Smoking cessation for improving mental health.

BACKGROUND: There is a common perception that smoking generally helps people to manage stress, and may be a form of 'self-medication' in people with mental health conditions. However, there are biologically plausible reasons why smoking may worsen mental health through neuroadaptations arising from chronic smoking, leading to frequent nicotine withdrawal symptoms (e.g. anxiety, depression, irritability), in which case smoking cessation may help to improve rather than worsen mental health. OBJECTIVES: To examine the association between tobacco smoking cessation and change in mental health. SE

System change interventions for smoking cessation.

BACKGROUND: System change interventions for smoking cessation are policies and practices designed by organizations to integrate the identification of smokers and the subsequent offering of evidence-based nicotine dependence treatments into usual care. Such strategies have the potential to improve the provision of smoking cessation support in healthcare settings, and cessation outcomes among those who use them. OBJECTIVES: To assess the effectiveness of system change interventions within healthcare settings, for increasing smoking cessation or the provision of smoking cessation care, or both.

Smoking cessation strategies in vascular surgery.

Tobacco abuse is a highly prevalent modifiable risk factor in vascular surgery patient populations. Despite the known benefits of smoking cessation, quitting smoking is difficult for most patients. Physician advice to stop smoking can help, though more intensive or multifactorial interventions have greater impact. Smoking cessation initiatives based in vascular clinics are feasible, although currently there is significant variation in physician delivery of smoking cessation interventions. Vascular surgeons are optimally poised to be able to capitalize on the "teachable moment" of the vascular

[Smoking cessation therapy in thoracic oncology].

In France, the number of tobacco-related deaths is estimated at 73,000 per year, including 44,000 from cancer and more than 20,000 from lung cancer (LC). Smoking cessation is the most effective measure to reduce the epidemic of LC, but it is also important in the management of patients with LC regardless on stage. In localized cancers, continuing to smoke is associated with decreased survival by increasing the risk of recurrence and of developing a second cancer. During the perioperative period, smoking cessation reduces infectious complications and length of hospitalization. At all stages of

Smoking cessation strategies in Crohn's disease.

Although smoking cessation is one of the most effective interventions in patients with Crohn's disease, the prevalence of smoking in these patients is higher than in the general population, and much higher than in patients with other smoking-related diseases. There is a lack of awareness of the benefits of cessation among patients and general practitioners, and a lack of engagement in smoking cessation by specialists. This article discusses the strategies that practitioners can use to motivate patients to quit and how the success of a quitting attempt can be maximised.

Smoking cessation alters subgingival microbial recolonization.

Smoking cessation improves the clinical manifestations of periodontitis; however, its effect on the subgingival biofilm, the primary etiological agent of periodontitis, is unclear. The purpose of this study was to investigate, longitudinally, if smoking cessation altered the composition of the subgingival microbial community, by means of a quantitative, cultivation-independent assay for bacterial profiling. Subgingival plaque was collected at baseline, and 3, 6, and 12 months post-treatment from smokers who received root planing and smoking cessation counseling. The plaque was analyzed by term

Health consequences of sustained smoking cessation.

OBJECTIVES: To estimate the risk of dying from all causes and from specified smoking-related diseases in people who were ex-smokers at two consecutive examinations, compared with never smokers and with people who were ex-smokers at the first examination but who had resumed smoking at the following examination. DESIGN: A prospective cohort study. SETTING: Three counties in Norway. PARTICIPANTS: A total of 23 560 men and 25 122 women, aged 20-49, screened for cardiovascular disease risk factors in the mid 1970s, rescreened after 3-13 years and followed throughout 2005 participated in this stu

Smoking cessation interventions in clinical practice.

OBJECTIVES: Physicians are in a unique position to advise smokers to quit by integrating the various aspects of nicotine dependence. This review provides an overview of interventions for smokers presenting in a clinical setting. RESULTS: Strategies used for smoking cessation counselling differ according to patient's readiness to quit. For smokers who do not intend to quit smoking, physicians should inform and sensitise them about tobacco use and cessation. For smokers who are dissonant, physicians should use motivational strategies, such as discussing barriers to cessation and their solutions

Smoking cessation intervention in clinical practice.

Physicians are in a unique position to advise smokers to quit because of their ability to integrate the various aspects of nicotine dependence. This review provides an overview on interventions with smokers presenting in a primary care setting. Strategies used for smoking cessation counseling differ according to patient's readiness to quit. For smokers who do not intent to quit smoking, physicians should inform and sensitize patients about tobacco use and cessation, especially by personalizing benefits to quit and challenging smokers' beliefs. For smokers who are insecure, physicians should us

Smoking cessation in drug treatment programs.

