Donor graft harvesting with air in pterygium surgery.

PURPOSE: Report a new technique for improvement of pterygium surgery: donor graft harvesting with air. CASE REPORTS: We describe a technique that was used with 138 patients who underwent surgery that included a conjunctival autograft and fibrin glue with air injection to dissect the Tenon capsule from the conjunctiva. RESULTS: No complications occurred with this technique and all patients achieved good results. DISCUSSION: The surgical time with this new technique was similar to that reported in the literature. In addition, the graft thickness was close to that reported with femtosecond las

Use of biodegradable collagen-glycosaminoglycan copolymer matrix implant to reduce postoperative fibrosis in strabismus surgery.

The occurrence of restrictive strabismus after conjunctival surgeries is widely described in the literature. Fibrosis causes adherence of the conjunctiva to the Tenon's capsule, intermuscular membrane, adipose orbital tissue, sclera, and extraocular muscle tissue. Fibrosis causes adherence of the conjunctiva to the Tenon's capsule, intermuscular membrane, adipose orbital tissue, sclera, and extraocular muscle tissue. We report a case of the use of a biodegradable collagen-glycosaminoglycan copolymer matrix implant (Ologen®) as an alternative adjuvant, used as a spacer to mechanically separate

Complications of dermis-fat orbital implantation.

Primary dermis-fat implantation can be effectively performed in cases without pre-existing systemic vascular disease or orbital burns. Meticulous handling of the graft (using a Goeller trephine and Tenon's capsule traction sutures), filleting Tenon's capsule, and avoiding cautery of the graft bed may minimize graft necrosis and atrophy. Pyogenic granulomas of the conjunctival-graft interface and graft hirsutism are easily managed. Keratinization of the socket, graft wound dehiscence, donor site hematomas, and wound dehiscence are avoided with careful surgical technique. Secondary dermis-fat or

Suture loop to aid in ganciclovir implant removal.

BACKGROUND: The ganciclovir implant (Vitrasert; Bausch & Lomb Inc, Claremont, Calif) has been a useful device in the management of cytomegalovirus retinitis. Seven months after placement, implant exchange is often considered. However, removal may be difficult and may result in vitreous hemorrhage or intraocular dislocation. OBJECTIVE: To describe a suture loop attached to the implant strut and left in the sub-Tenon space to aid in implant removal. METHODS: Twenty-five eyes of 17 patients received a ganciclovir implant with an 8-0 nylon suture loop left beneath the Tenon capsule and underwent

The role of Tenonplasty in the management of limbal and scleral ischemia due to acute ocular chemical burns.

Of the various manifestations of ocular chemical burns (OCBs), ischemia of the limbus and the peri-limbal sclera indicates poor prognosis and in severe cases threaten the integrity of the globe. Tenonplasty is a surgical procedure which involves advancing the Tenon's capsule over the ischemic areas to provide a vascular supply and to enable migration of the conjunctival epithelium. This review aims to provide an overview of the diagnosis of limbal ischemia and its management with Tenonplasty. A literature review was conducted using the keywords "Tenonplasty," "Tenon's capsule," "ocular chemica

Sub-Tenon's anaesthesia: an efficient and safe technique.

AIM: To evaluate sub-Tenon's anaesthesia as an alternative to peribulbar anaesthesia. METHODS: 109 consecutive patients listed for various eye operations (including cataract, trabeculectomy, and vitrectomy) under peribulbar anaesthesia were operated on under sub-Tenon's anaesthesia instead. After topical anaesthesia a buttonhole was fashioned through the conjunctiva and Tenon's capsule 10 mm posterior to the limbus. 1.5 ml of lignocaine 2% was then delivered to the posterior sub-Tenon's space using a blunt cannula. The surgical procedure was performed immediately after the completion of the a

A Single- Versus Double-Layered Closure Technique in Anophthalmic Surgery.

PURPOSE: To compare the frequency of orbital implant exposure and extrusion following eye removal with a simplified closure technique, closing Tenon's capsule and conjunctiva in 1 layer versus the classic technique of closure in 2 separate layers. METHODS: The authors conducted a retrospective case note review of patients who underwent evisceration or enucleation treated by 1 surgeon between 2001 and 2013. Between 2001 and 2004, Tenon's capsule and conjunctiva were closed in separate layers; after 2004, a simplified 1-layer closure following eye removal was used. The primary outcome parameter

Lacrimal dilator-facilitated incisionless vs. standard sub-Tenon's block: a randomized, prospective and non-inferiority comparative study.

BACKGROUND/OBJECTIVES: Standard sub-Tenon's block (STB) involves incision (dissection) of conjunctiva and Tenon's capsule with the help of blunt scissors and forceps, insertion of a blunt sub-Tenon's cannula under the Tenon's capsule and injection of local anaesthetic agent. STB is frequently associated with minor complications such as chemosis and postoperative subconjunctival haemorrhage but rare sight and life-threatening complications. To reduce these minor complications, several variations of incisionless STB have been described however, there are no comparative data. One such incisionles

Removal of Tenon fortified by conjunctival-limbal autograft in treatment of pterygium.

PURPOSE: To describe a simple and effective technique for the prevention of pterygium recurrence. METHODS: This is a retrospective consecutive single-center case series with recurrence rate as the main outcome measure. Dissection of the conjunctiva, the Tenon's layer, and the sclera was continued up to the insertion of medial rectus. The exposed Tenon's layer was excised for 2 to 3 mm. The entire free edges of the conjunctiva was tacked firmly to the sclera. The conjunctival-limbal graft was sutured firmly to the sclera, recipient limbus, and to the conjunctiva incorporating the reciprocal ed

Inhibition of AGTR1 attenuates cell proliferation after glaucoma filtration surgery via NF-κB pathway-mediated G0/G1-phase cell cycle arrest.

Fibroblast proliferation after glaucoma filtration surgery (GFS) plays a pivotal role in scar formation. Angiotensin type 1 receptor (AGTR1) is involved in tissue remodeling. Our previous study demonstrated that treatment with an AGTR1 blocker prolonged the survival of filtering blebs following GFS. However, whether AGTR1 participates in fibroblast proliferation after GFS remains unclear. This study examined the mechanisms underlying the involvement of AGTR1 in the activation of cell proliferation following GFS. AGTR1 expression was increased in Tenon capsule tissue of patients with glaucoma.

Tenon's patch graft for sterile perforation: follow-up with serial, high-resolution, anterior-segment, optical coherence tomography.

Management of corneal perforation depends on the size, location and cause of the lesion. Smaller perforations can be managed with tissue adhesives (TAs), amniotic membrane transplantation or Tenon's patch graft (TPG). Tenon's capsule is an autologous transplant and, as such, does not require to be supplied by an eye bank support. A patient with sterile perforation after foreign body removal came to us. Since the application of TA was already unsuccessful, TPG was performed. The graft integrated well and healed with a vascularised scar. We report the integration of the TPG at the perforation si

A Conjunctival Retractor during Strabismus Surgery.

The use of a conjunctival retractor that expands a small incision in the conjunctiva/Tenon's capsule facilitated the performance of strabismus surgery through a fornix incision. A wide field of exposure revealed the entire area of the muscle insertion. While there is a risk of conjunctival tearing in elderly patients, the use of the retractor is valuable.
