A pay for performance scheme in primary care: Meta-synthesis of qualitative studies on the provider experiences of the quality and outcomes framework in the UK.

BACKGROUND: The Quality and Outcomes Framework (QOF) is an incentive scheme for general practice, which was introduced across the UK in 2004. The Quality and Outcomes Framework is one of the biggest pay for performance (P4P) scheme in the world, worth £691 million in 2016/17. We now know that P4P is good at driving some kinds of improvement but not others. In some areas, it also generated moral controversy, which in turn created conflicts of interest for providers. We aimed to undertake a meta-synthesis of 18 qualitative studies of the QOF to identify themes on the impact of the QOF on individ

The impact of a pay-for-performance system on timing to hip fracture surgery: experience from the Lazio Region (Italy).

BACKGROUND: A tariff modulation mechanisms has been introduced in some Italian regions with the aim of reducing inappropriate admissions and improving quality of care. In response to a regional act, hospitals in Lazio adopted a clinical pathway for elderly patients with hip fracture and introduced a compensation system based on the quality of health care, as in a pay-for-performance model. The objective of the present study was to compare the proportion of surgery for hip fracture performed within 48 hours of admission among Lazio hospitals according to different payment systems, before and af

[Toward establishment of an economical basis for neurological care in Japan: appropriate assessment of techniques for diagnosis and treatments].

Appropriate assessments for medical techniques of clinical diagnostic examination have been one of long-standing issues in social insurance system in Japan. Assessments of those in the surgical fields in which the most of assessments are evaluated by surgical operations including related materials as well as surgeons' techniques, is rather easy to evaluate quantitatively. Comparing with this, medical fields including neurology, quantitative assessments of medical techniques either diagnostic or treatment is not so easy. In order to evaluate the burden in each technique in the medical fields, s

Comparison of one-stage versus two-stage anterior/posterior spinal fusion for neuromuscular scoliosis.

Twenty-six patients with progressive neuromuscular scoliosis underwent anterior/posterior (AP) spinal fusion. Thirteen of the patients underwent a one-stage fusion, and 13 underwent a two-stage fusion. Although one-stage AP spinal fusion provides adequate correction of severe curves and allows a more expeditious recovery without increasing patient morbidity or mortality, current third-party payor reimbursement policies provide little incentive for 1-day operations. In addition, the procedure is mentally and physically demanding and the apparent benefits must be weighed against the potential ha

Payment and Care for Hematopoietic Cell Transplantation Patients: Toward a Specialized Medical Home for Complex Care Patients.

Patient-centered medical home models are fundamental to the advanced alternative payment models defined in the Medicare Access and Children's Health Insurance Plan Reauthorization Act (MACRA). The patient-centered medical home is a model of healthcare delivery supported by alternative payment mechanisms and designed to promote coordinated medical care that is simultaneously patient-centric and population-oriented. This transformative care model requires shifting reimbursement to include a per-patient payment intended to cover services not previously reimbursed such as disease management over t

The Impact of Payment Reform on Medicaid Access and Quality: A National Survey of Physicians.

INTRODUCTION: Addressing health inequity should involve improving access and quality of care for individuals with low income. In the United States, programs that involve accountable care incentives and broader risk adjustment are poised to help address these issues. However, there is limited understanding of perspectives among physicians as key stakeholders in determining access and quality. METHODS: The authors surveyed physicians about how they perceived accountable care incentives and broader risk adjustment (greater inclusion of clinical, social, and behavioral risk factors) would impact

Improving Access To Substance Use Disorder Care In Nursing Facilities.

The population of aging patients with substance use disorder (SUD) requiring care in nursing facilities is growing, yet these facilities are not sufficiently prepared to provide evidence-based treatment for such conditions, including opioid use disorder. Policy and practice innovations are needed to address barriers to access and care within these settings. This article provides an overview of the regulatory landscape and reviews existing efforts to improve the uptake of care for SUD in US nursing facilities, including educational and technical assistance programs; policy toolkits; state-based

An international study to explore the challenges faced by the medical device industry in the development of compression products and reimbursement.

