Role of C-type lectins in mycobacterial infections.

Worldwide clinical cases due to multi drug- and extensively drug-resistant strains of Mycobacterium tuberculosis (M.tb) are increasing making the need for new therapies more critical than ever. A major obstacle for designing new drugs to treat mycobacterial infections is our limited knowledge of the interface between the bacillus (especially M.tb) and its host. The pulmonary innate immune system plays a key role in the recognition of microbes entering via the respiratory route. Although the specificity of this system is broad and based on the recognition of pathogen-associated molecular patter

Use of Perfluorodecalin for Bronchoalveolar Lavage in Case of Severe Pulmonary Hemorrhage and Extracorporeal Membrane Oxygenation: A Case Report and Review of the Literature.

We present a patient on venovenous extracorporeal membrane oxygenation for respiratory failure, secondary to complete airway obstruction from pulmonary hemorrhage after open pulmonary embolectomy. Obstruction was refractory to conventional therapy, including mechanical debridement and saline lavage. By using perfluorodecalin, we were able to transiently clear the airways of thrombus and reopen distal airways and alveoli more completely than with conventional therapies. The use of perfluorodecalin (versus radiopaque perflubron) enabled us to visualize this reopening on chest X-ray.

Primary congenital pulmonary hypoplasia of a neonate.

Pulmonary hypoplasia is a rare but usually lethal disease. We report a full-term male neonate who presented with respiratory failure immediately after birth. Chest X-ray revealed a small lung volume despite advanced ventilator support. Respiratory failure persisted and this baby died at 40.5 hours of age. The autopsy showed a lung-to-birth weight ratio of 0.69% and a radial alveoli count of 2.97. All this information confirmed the diagnosis of primary congenital pulmonary hypoplasia.

Pulmonary alveolar proteinosis associated with a disease-modifying antirheumatoid arthritis drug.

Pulmonary alveolar proteinosis is an uncommon disorder marked by the abnormal accumulation of surfactant within the alveoli. Secondary pulmonary alveolar proteinosis develops in patients who are immunosuppressed, usually with corticosteroids. We present a case of biopsy-proven pulmonary alveolar proteinosis in a patient undergoing therapy with the disease-modifying antirheumatoid arthritis drug leflunomide (Arava; Aventis Pharmaceuticals, Bridgewater, NJ). He was treated with whole lung lavage and discontinuation of leflunomide with good results. This is the first reported association of secon

[Plasmapheresis failure in the treatment of auto-immune pulmonary alveolar proteinosis].

INTRODUCTION: Auto-immune pulmonary alveolar proteinosis is a rare disorder characterized by the accumulation of surfactant proteins in the alveoli. CASE REPORT: We report a case of a 41-year-old smoker, presenting initially with acute respiratory failure. Whole lung lavages were effective initially but only for a few weeks. GM-CSF subcutaneous injections were not effective, and then plasmapheresis were tried. CONCLUSION: This is the fifth report of the use of this treatment in auto-immune pulmonary alveolar proteinosis. Plasmapheresis was not effective in our patient.

Pulmonary infiltration with eosinophila. Recurrence in an asthmatic patient treated with beclomethasone dipropionate.

We present the findings in a young woman diagnosed as having pulmonary infiltration with eosinophilia and asthma who was treated with beclomethasone dipropionate. Although the aerosol corticosteroid controlled bronchospasm, it did not prevent recurrent pulmonary infiltration with eosinophilia when the patient was not receiving oral therapy with corticosteroids. It is postulated that when administered in the usual dosage, an adequate amount of beclomethasone dipropionate does not reach the alveoli or interstitium to prevent or resolve the process of pulmonary infiltration with eosinophilia. [Pulmonary edema following obstruction of the upper airway].

The development of pulmonary edema after the relief of upper airway obstruction in two patients is described. Pulmonary edema in those patients was the result of increased negative intrapleural and intra-alveolar pressure during forceful inspiration and in the course of upper airway obstruction. An increase in the venous return occurs and the established transpulmonary pressure gradient promotes transudation into the interstitium and alveoli. Consequent alveolar hyperventilation results in hypoxia and acidosis. All patients developing pulmonary edema should be treated with positive pressure ve

[Physiopathology of accidental drowning].

The first phase of accidental drowning begins with asphyxia, due to either laryngospasm (10-15 percent of cases) or water aspiration. The second phase is characterized by water and electrolyte changes in the blood. The physiopathological modifications caused by drowning in fresh water differ from those of drowning in sea water. The hypotonic fresh water quickly diffuses in the bloodstream. The consequences are, in many cases, hypervolemia with pulmonary edema, hemolysis, hyperkalemia with risk of ventricular fibrillation, diminution of hemoglobin, and a relative decrease in plasma concentratio

Pulmonary neutrophil kinetics.

When in transit in the pulmonary capillaries, neutrophils can interact with the endothelium and gas in the alveoli. Neutrophils play a major role in lung defences and have been implicated in the pathogenesis of lung injury, such as emphysema and the adult respiratory distress syndrome. Moreover, the pulmonary capillaries receive all of the circulating neutrophils. It is surprising therefore, that relatively little is known of the factors which influence the passage of neutrophils through the pulmonary circulation. This article reviews our knowledge of neutrophil kinetics in the lungs and descr

Bilateral micronodular pulmonary infiltrate: is it important to make a histological diagnosis?

Pulmonary alveolar microlithiasis is a rare disease characterized by the deposition of calcium phosphate within the alveoli. We report the case of a 20-year-old man with a 6-week history of cough and shortness of breath on exertion. The chest radiograph demonstrated a bilateral symmetrical micronodular pattern. High-resolution computed tomography revealed bilateral diffuse fine nodular shadowing involving the mid zones, with sparing of the apices. The patient underwent a transbronchial lung biopsy, which confirmed the diagnosis of pulmonary alveolar microlithiasis.

