Advanced Care Planning in Parkinson's Disease: In-depth Interviews With Patients on Experiences and Needs.

Introduction: Advance care planning (ACP) is an iterative process of discussing the needs, wishes, and preferences of patients regarding disease-specific and end-of-life issues. There is ample evidence that ACP improves the quality of life and promotes the autonomy of patients with cancer and motor neuron disease who have a high disease burden and shortened life expectancy. In Parkinson's disease (PD) though, knowledge about the experiences and preferences of patients regarding ACP is scarce, despite the major disease burden associated with PD. Aim: This study aims to explore the experiences,

A Service-oriented Framework for Developing Personalized Patient Care Plans for COVID-19.

The COVID-19 pandemic has identified weaknesses and stresses in the existing healthcare and governance system, even in the most developed countries. Given the scale and scope of the pandemic, existing healthcare systems are heavily resource constrained, and home-based isolation has been considered as a potential first step for reducing both the disease spread and the stress on the healthcare system. However, the needs and requirements of home-based isolation are extremely unique for each patient, depending on their medical condition and comorbidities, family responsibilities, and environmental

Pilot implementation of an electronic patient-reported outcome measure for planning and monitoring participation-focused care in early intervention.

BACKGROUND: Family-centered care is a valued approach to improving child and family outcomes in early intervention (EI), yet there is need to implement interventions that support information exchange for shared decision-making when planning and monitoring EI care. This study aims at estimating the feasibility, acceptability, and value of implementing the Young Children's Participation and Environment Measure (YC-PEM), a valid electronic patient-reported outcome (e-PRO) that is designed to support family engagement when planning care and monitoring outcomes of care. METHODS: Data were gathered

Feasibility of LLM-assisted post-discharge tuberculosis care: A comparative study of medication counseling, patient education, and follow-up planning.

BACKGROUND: Tuberculosis (TB) remains a major global health concern, with poor post-discharge adherence driving relapse and drug resistance. While China's 2024 guidelines advocate for "intelligent post-discharge management," the role of Large Language Models (LLMs) in this specialized care remains unexplored. OBJECTIVE: To explore the feasibility of LLMs as auxiliary tools to complement TB specialists in post-discharge medication counseling, patient education, and follow-up planning. METHOD: This exploratory study compared two LLMs (ChatGPT-4o, DeepSeek-R1) with TB physicians using 17 standa

End-of-life care planning for ethnically diverse kidney patients in the COVID-19 era.

BACKGROUND: End-stage kidney disease (ESKD) is most prevalent amongst people from minority ethnic groups and people receiving haemodialysis were considered to be ‘clinically extremely vulnerable’ during the Covid- 19 pandemic. Covid-19 impacted the health of those from ethnically diverse backgrounds disproportionally. The Covid-19 pandemic brought death and dying to the forefront of public discourse, yet few studies have explored the impact of the Covid-19 pandemic on individual’s understanding of and willingness to engage in end-of-life care planning conversations. In addition, few studies ha

Detailed Dietary Assessment in Patients with Inoperable Tumors: Potential Deficits for Nutrition Care Plans.

Advanced cancer often results in reduced dietary intake; however, data on actual intake at the time of diagnosis are limited. In the present study, a detailed dietary intake assessment was performed in patients with metastatic lung and upper gastrointestinal cancer, before initiation of systemic therapy. Basic demographics and performance status (PS) were recorded. Nutritional status was evaluated through anthropometry, Mini Nutritional Assessment (MNA), and 3 nonconsecutive 24-hour dietary recalls. Of the 84 patients enrolled, 61.4% were protein, energy, or protein-energy undernourished, rega
