-preserving PD for inferior bile duct cancer. The tumor was composed of moderately differentiated tubular adenocarcinoma and was diagnosed as pStage III according to the UICC-TNM classification. Eighteen months later, CT showed a low-density mass in the remnant pancreas. We conducted

BACKGROUND: The morbidity of pancreatoduodenectomy remains high and the mortality may be significantly increased in high-risk patients. However, a method to predict post-operative adverse outcomes based on readily available clinical data has not been available. Therefore, the objective was to create a 'Pancreatectomy Risk Calculator' using the American College of Surgeons-National Surgical Quality Improvement Program (ACS-NSQIP) database.
METHODS: The 2005-2008 ACS-NSQIP data on 7571 patients undergoing proximal (n = 4621), distal (n = 2552) or total pancreatectomy (n = 177) as well as enucle

BACKGROUND: Distal pancreatectomy (DP) is frequently performed with splenectomy, although splenectomy can be linked to various untoward effects, and sparing of the main splenic vessels (SVs) is not necessary for successful spleen preservation (SP).
METHODS: We reviewed all DPs in a single-surgeon practice to assess feasibility and outcomes of SP.
RESULTS: Between 1997 and 2007, 41 of 177 pancreatic resections involved a DP (23%). There were 14 men (34%) and 27 women (66%), with a median age of 60 years (range: 34-86). Four procedures were en bloc resections, two total pancreatectomies, while

Standard total pancreatectomy (TP) combined with gastric resection often results in uncontrollable diabetes and malnutrition. Pylorus-preserving total pancreatectomy (PPTP) and standard TP for pancreatic cancer were compared in terms of operative outcomes, nutritional recovery, and long-term survival. Twenty-four patients with pancreatic ductal adenocarcinoma (n = 14) or intraductal papillary mucinous carcinoma (N = 10) underwent PPTP (n = 10) or standard TP (n = 14). There were no significant differences in age, gender, or tumor type or stage between the PPTP and standard TP groups. Early (wi

Significant complications continue to occur in a minority of patients undergoing pancreaticoduodenectomy; these frequently have fatal consequences. Analysis of 458 patients undergoing the Whipple procedure in this institution from 1972 to 1994 revealed that 16 patients with malignant periampullary tumours (ampullary, eight; pancreatic, seven; bile duct, one), and one patient with chronic pancreatitis subsequently required completion pancreatectomy. Postoperative difficulties after pancreaticoduodenectomy and indications for re-exploration were multifactorial: leakage (n = 8), pancreatitis (n =

BACKGROUND: This study evaluated the clinical features of patients with isolated recurrence of pancreatic ductal adenocarcinoma in the remnant pancreas after initial pancreatectomy. It specifically analyzed the postoperative and oncologic outcomes of patients who underwent completion total pancreatectomy to identify prognostic factors associated with 5-year survival.
METHODS: A retrospective review was conducted on 93 patients with isolated recurrence of pancreatic ductal adenocarcinoma in the remnant pancreas after initial pancreatectomy between 2009 and 2020. Of these, 59 patients underwent

BACKGROUND: The value of spleen preservation during distal pancreatectomy (DP) still remains controversial. Spleen-preserving DP with excision of the splenic artery and vein is a simplified technique for spleen preservation. The aim of this study was to compare the postoperative course of DP with or without splenectomy.
PATIENTS AND METHODS: From 1990 to 2005, 38 consecutive patients with benign or low-grade malignant disease underwent a spleen-preserving DP operation with excision of the splenic artery and vein (Conservative Group). They were compared with 38 patients who underwent conventio

BACKGROUND: This retrospective study evaluates the safety and cost-effectiveness of robotic distal pancreatectomy (RDP) during surgeons' early experience, a period for which data remain limited. We compared outcomes and costs of RDP versus open distal pancreatectomy (ODP) during the initial phase of our robotic program.
METHODS: RDP and ODP cases from January 2018 to May 2024 were analyzed using a prospective database to obtain clinicopathologic and 90-day outcomes. Costs were expressed as ratios to ODP operative cost. 1:1 propensity-matched cohorts were compared.
RESULTS: One hundred ninete

Adenocarcinoma of the body and tail of the pancreas is an aggressive malignancy, and classically there have been few survivors after surgery. Radical antegrade modular pancreatosplenectomy and distal pancreatectomy with celiac axis resection are new procedures for these tumors. Radical antegrade modular pancreatosplenectomy is designed to establish an operation with oncologic rationales both for the dissection planes used to achieve negative margins and the extent of node dissection. The extent of lymph node dissection is based on the descriptions of N1 lymph node drainage, and dissection plan