[Research progress of endogenous bone marrow-derived mesenchymal stem cells in pulmonary fibrosis].

Pulmonary fibrosis is a chronic, diffuse, interstitial lung disease involving the pulmonary interstitium, alveoli, and bronchioles caused by various causes. There is no effective treatment. Currently, exogenous bone marrow-derived mesenchymal stem cells (BM-MSCs) transplantation has attracted much attention as a new stem cell therapy in the treatment of pulmonary fibrosis. Less attention has been paid to the functional status of endogenous BM-MSCs during pulmonary fibrosis. Based on summary on the anti-pulmonary fibrosis effect of BM-MSCs and its mechanism, this review further discusses the ab

Immunohistochemical Examination of Novel Rat Monoclonal Antibodies against Mouse and Human Podoplanin.

This study aims to develop new monoclonal antibodies (mAbs) against mouse and human podoplanin. Rats were immunized with synthetic peptides, corresponding to amino acids 38-51 of mouse podoplanin or human podoplanin which is 100% homologous to the same site of monkey podoplanin; anti-mouse podoplanin mAb PMab-1 (IgG(2) (a)) and anti-human mAb NZ-1.2 (IgG(2) (a)) were established. In immunocytochemistry, the mouse melanoma B16-F10 and mouse podoplanin (mPDPN)-expressed CHO transfectant were stained by PMab-1; human lymphatic endothelial cells (LEC) and human podoplanin (hPDPN)-expressed squamou

Dysplastic basal-like cells in alveolar repair and regeneration.

The lung is constantly exposed to environmental insults, leading to the evolution of diverse repair mechanisms that involve regional stem and progenitor cells. However, in response to severe alveolar injury, aberrant basal-like cells emerge in the alveolar regions, as observed in acute viral infections and chronic pulmonary diseases like lung fibrosis. While these cells can rapidly reseal the epithelial barrier, they fail to restore normal lung function and, if persistent, represent dysplastic repair. This review summarizes recent advances in understanding the cellular origin, regulation, and

Familial pulmonary capillary hemangiomatosis resulting in primary pulmonary hypertension.

We describe the first cases of familial pulmonary capillary hemangiomatosis, a disorder in which capillaries in the lungs proliferate. Three siblings died from primary pulmonary hypertension. One developed pulmonary congestion preterminally after vasodilator treatment. The inheritance pattern seems autosomal recessive. Lung specimens obtained in two siblings showed extensive pulmonary capillary hemangiomatosis, with normal capillaries proliferating into veins and alveoli. Including our patients, four of the nine patients with pulmonary capillary hemangiomatosis have presented with the clinical

Developmental Therapeutics for Alveolar Generation and Regeneration: Implications for Bronchopulmonary Dysplasia and Prematurity-Associated Lung Disease.

The mechanisms by which alveoli form during lung development remain under investigation. The classical model proposes formation through secondary septation, whereas an alternative proposed model suggests that alveoli arise through epithelial budding within preformed mesenchymal stabilizing rings. Regardless of the precise morphogenetic mechanism, alveolar formation depends on coordinated interactions among epithelial progenitor cells, pulmonary endothelial cells, mesenchymal populations, extracellular matrix remodeling, and biomechanical forces. Stereologic imaging supports continued postnatal

Congenital alveolar proteinosis.

Pulmonary alveolar proteinosis is recently described as a rare cause of lung dysfunction and respiratory distress in term neonates. In several cases, a deficiency or insufficiency of surfactant protein B SP-B has been caused by a frame shift mutation in the gene encoding SP-B. Three siblings with congenital pulmonary alveolar proteinosis showed clinical and radiological evidence. Histopathological and immunohistochemical studies in the last sibling revealed deficiency of SP-B, one of the group of 3 specific lipoproteins that reduce the surface tension between air and liquid interface within pu

Antibodies produced in response to Cryptococcus neoformans pulmonary infection in mice have characteristics of nonprotective antibodies.

Murine cryptocococcal pulmonary infection elicited serum immunoglobulin M (IgM) and IgG to the capsular polysaccharide, but only IgG stained yeast cells in alveoli. Both isotypes produced punctuate immunofluorescence patterns on yeast cells like those of nonprotective antibodies. The difficulties involved in associating humoral immunity with protection in murine cryptocococcal infection could reflect nonprotective antibody responses.

[Mediastinal emphysema in children].

A review of 25 infants developing pneumediastinum and subcutaneous emphysema was done. These observations suggested that increasing airways presure creates excessive alveolar distention. Pulmonary interstitial emphysema results from movement of air out of alveoli into interstitial areas. From this point air could dissect along the perivascular or interstitial spaces to the mediastinum.

STUDIES ON THE MECHANISM OF RECOVERY IN PNEUMOCOCCAL PNEUMONIA : I. THE ACTION OF TYPE SPECIFIC ANTIBODY UPON THE PULMONARY LESION OF EXPERIMENTAL PNEUMONIA.

A uniformly fatal lobar pneumonia was produced in white rats by inoculation of the left main bronchus with virulent Type I pneumococci suspended in mucin. All of the animals succumbed in less than 5 days, half of them dying within 48 hours. In only 5 of 40 rats was the lesion confined to the left lung, and all but one developed pleurisy, pericarditis, or both. All had bacteriemia at the time of death. The pathogenesis of the pulmonary lesion was studied by examining the lungs of 35 rats killed at various intervals following inoculation. The pneumonic process spread rapidly until most of the le
