Retrieval, automaticity, vocabulary elaboration, orthography (RAVE-O): a comprehensive, fluency-based reading intervention program.

The most important implication of the double-deficit hypothesis (Wolf & Bowers, in this issue) concerns a new emphasis on fluency and automaticity in intervention for children with developmental reading disabilities. The RAVE-O (Retrieval, Automaticity, Vocabulary Elaboration, Orthography) program is an experimental, fluency-based approach to reading intervention that is designed to accompany a phonological analysis program. In an effort to address multiple possible sources of dysfluency in readers with disabilities, the program involves comprehensive emphases both on fluency in word attack, w

Diagnostic delay in colorectal cancer.

Recent guidelines recommend early diagnosis of colorectal cancer. Little data exists for comparison or to set standards. The aims of this study were to calculate the time to diagnosis for colorectal cancer from first hospital attendance and to identify any remedial factors felt to contribute to an undue delay in diagnosis. A one year retrospective audit was undertaken in a specialist teaching hospital. There were 59 patients and the median time to diagnosis was 17 days after first hospital consultation (mean 52 days). Some 34% had to wait more than one month for diagnosis. Incomplete examinati

The current status of faculty staffing and resident training in emergency radiology. Results of a survey.

The results of a survey of United States and Canadian radiology residency programs in hospitals maintaining major emergency departments indicate that (1) radiologic faculty assignment to emergency medicine may include "all faculty," "specific faculty," "specific and other faculty," "general," and "musculoskeletal" faculty; (2) a chief of emergency radiology section is designated in less than 35% of radiology departments providing emergency room services; (3) radiology resident rotation in emergency radiology occurs in less than 2/3 of the surveyed programs; and (4) radiology resident experienc

Dominant coloboma-microphthalmos syndrome associated with sensorineural hearing loss, hematuria, and cleft lip/palate.

Ocular colobomas and microphthalmos, isolated or as part of a syndrome, are usually sporadic and only rarely found in large families. A 4-generation family with autosomal dominant uveal coloboma and microphthalmos associated with cleft lip and palate was re-evaluated. Wide variability in expression is evident and more recently recognized manifestations include a complete spectrum of eye involvement, impairment of extraocular movement, mid-frequency sensorineural hearing loss, and hematuria. Learning difficulties requiring remedial teaching were present in one third of those affected and a neur

Psychotropic drug use and polypharmacy in a general hospital.

This paper presents the usage of psychotropic drugs by all general inpatients of a Boston teaching and referral hospital on a randomly chosen weekday. Of all surveyed inpatients, 42.8% were receiving at least one psychotropic medication. Sleep medications were the most frequently prescribed class of psychotropic drugs and flurazepam was the most commonly prescribed of all drugs. Phenothiazine and neuroleptics were given to control agitation, pain, or nausea, rather than psychosis. Antidepressants were prescribed without notated justification in the medical record, and if given for depression,

A dismantling study of comprehensive cognitive remediation for improving employment outcomes: what is the role of computer cognitive training?

BACKGROUND: Comprehensive cognitive remediation improves cognitive and functional outcomes in people with serious mental illness, but the specific components required for effective programs are uncertain. The most common methods to improve cognition are facilitated computerized cognitive training with coaching and teaching cognitive self-management strategies. We compared these methods by dismantling the Thinking Skills for Work program, a comprehensive, validated cognitive remediation program that incorporates both strategies. METHODS: In a randomized controlled trial we assigned 203 unemplo

Designing an Assistive Learning Aid for Writing Acquisition: A Challenge for Children with Dyslexia.

In Pakistan, the biggest challenge is to provide high quality education to the individuals with learning disabilities. Besides the well known affordance issue, there is a lack of awareness regarding the term dyslexia and remedial teaching training that causes the identification as well as remediation of the dyslexic individuals at early stages in Pakistan. The research was focused to exploit the benefits of using the modern mobile technology features in providing a learning platform for young dyslexic writers. Based on potential usability requirements of young dyslexic writers stated by remedi

Natural history of the fetal alcohol syndrome: a 10-year follow-up of eleven patients.

Of the eleven children who were the first to be diagnosed as having the fetal alcohol syndrome ten years ago, two are now dead, one is lost to follow-up, and the remaining eight continue to be growth deficient and dysmorphic. With menarche, which occurred with normal timing, the female patients developed increased body fat. The mothers were all severe chronic alcoholics. Four of the eight known survivors are of borderline intelligence and have needed some remedial teaching. The other four are severely handicapped intellectually and need complete supervision outside the home. The degree of grow

[Hormonal contraception interactions].