Substance abusers are more likely to smoke cigarettes than the general population. Yet, in most drug treatment programs smoking cessation receives little attention. The purposes of this study were to: (1) examine substance abuse counselors' attitudes toward and practices related to nicotine addiction and smoking cessation treatment; and (2) describe barriers to smoking cessation treatment and smoking policies in substance abuse treatment facilities. A total of 254 substance abuse counselors in Kentucky completed mailed questionnaires (53% response rate). Almost one-fourth were current smokers.

Transdermal nicotine replacement and smoking cessation.

Transdermal nicotine systems are a useful adjunct to smoking cessation. The larger-sized nicotine patches provide steady-state replacement of approximately 50 percent of the average serum nicotine level obtained by smoking one to one and one-half packs of cigarettes a day. This level is sufficient to prevent many symptoms of nicotine withdrawal, including physical craving, irritability, anxiety, restlessness, hostility, hunger and difficulty concentrating. In double-blind, placebo-controlled trials, transdermal nicotine systems have been about twice as successful as placebo in helping patients

Smoking Cessation and Incident Cardiovascular Disease.

IMPORTANCE: The association between smoking cessation and cardiovascular disease (CVD) risk in relation to cumulative smoking exposure remains poorly understood. OBJECTIVE: To evaluate the associations among smoking cessation, lifetime smoking burden, and CVD risk according to the number of years elapsed after smoking cessation. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study of the Korean National Health Insurance Service database investigated smoking duration and intensity between January 2006 and December 2008. Participants were categorized by self-reported smoking habi

Chatbots for Smoking Cessation: Scoping Review.

BACKGROUND: Despite significant progress in reducing tobacco use over the past 2 decades, tobacco still kills over 8 million people every year. Digital interventions, such as text messaging, have been found to help people quit smoking. Chatbots, or conversational agents, are new digital tools that mimic instantaneous human conversation and therefore could extend the effectiveness of text messaging. OBJECTIVE: This scoping review aims to assess the extent of research in the chatbot literature for smoking cessation and provide recommendations for future research in this area. METHODS: Relevant

Current treatment options in smoking cessation.

Cigarette smoking is the most important modifiable risk factor for premature mortality. Many common and serious diseases, including coronary artery disease, chronic obstructive lung disease, stroke, and cancer, are strongly linked to cigarette smoking. Smoking cessation is difficult due to nicotine addiction and withdrawal symptoms. Comprehensive programs for tobacco control can substantially reduce the frequency of tobacco use. Physicians can improve screening and increase cessation rates by asking patients about tobacco use at every office visit. The spectrum of available smoking cessation i

Non-nicotinic therapies for smoking cessation.

Cigarette smoking is the primary cause of numerous preventable diseases; as such, the goals of smoking cessation are both to reduce health risks and to improve the quality of life. Currently, the first-line smoking cessation therapies include nicotine replacement products and bupropion. The nicotinic receptor partial agonist varenicline has recently been approved by the FDA for smoking cessation. A newer product currently under development and seeking approval by the FDA are nicotine vaccines. Clonidine and nortriptyline have demonstrated some efficacy but side effects may limit their use to s

Serious mental illness and smoking cessation.

Persons with serious mental illness (SMI) are faced with substantial challenges to their health. This population is two to three times more likely to smoke cigarettes than persons who do not suffer from mental illness. In particular, young adults are at high risk for vulnerability to both SMI and cigarette smoking. Although there are proven methods for smoking cessation, both pharmacologic and non-pharmacologic interventions show limited usefulness for SMI who smoke. Alternative health care options as well as support groups and physical exercise are discussed as methods that may be useful in s

Effectiveness of hospital-based smoking cessation.

STUDY OBJECTIVES: Smoking cessation for current smokers is a health-care imperative. It is not clear which approaches to smoking cessation are the most effective in the hospital setting and which factors predict long-term abstinence. We hypothesized that a hospital-based smoking cessation program involving behavioral modification and support would provide an effective intervention for smoking cessation. DESIGN: Prospective cohort study. SETTING: Smoking cessation clinics in a tertiary referral, cardiothoracic hospital. PATIENTS OR PARTICIPANTS: Two hundred forty-three smokers and 187 never-
