Efficacy of peritoneovenous shunt for treating tolvaptan-resistant refractory ascites in a cirrhotic patient with portal vein thrombosis: A case report.

Peritoneovenous shunt is normally used for the treatment of refractory ascites. However, its efficacy in treating tolvaptan-resistant refractory ascites has not been reported thus far. In addition, the impact of peritoneovenous shunt on the prognosis of cirrhotic patients remains controversial. In the present report, a case of tolvaptan-resistant refractory ascites associated with liver cirrhosis and portal vein thrombosis is described. The male patient was diagnosed with hepatitis C virus-related liver cirrhosis at the age of 51 years. At the age of 56 years, the patient developed portal vein

Treatment of Refractory Chylous Ascites with an Innovative Peritoneovenous Shunt: Temporary Usage of a Continuous Renal Replacement System: A Case Report.

Esophagectomy and esophageal reconstruction are commonly chosen as surgical options for esophageal cancer. However, prolonged untreated chyle leakage is associated with a poor prognosis. We report the case of a patient with refractory chylous ascites. To limit the ongoing fluid loss, we utilized the chylous ascites as an additional fluid source in a renal replacement therapy system. A continuous renal replacement therapy (CRRT) drainage system was modified to drain both the chylous ascites and venous blood. The ascites drainage rate was determined empirically and regulated by a dial-flow exten

Efficacy of the Peritoneovenous Shunt as Palliative Care for Patients with Advanced Cancer with Refractory Ascites: A Randomized Controlled Trial (JIVROSG-0803 Study).

PURPOSE: A randomized controlled trial (UMIN-CTR 000003469) was conducted to evaluate the superiority of peritoneovenous shunt over other treatments for refractory ascites in patients with advanced cancer. METHODS: The study included patients with advanced cancer with symptoms due to refractory ascites. They were randomly allocated in a 1:1 ratio to receive either peritoneovenous shunt placement (test group) or another treatment (control group). The area under the curve of the symptom improvement score was compared between the groups as the primary endpoint. The areas under the curve of the E

Transcatheter arterial embolization for postoperative hemorrhage complicating surgical repair of incarcerated umbilical hernia subsequent to Denver peritoneovenous shunting: A case report.

A 50-year-old man with a refractory ascites was inserted a peritoneovenous shunt under local anesthesia. On the fifth postoperative day, abdominal pain occurred and were diagnosed as incarcerated umbilical hernia. Due to unsuccessful manual reduction, emergent hernia repair was performed. Postoperatively, wound bleeding was not controlled, and endovascular treatment was planned because enhanced computed tomography detected arterial extravasations. Bilateral inferior epigastric arteries were embolized with a 33.3% n-butyl-2-cyanoacrylate lipiodol mixture. The patient's symptoms subsequently imp

Long-term results of peritoneovenous shunt implantation for refractory ascites in patients with alcoholic liver cirrhosis: single-center retrospective study of 348 patients.

INTRODUCTION: Despite its reasonable pathophysiological basis, peritoneovenous shunt (PVS) implantation currently plays a very limited role in managing patients with refractory ascites. On the other hand, we have 20 years of experience using PVSs. The aim of this study was to evaluate the clinical efficacy and long-term outcomes of PVS implantation for the treatment of refractory ascites caused by alcoholic liver cirrhosis. METHODS: We conducted a retrospective review of 348 consecutive patients who underwent PVS placement. Survival data were compared in subgroups according to: (1) severity o

Implantation of a Denver shunt by left subclavian approach for preventing kinking.

Ascites often contributes to patient morbidity and discomfort. Implantation of a Denver peritoneovenous shunt is one of the treatments for refractory ascites. However, the dysfunction of the shunt is frequently observed and requires further improvement. Here, the present paper reported the improved implantation technique for the Denver peritoneovenous shunt. All the procedures were performed in the operating suite under general anesthesia. In all patients, the shunts were percutaneously placed via the left subclavian vein approach under digital fluoroscopic guide along with ultrasound. Fourtee

Treatment of hepatorenal syndrome.

Five patients with hepatorenal syndrome were treated with a LeVeen peritoneovenous shunt and furosemide. Four of the five patients responded immediately with urinary volumes in excess of 2 liters on the day of surgery and the subsequent 3 postoperative days. Urinary sodium excretion increased from 5 to 122 mEq/liter after insertion of a peritoneovenous shunt. Body weight decreased by 7.7 kg in 1 week. Serum creatinine decreased from 4.0 to 1.8 mg/dl in 1 week. There were two long-term survivors. Peritoneovenous shunting is an effective method of reversing the renal component in the hepatorenal

Malposition of venous end of Le Veen shunt: a preventable complication.

