Sleep Stage Classification of Pediatric Patients with Sleep-Disordered Breathing using Airflow Signals.

Sleep staging is a critical task for sleep health assessment. Recent work has shown that breathing variability may be characteristic of particular sleep stages. In this work, we train machine learning models on sleep airflow signals from pediatric patients to assess how well sleep stage can be classified from airflow signals, particularly in subjects with sleep-disordered breathing. We perform a 3-class classification task (Wake/NREM/REM) and achieve 66.9% balanced accuracy (BA) when classifying the sleep stage of healthy subjects, 61.4% BA when classifying the sleep stage of subjects with mil

Effect of ultrafiltration on sleep apnea and sleep structure in patients with end-stage renal disease.

RATIONALE: In end-stage renal disease (ESRD), a condition characterized by fluid overload, both obstructive and central sleep apnea (OSA and CSA) are common. This observation suggests that fluid overload is involved in the pathogenesis of OSA and CSA in this condition. OBJECTIVES: To test the hypothesis that fluid removal by ultrafiltration (UF) will reduce severity of OSA and CSA in patients with ESRD. METHODS: At baseline, on a nondialysis day, patients with ESRD on thrice-weekly hemodialysis underwent overnight polysomnography along with measurement of total body extracellular fluid volum

SleepMambaNet: a lightweight multimodal framework for sleep staging and sleep disorder screening with multicenter validation.

Objective.Manual sleep staging is labor-intensive, whereas many automated systems remain computationally demanding and are validated on a single dataset. This study aimed to develop a lightweight multimodal framework for automated sleep staging and exploratory sleep-disorder screening from polysomnographic (PSG) signals.Approach.SleepMambaNet combines efficient channel attention to integrate complementary information across PSG channels with bidirectional Mamba modules to model long-range temporal dependencies between sleep epochs. The framework was evaluated on the public ISRUC-S1 and ISRUC-S

Obstructive sleep apnea severity and morning hypertension in children: Age-stratified associations across developmental stages.

Obstructive sleep apnea (OSA) is common in children and has been linked to cardiovascular dysregulation, including elevated blood pressure (BP). This study examined whether OSA severity is independently associated with morning hypertension in a pediatric clinical cohort, and whether associations vary by developmental stage and sex. We retrospectively analyzed 806 children aged 1 to 18 years (55.2% Male) who underwent overnight polysomnography (PSG) and had BP recorded before and after the PSG at the University of Louisville and Norton Pediatric Sleep Center. OSA was categorized based on the to

The REM effect: How sleep stages influence learning strategies in complex reward-based decision-making.

Model-free (MF) and model-based (MB) learning strategies are complementary decision-making processes used in evaluating choices with potential rewards. Disorders involving compulsive behaviours (e.g., substance use, gambling) are suggested to emerge from an overreliance on MF learning, though the reasons for this bias remain unclear. Sleep disruptions, common in these disorders, could be a contributing factor, however no study has examined the impact of sleep and/or sleep loss on an individual's engagement of each strategy. Thus, this study examined the influence of sleep on MF/MB learning in

Association of Sleep Spindle Activity With Cognitive Decline in Early Clinical Stages of Alzheimer Disease.

BACKGROUND AND OBJECTIVES: Sleep architecture, including spindles and slow oscillations, is disrupted in Alzheimer disease (AD). How changes in these sleep elements relate to cognitive decline is less clear. Our objectives were to examine changes in sleep macroarchitecture and microarchitecture in early clinical stages of AD compared with older adult controls (CTLs) and to investigate their associations with longitudinal cognitive change. METHODS: This was both a cross-sectional and longitudinal study performed at Mass General Brigham Hospitals, where participants with early AD or CTLs underw

Memory consolidation and inducible nitric oxide synthase expression during different sleep stages in Parkinson disease.

BACKGROUND: Parkinson disease (PD) is a neurodegenerative disease characterized by motor and nonmotor dysfunctions, which include sleep disturbances. Rapid eye movement (REM) sleep is associated with numerous physiologic changes such as memory consolidation. Compelling evidence suggests that nitric oxide (NO) is crucial to both sleep regulation and memory consolidation. In our study, we explored changes in biologic molecules during various sleep stages and the effects of sleep on memory consolidation in PD. METHODS: Ten PD patients and 14 volunteers without PD participated in our study. The g

Segmenting cardiac-related data using sleep stages increases separation between normal subjects and apnoeic patients.

Inter-patient comparisons of cardiovascular metrics indicative of patient health have been shown to be successful in differentiating patients on a group rather than an individual level. This is in part due to the range of mental (as well as physical) activity-based variations for each patient and the difficulty assessing physical and mental activity during conscious states. In order to provide an objective scale for measuring central nervous system activity during sleep, the heart rate (RR) interval time series is divided into coarse sleep stage segments in which the LF/HF-ratio (the relative

[A study of the relationship between nocturnal intragastric pH and sleep stages of peptic ulcer].

We studied the correlation of the gastric acid secretion and the depth of sleep at night with no medication in 32 cases (19 gastric ulcer, 6 duodenal ulcer patients and 7 healthy controls). Using Memory PH: Monitor intragastric pH values were consecutively measured without antiulcer drugs. The depth of sleep were monitored continuously by recording without any sleeping drugs. In healthy controls, the pH values of awake phase was significantly lower than that of sleep stage 1, 4 and rapid eye movement (REM) phases. The pH values of REM phases were significantly high rather than that of sleep st

Dissociation of sleep stages between the two hemispheres in a case with unilateral thalamic tumor.