Lack of agreement over the international classification for compression therapy contributes to confusion over what measures are required to capture patient-reported and cost-effective outcomes of compression therapy for the heterogenous patient population. The medical device industry that manufactures compression products has important insights into these iissues, which have not been previously explored. This knowledge could provide clarity for improving the development of compression products and use of outcome measures internationally, which could improve access and uptake of compression. Ei

The impact of injury severity and transfer status on reimbursement for care of femur fractures.

BACKGROUND: This study investigates the impact of injury severity, patient origin, and payer on charges and payments associated with treatment of femoral fractures at a Level I trauma center. We hypothesized that transfer patients and patients with minor injury would be underinsured, whereas reimbursement rate would be higher for patients with severe injury. METHODS: Medical and financial records of 420 adult patients treated for femoral fractures at a public, urban Level I trauma center were reviewed. Facility and professional charges and payments were determined. Reimbursement rate was defi

Seven lessons for financial incentivisation schemes in healthcare: a qualitative study of NHS Resolution's Maternity Incentive Scheme.

OBJECTIVES: Avoidable harm and variations in quality have remained persistent features of health systems, but the track record of financial incentives in driving improvement is mixed. Based on an evaluation of NHS Resolution's Maternity Incentive Scheme (MIS), which focuses on safety of maternity care in English NHS trusts, we identify generalisable principles to inform design of financial incentive schemes in healthcare. DESIGN: Qualitative process evaluation based on interviews with those involved in the design and delivery of MIS. SETTING: Organisations involved in running MIS and organis

Impact Of Medicare Readmissions Penalties On Targeted Surgical Conditions.

The Hospital Readmissions Reduction Program, announced in 2010 to penalize excess readmissions for patients with selected medical diagnoses, was expanded in 2013 to include targeted surgical diagnoses, beginning with hip and knee replacements. Whether these surgical penalties reduced procedure-specific readmissions is not well understood. Using Medicare claims, we evaluated the penalty announcements' effects on risk-adjusted readmission rates, episode payments, lengths-of-stay, and observation status use. Risk-adjusted readmission rates declined for both procedures from 7.6 percent in 2008 to

Is the prescribing behavior of Chinese physicians driven by financial incentives?

The prescribing behavior to prescribe high-priced drugs has been hypothesized to be related to the increasing drug expenditures in China, but little empirical evidence exists. The purpose of this study was to examine whether Chinese physicians, driven by financial incentives, tend to prescribe high-priced drugs. The 2000-2008 drug data in the Yangtze River Basin Hospitals' Drug Use Analysis System were analyzed to examine the prescription patterns of penicillins and cephalosporins in Shanghai. Among the top-100 drugs (by volume), cephalosporins cost as 1.1- 2.3 times as penicillins and their v

Pricing and reimbursement of orphan drugs: the need for more transparency.

Pricing and reimbursement of orphan drugs are an issue of high priority for policy makers, legislators, health care professionals, industry leaders, academics and patients. This study aims to conduct a literature review to provide insight into the drivers of orphan drug pricing and reimbursement. Although orphan drug pricing follows the same economic logic as drug pricing in general, the monopolistic power of orphan drugs results in high prices: a) orphan drugs benefit from a period of marketing exclusivity; b) few alternative health technologies are available; c) third-party payers and patien

Characteristics and prescribing practices of clinicians recently waivered to prescribe buprenorphine for the treatment of opioid use disorder.

BACKGROUND AND AIMS: Expanding access to medication-assisted treatment with buprenorphine is a cornerstone of the opioid crisis response, yet buprenorphine remains underutilized. Research has identified multiple barriers to prescribing buprenorphine. This study aimed to examine clinician characteristics, prescribing practices and barriers and incentives to prescribing buprenorphine among clinicians with a federal Drug Addiction Treatment Act of 2000 (DATA) waiver to prescribe buprenorphine for opioid use disorder treatment. DESIGN: Electronic survey of 4225 clinicians conducted between March
