Management of Traumatic Brain Injury Sequelae With Alpha-2 Adrenergic Receptor Agonists.

OBJECTIVE: To describe clinical uses of the class of alpha-2 receptor agonist medications in the treatment of traumatic brain injury sequelae. Clinicians treating patients with traumatic brain injuries (TBI) will often see patients who have been prescribed alpha-2 agonists or may consider initiating use. DESIGN: Narrative review. CONCLUSION: This class of medication has a number of different indications (and side effects), and drugs within this class also have some unique properties that can be important factors in clinical decision-making. Although this class of medications has been availab

The Association of an Alpha-2 Adrenergic Receptor Agonist and Mortality in Patients With COVID-19.

There is a need for treatments to reduce coronavirus disease 2019 (COVID-19) mortality. Alpha-2 adrenergic receptor (α2 AR) agonists can dampen immune cell and inflammatory responses as well as improve oxygenation through physiologic respiratory parameters. Therefore, α2 AR agonists may be effective in reducing mortality related to hyperinflammation and acute respiratory failure in COVID-19. Dexmedetomidine (DEX) is an α2 AR agonist used for sedation. We performed a retrospective analysis of adults at Rush University System for Health hospitals between March 1, 2020 and July 30, 2020 with COVI

The alpha 2A-adrenergic receptor gene polymorphism modifies antidepressant responses to milnacipran.

OBJECTIVE: The alpha 2A-adrenergic receptor (ADRA2A) plays a central role in the regulation of systemic sympathetic activity. Recently, the functional defect of ADRA2A has been implicated as a cause of depression, attention deficit hyperactivity disorder, and Tourette syndrome. In this study, the effect of genetic variants of the ADRA2A gene on the response to selective serotonin reuptake inhibitors (SSRIs)/serotonin norepinephrine reuptake inhibitors (SNRIs) was examined in depressed patients. METHOD: Ninety-three Japanese depressed patients were recruited in the present study, assigned rand

Alpha-1B adrenergic receptor knockout mice are protected against methamphetamine toxicity.

Antidopaminergic gastrointestinal prokinetics (bromopride, clebopride, domperidone, levosulpiride and metoclopramide) have been exploited clinically for the management of motor disorders of the upper gastrointestinal tract, including functional dyspepsia, gastric stasis of various origins and emesis. The prokinetic effect of these drugs is mediated through the blockade of enteric (neuronal and muscular) inhibitory D2 receptors. The pharmacological profiles of the marketed compounds differ in terms of their molecular structure, affinity at D2 receptors, ability to interact with other receptor s

Does postsynaptic alpha 1-adrenergic receptor supersensitivity contribute to autonomic dysreflexia?

Quadriplegics often experience periods of severe hypertension known as autonomic dysreflexia. Clinically, these events have been well documented, but the mechanisms for mediating autonomic dysreflexia remain unclear. We used a chronic rat model to investigate the potential development of supersensitivity at postsynaptic alpha 1-adrenergic receptors as a contributing factor to the exaggerated sympathetic response characteristic of autonomic dysreflexia. Adult male Wistar and Sprague-Dawley rats were anesthetized and given spinal transection at T5. After 30 days, rats were reanesthetized and art

Characterization of alpha-2D adrenergic receptor subtypes in bovine ocular tissue homogenates.

The alpha-2 adrenergic receptors are known to be present in the mammalian eye and to mediate the effects of alpha-2 agonists used in the treatment of glaucoma. Little is known, however, regarding the relative densities of the three alpha-2 subtypes in the various tissues of the eye. We used receptor binding experiments with the radioligand [3H]RX821002 to characterize the alpha-2 adrenergic receptors in three tissues of the bovine eye, the ciliary body, retinal pigment epithelium/choriocapillaris and iris. The K(D) values in the three tissues were similar (0.12-0.14 nM), and the Bmax values ra

Alpha-1 Adrenergic Receptors Modulate Glutamate and GABA Neurotransmission onto Ventral Tegmental Dopamine Neurons during Cocaine Sensitization.

