Psychosocial and psychophysical assessments of patients with unexplained chest pain.

It is imperative to assess the psychosocial factors that may influence the subjective experiences and pain behavior of persons with chronic unexplained chest pain. Both psychologists and physicians tend to rely on self-report measures of psychological distress, which provide little unique information about patients with chronic chest pain to differentiate them from patients with other painful disorders such as irritable bowel syndrome, gastroesophageal reflux disease, or coronary artery disease. However, assessment of pain-coping strategies, spouse responses to the patient's pain behaviors, an

Prediction of overexertion injuries using biomechanical and psychophysical models.

This paper summarizes a three year epidemiological study conducted in five large industrial plants in order to evaluate the validity of two alternative modeling approaches to overexertion injury prediction. Detailed biomechanical and psychophysical job evaluations were performed for 55 industrial jobs comprised of 2934 potentially stressful manual materials handling tasks. The medical experiences of 6912 incumbent workers were monitored retrospectively for two years and prospectively for one year to establish a data base for comparison of the different models. The results show that each of the

Early detection of glaucomatous damage. I. Psychophysical disturbances.

In summary, automated perimetry and other psychophysical tests currently provide the most sensitive, reliable and reproducible means of clinically detecting early glaucomatous changes. Compared to optic disk and nerve fiber layer evaluation, such procedures provide a consistent, quantitative method of distinguishing between early abnormalities and normal variations.

Jaw muscle and joint psychophysics-relevance for clinical orofacial pain practice and research. A narrative review.

Diagnosis of jaw muscle and temporomandibular joint (TMJ) pain has been greatly standardized with the development and implementation of the Diagnostic Criteria for Temporomandibular Disorders (DC/TMD). A significant part of the DC/TMD examination-pain on palpation and jaw movements-relies on psychophysical principles in the clinical procedures. Thus, it is essential that examiners are aware of the strengths and limitations of such techniques. Here we first review the background and psychophysical techniques used in the clinic and then discuss opportunities to apply both simple and more advance

Peripheral coding of tonic mechanical cutaneous pain: comparison of nociceptor activity in rat and human psychophysics.

These experiments investigated temporal summation mechanisms of tonic cutaneous mechanical pain. Human volunteers provided psychophysical estimates of pain intensity, which were compared with discharge patterns of rat cutaneous nociceptors tested with identical stimulus protocols. Human subjects made either intermittent or continuous ratings of pain intensity during stimulation of the skin between the thumb and first finger. Stimulus intensities of 25, 50, and 100 g were applied with a probe of contact area of 0.1 mm(2) for 2 min. Pain perception significantly increased during stimulation (tem

A psychophysical and neuroimaging analysis of genital hedonic sensation in men.

Current understanding of human genital-brain interactions relates primarily to neuroendocrine and autonomic control, whereas interactions during sexual stimulation remain largely unexplored. Here we present a systematic approach towards identifying how the human brain encodes sensory genital information. Using a validated affective touch paradigm and functional magnetic resonance imaging, we found that hedonic responses to discriminatory versus affective tactile stimulation were distinctly different for both penile shaft and forearm. This suggests that, as with other body sites, genital skin c

Implementation of a fast method for measuring psychophysical tuning curves.

OBJECTIVE: To implement a fast method for measuring psychophysical tuning curves (PTCs) for use in clinical applications, such as assessment of frequency selectivity and detection of dead regions in the cochlea. DESIGN: The method is based on that described by Sek et al (2005) and has been implemented in software that can be run on a PC with a good-quality sound card. In addition to the main narrowband noise masker, a lowpass noise masker can be generated to prevent detection of a distortion band corresponding to the simple difference tone. RESULTS: The software includes a routine for measur

Psychophysical and neurometric detection performance under stimulus uncertainty.

Signal detection theoretical analyses of spike counts have revealed that some cortical neurons can exceed psychophysical sensitivity in cases where a sensory signal is specified exactly. It is not known whether this finding holds in the more natural situation where signal occurrence is temporally uncertain. We investigated the ability of rat barrel cortex neurons to detect faint and transient whisker deflections occurring at unspecified times. The progression from fully specified stimuli to temporal uncertainty degraded neuronal sensitivity such that it seems highly unlikely that single neuron

Psychophysical sensory examination in individuals with a history of methylmercury exposure.

