Surgical approach to a giant fibrolipoma of the supraglottic larynx.

This paper describes the surgical procedures for a fibrolipoma that first appeared as a giant tumour in the hypopharynx and extended to the cardiac antrum of the oesophagus. At the initial surgery, a pedunculated tumour originating from the left arytenoid of the larynx was found to occupy the cervical as well as thoracic oesophagus and was thus removed through a lateral pharyngectomy. A histological examination revealed fibrolipoma. However there was a recurrence of the tumour in the arytenoid and the patient suffered from dysponea. In addition, a submucosal tumour was also found in the left f

[Traditional transcutaneous approaches in head & neck surgery].

The treatment of laryngeal and hypopharyngeal malignancies remains a challenging task for the head and neck surgeon as the chosen treatment modality often has to bridge the gap between oncologically sound radicality and preservation of function. Due to the increase in transoral laser surgery in early tumor stages and chemoradiation in advanced stages, the usage of traditional transcutaneous approaches has decreased over the recent past. In addition, the need for a function-sparing surgical approach as well as highest possible quality of life has become evident. In view of these facts, rational

Synchronous perforations of the oesophagus and stomach by air insufflation: an uncommon complication of endoscopic dilation.

A 72-year-old woman had a history of carcinoma of the hypopharynx treated by total laryngectomy, circumferential pharyngectomy and free jejunal graft. Endoscopic dilation of the pharyngojejunal anastomotic stricture resulted in synchronous perforations of the oesophagus and stomach. We postulate that the perforations were caused by high intraoesophageal and intragastric pressure resulted from air insufflation during the procedure; in a situation simulating closed-loop obstruction, because of proximal obstruction by the endoscope at the stricture site and distal obstruction by pylorospasm. Jejunal interposition for repair of stricture or fistula after laryngectomy.

Complications following total laryngectomy may include pharyngocutaneous fistula or pharyngeal stricture. Traditional techniques of repair of fistula and stricture with local or regional flaps lead to a high rate of failure. In this study, we report 18 patients treated by secondary jejunal interposition (JI) to rehabilitate swallowing following recalcitrant postlaryngectomy stricture or fistula. All patients had undergone total laryngectomy with or without partial pharyngectomy for treatment of squamous cell carcinoma of the larynx (8) or hypopharynx (10). Four were stage II; 5, stage III; and

Novel techniques for prevention of post-operative pharyngocutaneous fistula in locally advanced laryngeal and hypopharyngeal cancers.

BACKGROUND: Pharyngocutaneous fistula is one of the most common complications following total laryngectomy. It increases hospital stay and the financial burden on patients, and prolongs nasogastric feeding. This paper presents novel techniques for prevention of pharyngocutaneous fistula. METHOD: A retrospective study was conducted at a tertiary referral centre to assess the effectiveness of continuous extramucosal pharyngeal suturing and the hydrogen peroxide leak test in prevention of pharyngocutaneous fistula in 59 patients who had undergone total laryngectomy with or without partial pharyn

The Utility of Recurrent Laryngeal Nerve Monitoring During Open Pharyngeal Diverticula Procedures.

OBJECTIVES: The recurrent laryngeal nerve is at risk of injury during open pharyngeal diverticula operations. The utility of recurrent laryngeal nerve (RLN) monitoring during these procedures was investigated. METHODS: A retrospective chart review was performed of 8 open pharyngeal diverticulectomies completed between 2009 and 2014. Intraoperative RLN monitoring took place during all operations. RESULTS: Open pharyngectomy and myotomy was successfully performed in all cases. In two cases the RLN was encountered outside of its normal course. The identity of the nerve was confirmed with electr

Immediate tracheoesophageal puncture for voice restoration in laryngopharyngeal resection.

A 2-year prospective study on primary tracheoesophageal puncture was carried out to evaluate the morbidity of the procedure and its success in restoring speech. Fifty-two patients, 36 of whom suffered from carcinoma of the larynx and 16, carcinoma of the hypopharynx, were entered into the study. Thirty-three patients underwent total laryngectomy with primary pharyngeal closure, 14 patients had, in addition, pharyngectomy and pectoralis major myocutaneous flap reconstruction, and 5 patients had pharyngolaryngoesophagectomy and gastric transposition. Tracheoesophageal or tracheogastric speech wa

Collision of Hypopharyngeal Small-Cell Neuroendocrine Carcinoma and Laryngeal Squamous Cell Carcinoma at the Aryepiglottic Fold: Case Report and Review of Literature.

BACKGROUND: Collision tumors are neoplasms in which two histologically different and topographically independent tumors are joined within the same mass. The aim of this paper is to present a rare case of collision carcinoma of the larynx (squamous cell and small-cell neuroendocrine carcinoma). CASE REPORT—DIAGNOSIS/TREATMENT: A 72-year-old patient was reported to our clinic because of hoarseness, dyspnea, and dysphagia. Neck CT showed a right-sided laryngeal tumor. The patient underwent total laryngectomy, partial pharyngectomy, and right-sided thyroidectomy with bilateral selective neck diss

Partial pharyngolaryngectomy with free flap for an unusual pharyngeal stenosis: surgical experience and literature review.

This report describes the case of a middle-aged man who attempted suicide, which resulted in laryngeal webbing and pharyngeal stenosis. The patient was compromised at the level of respiration, necessitating a tracheostomy. Alimentation was also affected, and feeding was done through a gastrostomy tube. Unfortunately, the fibrous tissues were resistant to dilatation and laser treatment. Hence, he underwent a modified partial horizontal supraglottic laryngectomy (PHSL) and pharyngectomy to excise all of the fibrous tissues formed after the incident. The resulting defect was closed with a radial

Evaluation and Management of an Uncommon Tumor of the Larynx: A Case Report and Literature Review of Laryngeal Low-Grade Myofibroblastic Sarcoma.

