A 44-year-old man with total thyroidectomy for papillary thyroid carcinoma (PTC) performed at the age of 38 years was referred for mediastinal metastases demonstrated on chest radiograph and high serum thyroglobulin (Tg 328 ng/ml). Computed tomography revealed mediastinal lymph node enlargement with left bronchial compression and reduction of bronchial diameter, as well as two metastases in the left lung. Bronchoscopic biopsy findings showed a poorly-differentiated PTC, while a whole body scan after 131-Iodine therapy demonstrated uptake in the right subclavicular region. External beam radioth

Concomitant thyroid cancer with pituitary tumor is uncommon. This study reports a case of advanced papillary thyroid carcinoma with pituitary adrenocorticotropic hormone (ACTH)-secreting tumor. A 58-year-old male patient had thyroid cancer in 1991 and presented with headache caused by pituitary tumor with apoplexy in 1993. Due to hypopituitarism, the patient underwent radioactive iodide ((131)I) for detection and treatment of metastatic thyroid cancer after the use of recombinant human thyroid-stimulating hormone (rhTSH) in 2000. During follow-up for thyroid cancer, (201)thallium scan proved t

Adrenal gland metastasis from papillary thyroid carcinoma (PTC) is a rare and diagnostically challenging event. Approximately 15 cases of adrenal involvement have been published to date. We report a man in his 70s with previously treated PTC (pT2N1bMx) who re-presented with right cervical lymph node recurrence and a right adrenal mass measuring 5.7×6.5 cm detected on staging cross-sectional imaging. A CT-guided biopsy confirmed metastatic PTC. We performed staged surgery - right cervical lymph node dissection followed by open right adrenalectomy 4 weeks later. Serum thyroglobulin fell from >50

IMPORTANCE: Unlike patient age, which has been a well-established and routinely used primary biological prognostic factor in the risk stratification of papillary thyroid cancer (PTC), the prognostic value of patient sex remains controversial in this common endocrine malignant neoplasm.
OBJECTIVE: To examine PTC-specific mortality risk of sex with respect to patient age and tumor genetics-specifically the status of BRAF V600E and TERT promoter (TERTp) mutations.
DESIGN, SETTING, AND PARTICIPANTS: This cohort study was conducted among patients with PTC with an overall median (IQR) follow-up ti

This case report highlights a rare association between membranous nephropathy (MN) and papillary thyroid carcinoma (PTC) in a 66-year-old female with progressive renal dysfunction. Following a confirmed diagnosis of MN with negative phospholipase A2 receptor (PLA2R) staining, imaging revealed a thyroid malignancy, and total thyroidectomy led to clinical improvement, suggesting a paraneoplastic mechanism. This case underscores the importance of oncologic screening in MN patients and the need for further research into the pathophysiological links between MN and thyroid cancer.

KEY CLINICAL MESSAGE: When managing patients with differentiated thyroid cancers (DTC) and lytic bone lesions, physicians should consider etiologies other than DTC bony metastases when there is no biochemical and functional radiographic evidence of extensive DTC burden.
ABSTRACT: Systemic mastocytosis (SM) is a clonal expansion of mast cells associated with an increased risk of solid malignancies. There is no known association between systemic mastocytosis and thyroid cancer. We report a young woman who presented with cervical lymphadenopathy, palpable thyroid nodule, and lytic bone lesions w

Prophylactic central compartment neck dissection (pCCND) is a CCND in patients with thyroid cancer who have no clinical, sonographic, or intraoperative evidence of abnormal lymph nodes. Whether pCCND should be performed in all patients with clinically node-negative papillary thyroid cancer (PTC) is controversial. Existing data reveal that for patients with clinically node-negative PTC there is no difference between treatment with total thyroidectomy plus pCCND and total thyroidectomy alone. The potential increased risk of hypoparathyroidism associated with CCND is not offset by any measurable

OBJECTIVE: Dermatomyositis (DM) is considered a paraneoplastic phenomenon and cancer may precede or follow the development of clinical features by several years. Despite the prevalence of thyroid cancer, reports of an association are rare. We report 3 cases of dermatomyositis and thyroid cancer, focusing on the clinical course of the rheumatologic condition following thyroidectomy. We also performed a comprehensive review of the current literature.
METHODS: We performed a chart review between 1960 and 2012 to identify patients with DM and papillary thyroid cancer (PTC) seen in Mayo Clinic, Ro

