Neuromonitoring for Spinal Cord Stimulation Lead Placement Under General Anesthesia.

Spinal cord stimulation (SCS) is a common therapeutic technique for treating medically refractory neuropathic back and other limb pain syndromes. SCS has historically been performed using a sedative anesthetic technique where the patient is awakened at various times during a surgical procedure to evaluate the location of the stimulator lead. This technique has potential complications, and thus other methods that allow the use of a general anesthetic have been developed. There are two primary methods for placing leads under general anesthesia, based on 1) compound muscle action potentials and 2

Economic botany collections: A source of material evidence for exploring historical changes in Chinese medicinal materials.

ETHNOPHARMACOLOGICAL RELEVANCE: Many Chinese medicinal materials (CMMs) have changed over centuries of use, particularly in terms of their botanical identity and processing methods. In some cases, these changes have important implications for safety and efficacy in modern clinical practice. As most previous research has focused on clarifying the evolution of CMMs by analyzing traditional Chinese materia medica ("bencao") literature, assessments of historical collections are needed to validate these conclusions with material evidence. AIM OF THE STUDY: Historical collections of Chinese medicin

Challenges in non-radiographic axial spondyloarthritis.

Non-radiographic axial spondyloarthritis (nr-axSpA) is part of the spectrum or continuum of axSpA and is characterized by the absence of definite structural damage in the sacroiliac joints. The introduction of the concept of axSpA with two - non-radiographic and radiographic - stages stimulated activities related to the improvement of early diagnosis and treatment of this disease, but also opened a number of controversial topics in the field. In this article, we are discussing several challenges related to the concept of nr-axSpA, the diagnosis, and the treatment in the historical context and

[Antidepressants - stimulators for the release of norepinephrine and serotonin (history of creation, study of neurochemical effects and classification)].

The history of the creation and putting into practice of antidepressants and experimental agents - blockers of α2-adrenergic receptors and serotonin 5-HT2-receptors is described. The author analyzes the history of development of mianserin, mirtazapine and other drugs and their position in the classification of antidepressants. On the basis of a generalization of historical facts, the rationality of assigning mianserin, mirtazapine, and possibly other compounds similar in chemical structure and mechanism of action to one neurochemical group and its designation by the term 'stimulators of the re

Outcomes of a Program to Support Outpatient Antibiotic Adherence Among People Experiencing Housing Instability With Serious Injection Drug-Related Infections in Boston, MA: A Propensity Score Matched Study.

BACKGROUND: Antibiotics are effective for treating serious injection drug-related infections (SIRIs). However, completion rates are often low among people who inject drugs. We evaluated Boston Healthcare for the Homeless Program Infectious Disease Outreach Liaison Team (BOLT), an outpatient liaison program for people experiencing housing instability hospitalized with SIRI, intended to increase intravenous or oral antibiotic completion. METHOD: We included BOLT enrollees (1 June 2023 to 31 August 2024) and historical controls (1 June 2020 to 28 February 2023). Primary outcomes were the number

Role of epidermal growth factor in obstetrics and gynecology.

During the past 30 years, a number of growth factors have been isolated and characterized. These include insulin and insulin-like growth factors, the fibroblast growth factors, the hematopoietic colony-stimulating growth factors, and the epidermal growth factor (EGF) group. Epidermal growth factor, which stimulates the growth of a variety of tissues, has been extensively studied since its discovery in the early 1960s. This review presents an historic perspective, basic scientific aspects, laboratory methods, and the clinical application of EGF in the specialty of obstetrics and gynecology. Exp

Clostridium difficile colitis in the setting of subacute thyroiditis: the chicken or the egg.

Subacutethyroiditis is a self-limited inflammatory condition commonly of viral aetiology, that manifests through phases of thyroid hormone changes over a 6-8 month period. A 24-year-old active duty military man, undergoing treatment for recurrent Clostridiumdifficile infection, presented for clinical evaluation and was found to have a thyroid stimulating hormone level of 0.003 mg/dL. Further labs revealed a normal T4, elevated T3 at 5.0 pg/mL and elevated C reactive protein at 3.69 mg/L. The patient was followed with monthly labs and the abnormal thyroid stimulating hormone and triiodothyronin

Thyroid hormone action on skin.

PURPOSE OF REVIEW: To review the current understanding regarding thyroid hormone action on skin. To provide a historical context for the recent findings. RECENT FINDINGS: Although direct thyroid hormone actions have been demonstrated on multiple aspects of cutaneous biology, rigorous study remains scant. Still, there is a slowly evolving literature supporting the concept that thyroid hormone can directly stimulate epidermis, dermis, and hair. That action may be accessed to treat cutaneous disease. SUMMARY: Here, we review the literature regarding thyroid hormone action on skin along with ski

Use of an infrared camera to improve the outcome of facial nerve monitoring.

Intraoperative cranial nerve neuromonitoring (IONM) has significantly reduced morbidity historically associated with skull-base procedures. It provides the surgeon with immediate feedback to preserve both anatomic and physiologic function of monitored nerves. However, when the surgeon cauterizes tissue near the nerve, the monitoring equipment is saturated by the cautery signal, eliminating IONM information at a critical time. The use of an infrared camera under the drapes to observe the face for movement due to nerve stimulation is an invaluable adjunct to traditional IONM. Monitoring may also

Transcranial electrical stimulation nomenclature.