The ventral tegmental area (VTA) plays an important role in the reward and motivational processes that facilitate the development of drug addiction. Presynaptic α1-AR activation modulates glutamate and Gamma-aminobutyric acid (GABA) release. This work elucidates the role of VTA presynaptic α1-ARs and their modulation on glutamatergic and GABAergic neurotransmission during cocaine sensitization. Excitatory and inhibitory currents (EPSCs and IPSCs) measured by a whole cell voltage clamp show that α1-ARs activation increases EPSCs amplitude after 1 day of cocaine treatment but not after 5 days of

Blockade of alpha-2 adrenergic receptors in the rostral ventrolateral medulla attenuates the sympathoinhibitory response to cocaine.

The purpose of this study was to determine whether neurons in the rostral ventrolateral medulla play a role in the sympathoinhibitory response elicited by i.v. administration of cocaine and, if so, to identify the type(s) of receptors involved. Adrenergic antagonists were microinjected bilaterally into the rostral ventrolateral medulla in pentobarbital-anesthetized rats in an attempt to block the decrease in sympathetic nerve discharge (SND) elicited by cocaine (1 mg/kg i.v.). After the bilateral microinjection of saline, cocaine elicited a -56 +/- 5% (mean +/- S.E.) decrease in SND lasting 36

Alpha-1-adrenergic receptors in the nucleus tractus solitarii region of rats with experimental and genetic hypertension.

The binding of the alpha 1-adrenergic receptors antagonist, 125I-HEAT, to membranes of nucleus tractus solitarii (NTS) regions of the brains of neurogenic hypertensive, spontaneously hypertensive (SH), and deoxycorticosterone (DOCA)/salt hypertensive rats and their respective controls was studied to quantitate the expression of alpha 1-adrenergic receptors. Scatchard analysis of the binding studies revealed a 1.8-fold increase in the Bmax of alpha 1-adrenergic receptors in NTS region membranes of SH rats when compared to their Wistar-Kyoto (WKY) control without significant difference in the Kd

Differentiation of alpha 1-adrenergic receptors linked to phosphatidylinositol turnover and cyclic AMP accumulation in rat brain.

Activation of alpha 1-adrenergic receptors in slices of rat brain increases inositol phosphate accumulation, increases basal cyclic AMP accumulation, and potentiates the increase in cyclic AMP caused by adenosine. We compared these three responses to determine whether they are mediated by the same receptors. The increase in inositol phosphates and the potentiation of cyclic AMP accumulation in cerebral cortex were largely blocked by chelation of extracellular calcium, whereas the increase in basal cyclic AMP was not affected. The magnitude of the increase in inositol phosphates in different br

Decreased alpha 2-adrenergic receptors on platelet membranes from diabetic patients with autonomic neuropathy and orthostatic hypotension.

Platelet adrenergic receptors were studied in normal subjects and diabetic patients with autonomic neuropathy to determine the relationship between adrenoreceptor status and orthostatic hypotension. The binding of [3H]clonidine and [3H]yohimbine to platelet membranes was measured in diabetic patients with autonomic neuropathy and orthostatic hypotension (n = 12) and without orthostatic hypotension (n = 11), diabetic patients without autonomic neuropathy (n = 12), and normal subjects (n = 9). Mean basal and standing plasma norepinephrine levels were not different in the four groups, and there w

Characteristics of [3H]prazosin binding to guinea pig brain alpha-adrenergic receptors: effects of estradiol and progesterone.

[3H]Prazosin was found to bind to sites on guinea pig brain membranes with alpha 1-adrenergic receptor characteristics. Treatment of ovariectomized guinea pigs with estradiol benzoate (EB) or EB followed by progesterone (P) did not affect [3H]prazosin binding to membranes from hypothalamus, preoptic area, amygdala or cerebral cortex. When added to the incubation mixture of the assay, estradiol, P, and other steroids decreased [3H]prazosin binding but only at high concentrations. These results do not support the idea that estrogen and progestin influence reproductive physiology through effects

Platelet function in essential thrombocythemia. Decreased epinephrine responsiveness associated with a deficiency of platelet alpha-adrenergic receptors.

