Importance of monitoring zones in the detection of arrhythmias in patients with implantable cardioverter-defibrillators under remote monitoring.

INTRODUCTION: Implantable cardioverter-defibrillator (ICD) monitoring zones (MZ) provide passive features that do not interfere with the functioning of active treatment zones. However, it is not known for certain whether programming an MZ affects arrhythmia detection by the ICD. The aim of the present study is to assess the clinical relevance of MZ in a population of patients with ICDs. METHODS: In this retrospective analysis of patients with ICDs, with or without cardiac resynchronization therapy, for primary prevention under remote monitoring, the MZ was analyzed and recorded arrhythmias we

Remote Patient Monitoring With Continuous Glucose Monitoring Improves Glycemic Outcomes in Patients With Type 2 Diabetes at a Safety-Net Hospital.

OBJECTIVE: The Bronx has a high prevalence of type 2 diabetes mellitus (T2DM) but limited primary care access, delaying evaluation and treatment. Remote patient monitoring (RPM) with continuous glucose monitoring (CGM) may improve outcomes in T2DM. We evaluated whether RPM with CGM improved hemoglobin A1c (HbA1c) compared to standard care at our safety-net hospital. METHODS: This retrospective cohort study compared a pilot RPM diabetes program to a control cohort receiving standard care. RPM and control patients were ≥18 years old with T2DM and had HbA1c >8.5% (69 mmol/mol), and basal insulin

Practical implementation of remote continuous glucose monitoring in hospitalized patients with diabetes.

PURPOSE: Inpatient diabetes management involves frequent assessment of glucose levels for treatment decisions. Here we describe a program for inpatient real-time continuous glucose monitoring (rtCGM) at a community hospital and the accuracy of rtCGM-based glucose estimates. METHODS: Adult inpatients with preexisting diabetes managed with intensive insulin therapy and a diagnosis of coronavirus disease 2019 (COVID-19) were monitored via rtCGM for safety. An rtCGM system transmitted glucose concentration and trending information at 5-minute intervals to nearby smartphones, which relayed the dat

Mechanisms of inappropriate defibrillator therapy in a modern cohort of remotely monitored patients.

INTRODUCTION: Defibrillator (ICD) technology and monitoring are evolving rapidly. We investigated the mechanisms of inappropriate ICD therapies in a modern cohort of patients followed at our institution via remote monitoring. METHODS: From September 2009 to March 2011, a total of 2,050 ICD patients (19,600 patient-months) were remotely followed. All events (shocks and antitachycardia pacing) were adjudicated by arrhythmia specialists. RESULTS: A total of 249 patients received ICD therapy (34% inappropriate therapy). Inappropriate ICD shocks affected 33 (1.6%) patients. There were a total of

Patient Communication Outside of Visits: Implications for Remote Monitoring and Digital Care Models in Myasthenia Gravis.

INTRODUCTION/AIMS: Myasthenia gravis (MG) is a rare autoimmune disorder requiring individualized, specialized treatment, yet access to specialized care is limited and documentation fragmented. Several remote monitoring tools have emerged to address this gap, yet their integration into clinical care remains challenging. The aim was to better understand the nature and context of care-related interactions in order to inform the development of clinically meaningful and patient-centered digital health solutions for MG. METHODS: We conducted a survey among physicians and nurses at a tertiary MG cen

Variations in provider practices in remote patient monitoring on peritoneal dialysis in the USA and Canada.

Automation has allowed clinicians to program PD treatment parameters, all while obtaining extensive individual treatment data. This data populates in a centralized online platform shortly after PD treatment completion. Individual treatment data available to providers includes patients' vital signs, alarms, bypasses, prescribed PD treatment, actual treatment length, individual cycle fill volumes, ultrafiltration volumes, as well as fill, dwell, and drain times. However, there is no guidance about how often or if this data should be assessed by the clinical team members. We set out to determine

Response Rate Patterns in Adolescents With Concussion Using Mobile Health and Remote Patient Monitoring: Observational Study.

BACKGROUND: A concussion is a common adolescent injury that can result in a constellation of symptoms, negatively affecting academic performance, neurobiological development, and quality of life. Mobile health (mHealth) technologies, such as apps for patients to report symptoms or wearables to measure physiological metrics like heart rate, have been shown to be promising in health maintenance. However, there is limited evidence about mHealth engagement in adolescents with a concussion during their recovery course. OBJECTIVE: This study aims to determine the response rate and response rate pat