Transcranial electrical stimulation (tES) aims to alter brain function non-invasively by applying current to electrodes on the scalp. Decades of research and technological advancement are associated with a growing diversity of tES methods and the associated nomenclature for describing these methods. Whether intended to produce a specific response so the brain can be studied or lead to a more enduring change in behavior (e.g. for treatment), the motivations for using tES have themselves influenced the evolution of nomenclature, leading to some scientific, clinical, and public confusion. Self-reported pathways through which illicitly manufactured fentanyl enters the stimulant supply: Novel insights from people incarcerated for drug manufacturing and distribution.

BACKGROUND: The unintentional consumption of stimulants containing fentanyl among people who intend to only use stimulants contributes to overdose mortality in North America. Research exploring how fentanyl appears in the stimulant supply among people who manufacture and/or distribute drugs (PWDD) is critical to understanding supply-side factors that shape stimulant and opioid-involved overdose risk. METHODS: From April to July 2023, thirty PWDD incarcerated at the Rhode Island Department of Corrections completed an in-depth interview about stimulant and opioid-involved overdose. Data were th

Thromborepair: the tissue repair partner to thromboinflammation.

Platelets are small blood cells, historically viewed principally as mediators of hemostasis and thrombosis. However, it is now well established that platelets are pivotal modulators of the inflammatory response, establishing the concepts of thromboinflammation and immunothrombosis. Here, we propose the novel term "thromborepair" to describe how platelets promote tissue repair by facilitating leukocyte extravasation, resolving tissue inflammation, and coordinating fibrosis and angiogenesis. This review aims to discuss these processes and stimulate further exploration of platelet-based therapies

Premature progesterone elevation in controlled ovarian stimulation: to make a long story short.

Over the past decades many of us have contributed to the controversy surrounding the origins and consequences of premature progesterone elevation during controlled ovarian stimulation. In this article, we attempt to retrace the progression of information on this complex subject which required reviewing a number of publications that often contradicted one another. The definition of premature progesterone elevation, the pathophysiological mechanisms underlying the high peripheral progesterone levels, and the debated consequences of this event on in vitro fertilixation-embryo transfer outcome wil

Male reproductive hormones and thyroid function in pesticide applicators in the Red River Valley of Minnesota.

In the present effort, 144 pesticide applicators and 49 urban control subjects who reported no chronic disease were studied. Applicators provided records of the season's pesticides used by product, volumes, dates, and methods of application. Blood specimens for examination of hormone levels were obtained in summer and fall. In the herbicide-only applicator group, significant increases in testosterone levels in fall compared to summer and also elevated levels of follicle-stimulating hormone (FSH) and luteinizing hormone (LH) in the fall were noted. With respect to fungicide use, in an earlier c

The Problem with the Sex Toy Narrative: Design and Medical Intent in Early Vibrators.

This article challenges the dominant interpretation of early twentieth-century vibrators as inherently sexual objects. Rather than reading vibrators retrospectively through the lens of the modern sex toy industry, it examines hand-cranked and early electric devices from the perspective of design, use, and medical context. Drawing on artifacts, manuals, advertisements, and medical literature from Britain and the United States, the article argues that early vibrators were shaped primarily by therapeutic and cosmetic practices linked to massage medicine, gynecological therapy, and orificial philo

Prokinetics in the Management of Functional Gastrointestinal Disorders.

PURPOSE OF REVIEW: The goal of this review is to review the current status of prokinetics and to place it in historical context. Impaired motility and thus propulsion have long been thought to play important roles in the pathogenesis of a number of gastrointestinal disorders including gastroesophageal reflux disease (GERD), gastroparesis, chronic idiopathic pseudo-obstruction, and constipation. Historically, disordered motility was also thought to contribute to a number of functional gastrointestinal disorders such as functional dyspepsia (FD) and irritable bowel syndrome (IBS). History of surgery of the esophagus.

The progress of esophageal surgery from the beginning of medical history through five historical eras, up to the second half of the 20th century, is reviewed. Progress was slow from the first surgical repair of the esophagus in ancient Egypt in 2500 B.C. until the end of the 19th century, when scientific discoveries made possible the solid beginnings of abdominal surgery. Thoracic surgery followed in the 20th century, with rapid strides in World War II. These wartime advances stimulated an interest in esophageal surgery in the postwar era, when operative techniques became well standardized and

A Review of Parameter Settings for Invasive and Non-invasive Vagus Nerve Stimulation (VNS) Applied in Neurological and Psychiatric Disorders.

Vagus nerve stimulation (VNS) is an established form of neuromodulation with a long history of promising applications. Earliest reports of VNS in the literature date to the late 1800's in experiments conducted by Dr. James Corning. Over the past century, both invasive and non-invasive VNS have demonstrated promise in treating a variety of disorders, including epilepsy, depression, and post-stroke motor rehabilitation. As VNS continues to rapidly grow in popularity and application, the field generally lacks a consensus on optimum stimulation parameters. Stimulation parameters have a significant

Designing a bioelectronic treatment for Type 1 diabetes: targeted parasympathetic modulation of insulin secretion.

The pancreas is a visceral organ with exocrine functions for digestion and endocrine functions for maintenance of blood glucose homeostasis. In pancreatic diseases such as Type 1 diabetes, islets of the endocrine pancreas become dysfunctional and normal regulation of blood glucose concentration ceases. In healthy individuals, parasympathetic signaling to islets via the vagus nerve, triggers release of insulin from pancreatic β-cells and glucagon from α-cells. Using electrical stimulation to augment parasympathetic signaling may provide a way to control pancreatic endocrine functions and ultima