Platelets from two patients with essential thrombocythemia failed to aggregate or release serotonin in response to concentrations of epinephrine that aggregated platelets from normal controls. Therefore, we studied their alpha-adrenergic receptors, using 3H-dihydroergocryptine (3H-DHE), an alpha-adrenergic antagonist. These platelets contained an average (mean +/- S.E.) of 210 +/- 18 and 227 +/- 27 3H-DHE binding sites per platelet--less than half that found on control platelets, 464 +/- 37 (P less than 0.01). In contrast, platelets from two other patients with essential thrombocythemia respon

The role of alpha- and beta- adrenergic receptors in some actions of catecholamines on intestinal smooth muscle.

1. Experiments were carried out to determine the type of adrenergic receptor concerned in the action of noradrenaline in increasing the rate of exchange of potassium ions in the taenia of the guinea-pig caecum. The preparations were bathed in potassium-rich solutions in order to depolarize the muscle fibres and so eliminate changes in potassium flux secondary to alterations in membrane potential.2. Noradrenaline was more effective than isoprenaline in increasing both the influx and efflux of (42)K, suggesting that the alpha-receptors were involved. This action on potassium permeability was abo

Effects of peripheral nerve injury on alpha-2A and alpha-2C adrenergic receptor immunoreactivity in the rat spinal cord.

Neuropathic pain resulting from peripheral nerve injury can often be relieved by administration of alpha-adrenergic receptor antagonists. Tonic activation of alpha-adrenergic receptors may therefore facilitate the hyperalgesia and allodynia associated with neuropathic pain. It is currently unclear whether alpha2A- or alpha2c-adrenergic receptor subtypes are involved in the pro-nociceptive actions of alpha-adrenergic receptors under neuropathic conditions. We therefore investigated the effects of peripheral nerve injury on the expression of these subtypes in rat spinal cord using immunohistoche

Desensitization of postjunctional alpha 1- and alpha 2-adrenergic receptor-mediated vasopressor responses in rat harboring pheochromocytoma.

Prolonged stimulation of tissues by adrenergic agonists may lead to diminished responsiveness of the tissues to subsequent activation by catecholamines; this phenomenon has been termed desensitization or tachyphylaxis. We have examined the in vivo consequences of prolonged stimulation of vascular alpha-adrenergic receptors in rats harboring pheochromocytoma, a tumor that secretes catecholamines. In both early (3-4 weeks after implantation) and late (6-7 weeks after implantation) stages of tumor development, New England Deaconess Hospital rats with transplanted pheochromocytomas developed hyper

Binding and functional characterization of alpha-2 adrenergic receptor subtypes on pig vascular endothelium.

Alpha-2 adrenergic receptor subtypes were characterized in membranes of pig vascular endothelium using [3H]rauwolscine. Alpha-2 adrenergic receptor subtypes that mediate endothelium-dependent vascular relaxation were studied in vitro by using ring segments of pig epicardial coronary arteries. Specific [3H]rauwolscine binding in endothelial membranes was saturable and to a single class of high-affinity sites with a mean KD of 0.217 +/- 0.05 nM and Bmax of 156 +/- 28 fmol/mg of protein. Nonlinear regression analysis indicated that competition binding curves for drugs that distinguish the alpha-2

Platelet alpha 2-adrenergic receptor binding to 3H-yohimbine and personality variations in normals.

The authors examined platelet alpha 2-adrenergic receptor binding to 3H-yohimbine and several personality variables in 58 adult males in a campus community. Subjects with high receptor Bmax levels exhibited personality traits of less social inhibition and more sensation seeking and thrill seeking behavior, were more playful and autonomous, and came from healthier, more intact families. The results also suggested that high affinity states of platelet alpha 2-adrenergic receptors (low Kd) correlate with traits suggestive of stability, i.e., Autonomy, Dominance, Nurturance, Order, Succorance, and