OBJECTIVE: The purpose of this article is to discuss the probable ways of interactions between some drugs or remedies and steroid contraceptives. DESIGN: A review article. SETTING: OBGYN Clinic of the 1t Faculty of Medicine, Prague, Teaching Hospital Bulovka. SUBJECT: Low dose oral contraceptives are very popular these days. Some drugs and remedies could negatively influence the levels of ethinyl estradiol (EE) and/or progestins and thus increase the possibility of their failure. These drugs mostly implicate as an inducer of the CYP450 system (liver) and as an inducer of P-glycoprotein tran

Longitudinal outcomes of an individualized and intensive reading intervention for third grade students.

Early intervention is known to reduce reading disabilities; however, treatment response is variable, and some students have persistent deficits that require intensive supports. This study examined the immediate and 1-year outcomes of an individualized and intensive reading program for third grade students, which was delivered throughout the school day for an average of 189 hr of instruction over 3 months. These students' performances were compared with two comparison groups, including poor readers who received small group supports and good readers who did not have additional reading instructio

Evaluating the Impact of a Multistrategy Inference Intervention for Middle-Grade Struggling Readers.

PURPOSE: We examined the effectiveness of a multistrategy inference intervention designed to increase inference making and reading comprehension for middle-grade struggling readers. METHOD: A total of 66 middle-grade struggling readers were randomized to treatment (n = 33) and comparison (n = 33) conditions. Students in the treatment group received explicit instruction in 4 inference strategies (i.e., clarification using text clues; activating and using prior knowledge; understanding character perspectives and author's purpose; answering inferential questions). Academic characteristics of self-identified illiterates.

The purpose was a descriptive account of the academic characteristics of 35 adults (22 men and 13 women) who perceived themselves as needing help in reading. Ages of participants ranged from 16 through 60 years. All had completed Grade 6, mean of Grade 11. Sixteen were functionally illiterate, that is, read below Grade 5. Twenty-seven were unemployed. The most common deficiencies found were in phonics, comprehension, and perception. All aspects of phonics, but especially the sounds of the short vowels, were a problem. Difficulties in perception were evident in reversals of letters and words, m

Rehabilitation of alexia: a case study.

This case study described the success of a technique labeled Multiple Oral Rereading (MOR) in the remediation of a case of acquired alexia in an adult male. Unusually slow visual-verbal processing characteristic of this patient is sometimes noted in dyslexic children. The possibility that MOR might prove effective for other selected cases of acquired alexia and "developmental" dyslexia is considered.

The effectiveness of Additional Literacy Support (ALS) in Years 3 and 4.

This study compared the progress in reading and spelling of 256 children in 11 classes in 9 English primary schools in Years 3 and 4, and a partially overlapping sample of 126 children who received additional help with literacy during 1 year. Teachers and teaching assistants used either Additional Literacy Support (ALS), a highly structured set of small group teaching materials devised by the English National Literacy Strategy, or a wide variety of other materials including other published intervention programmes, reading scheme-based, computer-based and individually designed interventions, or

Orthographic processing efficiency in developmental dyslexia: an investigation of age and treatment factors at the sublexical level.

Reading fluency beyond decoding is a limitation to many children with developmental reading disorders. In the interest of remediating dysfluency, contributing factors need to be explored and understood in a developmental framework. The focus of this study is orthographic processing in developmental dyslexia, and how it may contribute to reading fluency. We investigated orthographic processing speed and accuracy by children identified with dyslexia that were enrolled in an intensive, fluency-based intervention using a timed visual search task as a tool to measure orthographic recognition. Effects of three interventions on the reading skills of children with reading disabilities in grade 2.

In a longitudinal intervention study, the effects of three intervention strategies on the reading skills of children with reading disabilities in Grade 2 were analyzed. The interventions consisted of computerized training programs: One bottom-up intervention aimed at improving word decoding skills and phonological abilities, the second intervention focused on top-down processing on the word and sentence levels, and the third was a combination of these two training programs (n = 25 in each group). In addition, there were two comparison groups, 25 children with reading disabilities who received

Early identification and remediation of phonological-processing deficits in first-grade children at risk for reading disabilities.

The present study assessed 486 first-quarter first graders on their reading and phonological-processing skills and intelligence. Based on this assessment, and using the classification data from Hurford et al.'s (1993) study, 99 children were identified as being at risk for reading difficulties: 53 children at risk for reading disabilities (RD) and 46 children at risk for becoming "garden-variety" poor readers (GV). Half of the RD and GV groups received the phonological-processing intervention. Posttraining assessment indicated that the training procedure not only was effective in increasing th
