Outcome of type-1 tympanoplasty.

OBJECTIVE: The outcome of 97 patients who underwent tympanoplasty type-1 surgery by different ear, nose and throat surgeons was evaluated. METHODS: This retrospective study was carried out from the records pertaining to all patients who had tympanoplasty type-1 at King Fahd Specialist Hospital, Buraidah, Al-Qassim, Kingdom of Saudi Arabia, during the years April 1995 through to April 2000. Specific criterias were setup for selecting all the patients involved and excluding others. Thus 97 ears were selected in this study. The factors presumed to influence the outcome included age, sex, nationa

Meta-analysis of pediatric tympanoplasty.

OBJECTIVE: To determine which preoperative conditions or surgical techniques may influence the success of tympanoplasty in the pediatric population. DATA SOURCES: A MEDLINE search of the English-language literature from 1966 to May 1997 was conducted using the search terms pediatric or child and tympanoplasty or myringoplasty. STUDY SELECTION: Articles that provided age-specific data on tympanoplasty or myringoplasty were included. Of the original 651 studies retrieved, 30 were accepted for inclusion. The principal reason for exclusion was inability to separate adult and pediatric results in

Inlay tympanoplasty: cartilage butterfly technique.

For closure of a nonmarginal tympanic membrane perforation, currently popular techniques utilize either an underlay or an onlay approach. However, both procedures require incising canal skin. A transcanal inlay procedure could provide theoretical advantages of ease, speed, and comfort. Specifically designed cartilage that could facilitate the transcanal approach similar to placement of a solid tube was employed and evaluated. A transcanal cartilage butterfly inlay technique was found to be efficient and effective to close a subgroup of small-to-medium-sized tympanic membrane perforations inclu

Hearing results with cartilage tympanoplasty.

Cartilage has shown promise as a graft material to close perforations in the tympanic membrane (TM), particularly in cases of advanced middle ear pathology. Although it is similar to fascia, its more rigid quality tends to resist resorption and retraction. However, it is this rigid quality that has led many to anticipate a significant conductive hearing loss when using cartilage to reconstruct the TM. Because little has been reported in the literature comparing hearing results using cartilage with results using other grafting materials, this retrospective study was conducted to compare the hea

[Tympanoplasty on only hearing ears].

Tympanoplasty on only hearing ears was performed on 11 patients in Osaka University Hospital and Osaka Rosai Hospital during 1986 1994; this was 0.68% of all cases of middle ear surgery. The patients consisted of 5 with cholesteatoma, 5 with chronic otitis media and 1 with cholesterin granuloma. The mean age was 47.6 years (13-68 years) and the mean follow-up period was 2 years and 7 months (6 months-4 years). Postoperative hearing results was obtained in 9 patients (81.8%) according to the criteria of Otology Japan (1987), and all ears were dry at the final examination. Compared with the preo

Type I tympanoplasty in children.

To identify factors that could influence the success of tympanoplasties in children, a retrospective study was done of 51 tympanoplasties performed on children aged 9-14 years during the years 1984-1989. Surgical success was defined as an intact membrane found after 3 years' follow-up. The success rate of tympanoplasties was 92%. A functioning eustachian tube was found to be important for graft take. No difference was found in the rate of graft take whether fascia of the temporal muscle or tragal perichondrium was used. An audiologic improvement was seen in 70.6% of the patients. We conclude t

Type 1 tympanoplasty in children.

OBJECTIVE: To identify factors affecting the surgical success rate and reperforation rate in type 1 tympanoplasty. Controversy continues regarding the advisability of this procedure in young children, largely because of the likelihood of recurrent middle ear disease and eustachian tube dysfunction. DESIGN: Retrospective medical record review of a case series. SETTING: Pediatric hospital that serves both as a primary care and referral center. PATIENTS: All private patients younger than 18 years, undergoing type 1 tympanoplasty from 1985 through 1989, for whom at least 6 months' follow-up was

Tympanoplasty: Factors influencing surgical outcome.