A case with an astrocytoma in the right basal ganglia, suspected of having originated from the posterior ventro-lateral part of the thalamus, without any clinical findings indicating severe internal hydrocephalus and cortical damaged, showed dissociation of sleep stages between the two hemispheres, i.e., the affected side always fell earlier into deeper stages of sleep and was more difficult to arouse by sonic stimulations than the other side. It is assumed that this bilateral dissociation with ipsilateral deactivation is due to the severe unilateral reduction of EEG activating influences from

Emotional Processing in a Nap Paradigm: The Role of Sleep Stages in Memory and Reactivity.

It is widely known that sleep has a role in modulating emotional processing. However, while sleep facilitates the consolidation of emotional memories, its influence on emotional reactivity remains unclear. Most prior work has not fully addressed emotional reactivity, often relying solely on subjective or physiological measures, nor has it disentangled the contribution of different sleep stages. Here, using a between-subjects design, we asked participants to complete two picture-based tasks assessing emotional memory and emotional reactivity before and after a manipulation condition (nap, N = 4

Daily changes in sleep stages and associated cardiovascular parameters during pregnancy: using a wearable device.

OBJECTIVE: Growing evidence links sleep disturbances with gestational hypertension and/or preeclampsia. Most studies rely on self-reported surveys that cannot capture sleep stages as objective indicators of cardiovascular health. The objective of this study was to utilize a wearable device to describe changes in nightly sleep stages during pregnancy and investigate their relationships with resting heart rate (HR) and heart rate variability (HRV). METHODS: This longitudinal descriptive study encompassed 981 observation days among 14 pregnant women, 86% of whom self-identified as Hispanic. The

Spectral and Microstate EEG Analysis in Narcolepsy Type 1 and Type 2 Across Sleep Stages.

BACKGROUND: The primary distinction between narcolepsy type 1 (NT1) and narcolepsy type 2 (NT2) is the presence or absence of cataplexy, which is commonly determined through clinical interviews, though it can be prone to error due to vague patients descriptions. OBJECTIVE: This study aimed to investigate EEG microstate differences between NT1 and NT2 and their correlation with clinical assessments. METHODS: Polysomnography (PSG) and the Multiple Sleep Latency Test (MSLT) were performed on 14 NT1 and 13 NT2 patients from three hospitals, with data from the ISRUC-SLEEP dataset serving as the c

Sleep disturbances by disease type and stage in Huntington's disease.

INTRODUCTION: Sleep disturbances are a common symptom in patients with Huntington's disease (HD). However, it is unclear when in the disease course of HD sleep disturbances become more frequent compared to the general population. This study investigated the frequency and odds of developing sleep disturbances between adults with HD or at-risk for HD and non-HD controls. METHODS: Participants from the Enroll-HD study were split by both disease type and disease severity using CAG length, diagnostic confidence level, and total functional capacity score. Multivariate logistic regression was used t

Sleep stage detection using only heart rate.

Getting enough quality sleep plays a vital role in protecting our mental health, physical health, and quality of life. Sleep deprivation can make it difficult to concentrate on daily activities, and lower sleep quality is associated with hypertension, hyperglycemia, and hyperlipidemia. The amount of sleep we get is important, but in recent years, quality sleep has also been deemed significant. Polysomnography, which has been the gold standard in assessing sleep quality based on stages, requires that the subject be attached to electrodes, which can disrupt sleep. An easier method to objectively

Effect of sleep stage on breathing in children with central hypoventilation.

The early literature suggests that hypoventilation in infants with congenital central hypoventilation syndrome (CHS) is less severe during rapid eye movement (REM) than during non-REM (NREM) sleep. However, this supposition has not been rigorously tested, and subjects older than infancy have not been studied. Given the differences in anatomy, physiology, and REM sleep distribution between infants and older children, and the reduced number of limb movements during REM sleep, we hypothesized that older subjects with CHS would have more severe hypoventilation during REM than NREM sleep. Nine subj

[Diagnosis of sleeping sickness stage: towards a new approach].

Diagnosis of the neurological disease stage in Trypanosoma brucei (T.b.) gambiense infection is essential to select an optimal chemotherapy. The actual parameters for stage determination, the cerebrospinal fluid (CSF) cell count, total protein concentration and trypanosome detection, are insufficiently specific and sensitive. In order to identify new parameters for stage determination, we studied the neuro-inflammatory immune response in the central nervous system, notably the intrathecal humoral immune response in sleeping sickness patients. The presence of intrathecal IgM synthesis was ident

Instantaneous change in sleep stage with noise of a passing truck.

The transient effects of passing truck noise on sleep polygraphs of 8 men were studied. The percentage of the sleep Stage 2 which changed from Stage 2 to Stage 1, waking or movement time, was calculated. The shallowing of REM sleep was identified when it had changed to other stages, and the shallow epoch percentage was calculated for the REM sleep. The percentage of shallow epochs for Stage 2 significantly increased against that of the control condition after exposure to noise but was not statistically significant for Stage REM. In conclusion, the shallowing or changes in the sleep Stage 2 or

Atropine effects on the intermediate stage and paradoxical sleep in rats.

Paradoxical sleep (PS) in the rat, cat and mouse is preceded and sometimes followed by a short-lasting intermediate stage (IS) characterized by high amplitude cortical spindles and low frequency theta rhythm. This stage, which is mimicked by an intercollicular transection, is massively extended at the expense of PS by low doses of barbiturates. Since the pontine cholinergic cell activation of PS is suppressed by barbiturates, we studied whether atropine, an antimuscarinic compound, extends IS at the expense of PS. Atropine sulfate was given at 5, 10 and 20 mg/kg IP. All doses increased dose de