The Le Veen peritoneovenous shunt for the treatment of refractory ascites is a useful adjunct in the management of these cases. For proper shunt function the venous end of the shunt must lie within the superior vena cava. This case report illustrates how an unsuspected malposition of the venous end of the shunt was discovered and documented. An alternate means of inserting the venous end of the Le Veen shunt that will guarantee that it lies in the superior vena cava is suggested.

Role of Interventional Radiology in Complications of Portal Hypertension.

Portal hypertension, a consequence of end-stage liver disease and liver cirrhosis can lead to significant morbidity and mortality for patients through abnormal fluid accumulation as well as the formation of portosystemic shunts and varices. Treatment of the sequelae of portal hypertension can be achieved through endovascular management by referral to an interventional radiologist on an outpatient or emergent basis as required. Current techniques include the placement of peritoneovenous shunts and tunneled peritoneal drains, the creation of transjugular intrahepatic portosystemic shunts, or the

Sterile cerebrospinal fluid ascites presenting as high SAAG ascites: a case report.

BACKGROUND: Cerebrospinal fluid ascites is a rare complication of ventriculoperitoneal shunting and is the result of infection and subsequent peritonitis in the majority of cases. Sterile cerebrospinal fluid ascites in which no known infectious etiology is identified, is even more unusual. CASE PRESENTATION: A 26-year-old female with Loeys-Dietz syndrome and congenital hydrocephalus treated with a ventriculoperitoneal shunt, was evaluated after developing new-onset ascites of unclear etiology after abdominal surgery for repair of an aortic aneurysm requiring multiple therapeutic paracenteses.

Hepatoatrial anastomosis and tricuspid valve replacement in Budd-Chiari syndrome 8.

The rare complication of tricuspid valve stenosis due to displacement of a peritoneovenous shunt catheter developed in a patient with Budd-Chiari syndrome. Surgical management included shunt removal and tricuspid valve replacement combined with hepatoatrial anastomosis. The tricuspid valve was replaced with a bioprosthesis, and liver resection opened the stenotic confluence of the hepatic veins. The right atrium was sutured to the liver capsule, thereby restoring physiologic hepatic venous blood flow.

[Peritovenous shunt (Denver Shunt) in therapy resistant ascites in childhood].

This paper describes a 14 year old girl with massive, intractable ascites. After implantation of a peritoneovenous shunting-system (Denver-Shunt) the ascites completely disappeared and the girl's state of health improved markedly. Although the surgical procedure is simple many problems may arise postoperatively which demand intensive monitoring. As there is almost no experience in childhood a shunting-system should only be employed if medical therapy has failed over a long period and respiration, mobility or nutrition of the child is considerably affected by the ascites.

Removal of Peritnoeo-venous-atrial shunt thrombus without cardiopulmonary bypass.

Thrombus formation is not uncommon in longstanding intracardiac catheters, but formation of a thrombus at the tip of a Peritnoeo-venous-atrial shunt, causing obstruction of the tricuspid valve, is a rare complication and frequently unrecognized. A large intracardiac thrombus causing valve obstruction requires surgical removal with the support of cardiopulmonary bypass which is associated with significant morbidity. We successfully removed a thrombus attached to the tip of peritoneovenous shunt without cardiopulmonary bypass in a 25-year-old man.

Hepatorenal syndrome: resolution after transjugular intrahepatic portosystemic shunt.

Hepatorenal syndrome (HRS), a functional renal failure associated with severe liver disease, is believed to result from diminished effective arterial volume and altered renal hemodynamics. Death is almost inevitable after its development and therapeutic options are limited. Surgical shunts carry an increase in morbidity and mortality, and peritoneovenous shunts are frequently complicated by infection and coagulopathy. We report a case of hepatorenal syndrome successfully treated with transjugular intrahepatic portosystemic shunt (TIPS). This is an unusual therapeutic option for HRS and may lea

Budd-Chiari syndrome. A report of 3 cases.

Three patients with the Budd-Chiari syndrome are presented. This is a rare condition characterized by hepatomegaly, progressive and refractory ascites, distension of the abdominal wall veins, abdominal pain and leg oedema. These features are attributed to congestion of the liver and portal hypertension. The condition has been notoriously difficult to treat medically. Surgical measures are directed towards relieving the liver congestion and lowering pressure in the portal system by portal-systemic shunting operations. In some cases refractory ascites may be treated by peritoneovenous shunting w