We reviewed the records of 183 patients who had undergone myringoplasty during a six year period at the Asir Central Hospital (ACH). This study was limited to cases of repair of uncomplicated perforation of tympanic membrane that did not require ossiculoplasty or mastoidectomy. One hundred and thirty-four (73.2%) were successful myringoplasty and 78 (75.7%) patients had their hearing improved postoperatively. Various factors liable to influence the success rate such as status of middle ear, size and site of perforations, surgical approach and presence of tympanosclerosis were analyzed. Only st

Eustachian tube function in tympanoplasty.

Prognostic value of preoperative Eustachian tube function was examined in 77 ears subjected to type 1 tympanoplasty. Eustachian tube function was evaluated by positive and negative pressure equalization tests, and clearance test. Based on the results of these three tests, the ears were classified into four grades. The outcome of the ear surgery was judged by postoperative hearing, presence of otorrhea, atelectasis and perforation of the eardrum. Results of the positive pressure equalization and clearance tests of the tube were correlated with the outcome of ear surgery, although the negative p

Cochlear hearing loss in tympanoplasty.

In tympanoplasty, trauma to the cochlea is usually caused by the transmission of vibration induced during the removal of cholesteatoma, granulation tissue, and tympanosclerosis surrounding the ossicular chain. In 100 sequential cases 20 showed evidence of cochlear trauma. Hearing was recovered in 10 of these cases during the immediate postoperative period. In nine the hearing was partially recovered but remained below the preoperative bone-conduction level, and in one there was a total cochlear hearing loss. When the stapes footplate was fractured or dislocated and a perilymphatic fistula deve

[Tympanoplasty of today (author's transl)].

The author explains why he developed the access to the meso-hypotympanum via the angle between the facial nerve and the chorda tympani in 1953 which he then demonstrated at many occasions. The value of this access--later used by other ear surgeons and named mastoidectomy with intact canal wall technique or posterior tympanotomy--for chronic otitis media was always denied by the author. These methods do not give sufficient way to the main focus of pathology in the epitympanum mainly to the pretympanic recess in front of the head of the malleus; The danger of deep penetrating cholesteatoma in th

[Tympanoplasty for patients with destruction of the stapes].

Mode of tympanoplasty allowing reconstruction of the drum and erosioned malleus handle is described. For this purpose an autoossicle fragment of the lengthened shape was inserted into the blind channel created in fascia transplant which "dressed" on the handle's remnant for the osseous fragment and remnants of the handle to appear in one fascia channel. This method was applied in 11 patients with subtotal drum perforation and erosion of the hammer's handle. In 9 of them 0.5-2 years later the defect of the drum fully disappeared. In 7 patients the handle resumed normal length while the drum bec

Tympanoplasty in children--a prospective study.

Considerable controversy surrounds the subject of tympanoplasty in children. This prospective study looked at the results of type-I tympanoplasty in children. Forty-five children in the age group of five to 14 years were selected for the study. All these cases had a central perforation without any evidence of cholesteatoma. The ear to be operated had to be dry for at least six weeks before surgery. Type I tympanoplasty was performed on these patients with autograft temporalis fascia by either the underlay or overlay technique. The overall success rates in 45 operations evaluated one year post-

[The results of tympanoplasty with titanium prostheses].

In order to improve the hearing results of tympanoplasty, we performed the operation on 113 cases (124 ears) by using ossicular replacement prostheses made of Titanium. The results demonstrated that among all the operated ears, 73/124 (58.9%) had obtained a narrow gap (< 20 dB HL) between BC and AC, 89/124 (71.8%) had regained socially acceptable hearing ability. The total effective rate of hearing gain (> 10 dB HL) was 115/124 (92.7%). No reject reaction and recurrent cholesteatoma were found. This operation method, based on animal experiments and applied anatomic study of human suprastapedia