Low-grade myofibroblastic sarcoma (LGMS) of the larynx is an uncommon entity. These mesenchymal tumors of the larynx are rare and account for approximately 0.3% to 1.0% of all neoplasms at this site. We report a rare case of LGMS of the larynx that involved the larynx of a 63-year-old man with a history of hoarseness of voice. The patient was treated with total laryngectomy with partial pharyngectomy without any adjuvant treatment. Histopathologically, the tumor was composed of spindle cells that manifested variable cellular anaplasia and expressed smooth muscle actin (SMA). Our patient is dis

Extent of neck dissection after transoral robotic surgical resection of oropharyngeal squamous cell carcinoma: Report of a case and potential indications for inclusion of level I in a selective neck dissection.

BACKGROUND: With new minimally invasive surgical techniques playing a role in the management of early oropharyngeal carcinomas, appropriate surgical management of the neck is an increasingly important consideration. METHODS: We present a case of a 47-year-old patient with a clinical T2N1M0 squamous cell carcinoma (SCC) of the right tonsil with anterior extension toward the floor of the mouth from the right glossotonsillar sulcus. Imaging revealed a solitary necrotic lymph node in level IIA. The decision was made to proceed with a transoral robotic pharyngectomy and selective neck dissection o

High complication risk of salvage surgery after chemoradiation failures.

CONCLUSIONS: Although organ preservation can be achieved with chemoradiation protocols for laryngeal or pharyngeal cancers, salvage surgery is accompanied by high complication rates. OBJECTIVES: To determine the rate of complications associated with salvage surgery after chemoradiation for laryngeal and pharyngeal cancers. METHODS: A multicenter retrospective study was performed of 24 patients treated with total laryngectomy combined with total or partial pharyngectomy between 1995 and 2004 who had previously been treated with chemoradiation. The main outcome measures were early and late com

Suitability of near-total laryngopharyngectomy in piriform carcinoma.

The surgical specimens from total laryngectomy and partial pharyngectomy in 20 consecutive cases of piriform sinus carcinoma were studied by whole-organ serial sectioning in the horizontal plane. There had been gross clinical involvement or fixation of the vocal cord in 19 of these cases. Histopathologic evidence of cartilage invasion or piriform apex involvement was found in 17 specimens. In 13 cases, it appeared, retrospectively, that the cancer could have been adequately encompassed without resecting the entire contralateral side of the larynx. When this element is spared, a sphincteric spe

Extranodal Epstein-Barr virus-negative Hodgkin lymphoma of the oropharynx.

Classical Hodgkin lymphoma (cHL) arising from extranodal regions of the head and neck is uncommon, with the lymphocyte-rich (LR) subtype referenced only once in the literature. This report herein illustrates a man in his 40s with a 6-month history of a painless lump in his throat, with Magnetic Resonance Imaging (MRI) revealing a 3-cm mass in the left posterolateral oropharynx. Histopathologic examination after transoral pharyngectomy revealed LR cHL. In situ hybridisation (EBER1) indicated negative Epstein-Barr virus (EBV) testing. 27 months after excision, the LR cHL recurred in the left nas

Reconstruction of an obliterated Eustachian tube: transnasal lighted guidewire catheter stenting.

An obliterated Eustachian tube (ET) is a rare occurrence that can lead to chronic otitis media (OM) and aural fullness even with treatment. Our study presents a review of the literature on methods of stenting the ET. We additionally present a case of a man with mucoepidermoid carcinoma of the ET who underwent a radical nasal pharyngectomy with reconstruction and adjuvant radiation, and who had symptoms of intolerable otorrhea after tympanostomy tube placement to treat aural fullness and mucoid OM. We used a novel method of stenting the ET using a transnasal lighted guidewire catheter and stero

Dermatomyositis paraneoplastic syndrome before symptomatic tonsillar squamous cell carcinoma: a case report.

BACKGROUND: Paraneoplastic syndromes are systemic or organ-related functional tumor-associated changes that arise distant to the tumor. METHODS AND RESULTS: We present a rare case of a 63-year-old man with dermatomyositis as a paraneoplastic syndrome developing more than a year before clinical manifestations of tonsillar squamous cell carcinoma (SCC). He subsequently developed stage T1N2bM0 IVA tonsillar SCC. He was treated on a research protocol with 3 weeks of neoadjuvant rapamycin therapy before right transoral lateral pharyngectomy and modified radical neck dissection with preservation of

A novel technique in the operative treatment of pyriform sinus carcinoma.

Pyriform sinus carcinoma is a highly malignant carcinoma of the head and neck region as a result of its location and its potential for metastatic spread. Decisions regarding the operative procedure remain very difficult. In this paper, we present a modified partial laryngo-pharyngectomy in which, following extended vertical laryngectomy. the cancerous mass is removed from the involved pyriform sinus and the remaining hypopharynx is reconstructed by using the preserved outer perichondrium of the resected thyroid cartilage. This technique has been applied with adequate success during the last fi

Gastric pull-up for cancers of the hypopharynx and cervical esophagus: our experience.

Twenty-five cases of squamous cancer of the hypopharynx and cervical esophagus treated with laryngopharyngoesophagectomy and pharyngogastric anastomosis are presented. In all cases the lesion was such that a complete circumferential pharyngectomy was necessitated. Twelve patients had received full radiation therapy and surgery was undertaken for residual and recurrent disease. Eleven patients had a concurrent unilateral radical neck dissection and two patients a concurrent bilateral neck dissection. Five patients died within 1 month following surgery. The causes of death are discussed. The mos