Paresthesias are the first symptom that people report following toxic doses of methylmercury. The authors conducted a psychophysical study of tactile sensation to evaluate the somatosensory abilities of subjects living in a methylmercury-polluted area around Minamata City, Japan. The authors examined control subjects and methylmercury-exposed subjects with and without numbness. A history of methylmercury exposure was taken and a neurological examination performed. Aluminum-oxide abrasive papers were used as stimuli in a psychophysical sensory examination of fine-surface-texture discrimination.

Abnormal psychophysical visual perception in Parkinson's disease patients.

Several visual dysfunctions in Parkinson's disease (PD) are described. Most of them are subtle or only demonstrated by stimulus-specific electrophysiologic or psychophysical testing. However, these minor deficits are thought to be of clinical relevance as they are related to direct or indirect complaints. Special emphasis is laid on visual hallucinations. These are most likely of multifactorial origin. The relation between hallucinations in PD and in dementia with Lewy bodies has to be elaborated further. Visual loss, as a possible and reversible cause of visual hallucinations should be active

Psychophysical evidence of nociceptor sensitization in vulvar vestibulitis syndrome.

Vulvar vestibulitis syndrome (VVS) is a long lasting disorder of superficial dyspareunia in young women. Quantitative sensory testing, including mechanical and temperature pain thresholds and warm/cold difference limen (WCL), was performed in the vestibular mucosa in 22 women (mean age 25.0 years) with vestibulitis and 20 control subjects (mean age 25.6 years). The tests were carried out on days 7-11 of the menstrual cycle. Patients had allodynia to mechanical testing with von Frey filaments, 14.3+/-3.1mN in the symptomatic posterior area as compared with 158+/-33.5mN in healthy subjects, P<0.

Diagnosis of misdiagnosis: on some of the origins and functions of psychophysical misdiagnosis.

Four cases of Peripheral Neurological Disorders (PND) are reviewed as models of misdiagnosis. Weaknesses in the logical process of diagnosis formation, combined with the dismissive function of the diagnosis, may lead to psychiatric misdiagnoses such as hysteria. The same logical and emotional factors can prevent correction of misdiagnosis and thus the misery of the patient may continue. Comments are made about the dangers of misusing the diagnosis of hysteria.

Average evoked responses and psychophysical performance in patients with pseudohypoparathyroidism.

Perceptual function in eight patients with pseudohypoparathyroidism (PHP) was studied with two neurophysiological measures, the average evoked response (AER) and nerve conduction velocity. A battery of psychophysical tasks including reaction time, size estimation, `hidden pictures,' and the rod and frame procedure was also used. Patients with PHP had significantly longer latency visual AER and slower reaction time than a group of normal volunteers, and they performed erratically and poorly on the other psychophysical tasks. Differentiating these patients with diffuse mental deficiency were two

Psychophysical Characterization of Osseoperception Across Upper-Limb Bony Landmarks.

Somatosensory feedback is essential for fine motor control, yet it remains a challenge to restore it in assistive technologies such as prostheses or exoskeletons. This study investigates osseoperception-the perception of vibrotactile and auditory sensations through bone conduction- a feedback approach that has unique advantages but is rarely considered in literature. We have systematically assessed the psychometric properties of this method across four anatomical sites: the pisiform bone (PB) at the wrist, the olecranon (OL) at the elbow, the clavicle (CL) at the shoulder, and the mastoid proc

Psychophysical Effects of Grasping Conditions of a Handheld Device on Vibrotactile Perceptions.

Tactile perception varies between individuals and the state of the grasp. To investigate the psychophysical effects on vibrotactile perceptions influenced by grasp type and strength of a handheld device, perceived intensities were tested against various grasp strengths and stimulus levels using 40 and 250 Hz vibrations under power grasp and precision grasp conditions. Grasp strength was measured with a pressure distribution sensor on the cylindrical device capable of presenting vibrations up to approximately 2.0 m/s$^{2}$. The analysis using a physical model supported the measured acceleration

Nasal trigeminal sensitivity after total laryngectomy: A psychophysical study.

OBJECTIVES: Total laryngectomy permanently diverts respiratory airflow through a tracheostoma, abolishing nasal breathing. Although olfactory loss is well described, it remains unclear whether nasal trigeminal chemosensation is perceptually accessible under natural airflow-deprived conditions. We aimed to assess trigeminal performance after total laryngectomy under both airflow-assisted (menthol lateralization detection threshold [LDT]) and non-assisted conditions. MATERIAL AND METHODS: A prospective cross-sectional study assessed trigeminal function in patients undergoing total laryngectomy