Papillary thyroid cancer (PTC) is the most common endocrine malignancy and commonly metastasizes to regional lymph nodes. Surgical treatment of cervical lymph nodes in PTC remains controversial. It has traditionally been accepted that regional lymph node metastases in PTC may increase local recurrence rates but do not ultimately affect survival. This conventional wisdom has been challenged by recent reports indicating that regional lymph node metastases do increase mortality. Thus, there has been renewed interest in operative control of nodal disease for PTC. A systematic review of central lym

Patients with celiac disease have an increased rate of malignancies that are not limited to lymphomas. Thyroid carcinoma has not previously been associated with celiac disease. However, among a cohort of patients with celiac disease, we identified an increased risk of papillary carcinoma of the thyroid, standard morbidity ratio of 22.52 (95% confidence interval 14.90-34.04; P < .001), compared to United States national surveillance data. These patients were on a gluten-free diet. Only 1 had Hashimoto's thyroiditis, suggesting that mechanisms apart from autoimmune thyroiditis contribute to the

The tall-cell variant of papillary thyroid carcinoma (TCV) has been described as an aggressive tumor with a significantly higher incidence of recurrence and mortality than other forms of papillary carcinoma. In some series it has accounted for up to 10%, whereas in other series it has not been reported at all, indicating that there are difficulties identifying it. In a series of 162 consecutively treated patients with papillary thyroid carcinoma treated by total thyroidectomy according to a highly standardized procedure, all specimens were specifically examined by an international group of pat

The recent report by Gao and colleagues highlights an infrequent yet clinically significant manifestation of papillary thyroid carcinoma, PTC: isolated cutaneous metastasis appearing 8 years post-resection. While the rarity of the case is undeniable, this critique explores the nuances of the diagnostic discordance observed-specifically the thyroglobulin, Tg, negativity in the metastatic lesion despite PAX-8 and TTF-1 positivity-and challenges the authors' hypothesis regarding the role of thyrotropin suppression. Furthermore, we examine the therapeutic decision to pursue conservative local exci

Papillary thyroid cancer is one of the most common malignancies in pediatrics, with increasing prevalence. We analyzed the single-cell transcriptomic landscape from 11 pediatric patients. The compositions and functions in the tumor microenvironment showed remarkable differences. Endo_tip was primarily from tumor, receiving angiogenesis-associated signals from epithelia. Abundant immune cells infiltrated into tumor. SPP1+ M2-macrophage and tumor cells interacted with T cells via immunosuppressive ligand-receptor pairs. Trajectory inference identified genes in evolutionary branchpoints that may

In differentiated thyroid cancer (DTC), the incidence of bone metastases of papillary thyroid cancer (PTC) is relatively low, with primary sites including the spine, pelvis, and ribs. While metastasis to the phalangeal bones is particularly rare. Here we reported a case of right ring figure metastasis in a 53-year-old woman with PTC. The right ring figure lesion was confirmed to be metastatic PTC by pathologic examination and immunohistochemistry.

Thyroid Cancer (TC) is the most common endocrine cancer, of which papillary thyroid cancer (PTC), a well-differentiated type of TC, accounts for 80-90%. Long non-coding RNAs (lncRNAs), which comprise non-protein-coding segments of the genome, have been found to play a crucial role in various biological processes, including cancer development. The activity of lncRNAs is modified through epigenetic modifications, with N6-Methyladenosine (m6A) modifications implicated in the progression of several malignancies. The activity of m6A is further regulated by modifying enzymes classified as "readers",

INTRODUCTION AND OBJECTIVES: Locoregional recurrence occurs in up to 28% of patients with papillary thyroid cancer (PTC). Several risk factors contribute to this metastatic process. The latest of them is perinodal infiltration (PNI). PNI refers to the pathological extension of tumor cells through the lymph node (LN) capsule into the perinodal fibroadipose tissue. It has been accepted as a poor predictive variable. This study aimed to evaluate the prognostic significance of PNI in recurrence of PTC and identify predictive parameters associated with the recurrence in PNI (+) patients.

Papillary thyroid cancer (PTC) is the most frequent malignant thyroid tumor in Germany. The diagnosis can only be confirmed by histological examination of the suspicious tissue. The clinical signs, sonographic findings, and the results of fine-needle aspiration, possibly supplemented by subsequent molecular genetic analysis, can confirm the suspected diagnosis before surgery. The prognosis is very good if the diagnosis is made early and, depending on the size of the tumor, complete surgical removal is achieved, with a 10-year survival rate of more than 90%; however, histologically PTC has seve
