OBJECTIVE: Although double cervix is often considered to indicate uterus didelphys, it may be challenging to determine the true associated anomaly as double cervix occurs with other müllerian anomalies. Our purpose is to report the frequency of uterine müllerian anomalies that occur with a double cervix and identify imaging or clinical criteria to help classify the associated anomaly.
STUDY DESIGN: After institutional review board approval, an electronic search was performed to identify patients with double cervix and pelvic magnetic resonance imaging (MRI) between 1976-2010 (using Internatio

INTRODUCTION: A didelphys uterus is a rare Müllerian duct anomaly (MDA) caused by the incomplete fusion of the Müllerian ducts, leading to the formation of two distinct uterine cavities. The occurrence of simultaneous twin pregnancies, with each fetus developing in a separate horn of a didelphys uterus, is estimated at approximately 1 in 1,000,000 cases. This case report describes a rare instance of a spontaneous dichorionic diamniotic twin pregnancy, with one fetus in each horn of a didelphys uterus.
CASE DESCRIPTION: This is the case of a 35-year-old woman with unilateral renal agenesis who

Herlyn-Werner-Wunderlich (HWW) syndrome is a rare congenital condition characterized by renal agenesis, uterine didelphys, and obstructed hemivagina. This report presents the case of a 19-year-old woman who reported lower abdominal pain and offensive vaginal discharge. Imaging revealed a didelphys uterus, two vaginas, two cervixes, hematocolpos, and an absent right kidney. Surgical intervention involved draining the hematocolpos and excising the uterine septum. After surgery, the patient successfully conceived and had a full-term pregnancy, delivering via cesarean section without complications

A longitudinal vaginal septum is a congenital anomaly resulting from incomplete Müllerian duct development and is often associated with uterine didelphys, which can lead to reproductive symptoms and challenges. Surgical correction may be necessary for symptomatic individuals, those facing reproductive barriers, or women planning for vaginal birth. We present the case of an 18-year-old woman with uterine didelphys and a complete symmetric longitudinal vaginal septum who elected surgical resection of the septum. The procedure was performed using a LigaSure device and was uncomplicated. This case

A 42-year-old woman with invasive lobular carcinoma of the left breast underwent staging with 68 Ga-FAPI PET/CT, which demonstrated intense uptake in the primary breast lesion and ipsilateral nodal metastases. Incidentally, symmetric intense pelvic tracer uptake was identified, corresponding on fused PET/CT to 2 distinct uterine horns consistent with uterus didelphys. The characteristic bilateral rounded appearance on PET images produced a striking "Mickey Mouse" sign, reflecting physiological FAPI accumulation within duplicated uterine stromal tissue. Subsequent ultrasonography confirmed uter

A unique case of triplet pregnancy in a woman with uterus didelphys is described, with fetuses A and B in the right uterine horn and fetus C in the left horn. Missed abortion of fetus A was diagnosed in the 22nd week of gestation by real-time ultrasonography, but only in the 27th week of gestation did the right uterine horn contract, resulting in expulsion of fetus A. Fetus B was delivered by cesarean section 2 hours later. Fetus C remained in the left uterine horn until the 37th week of gestation, when a viable 2490-g female infant was delivered by cesarean section. Triplet pregnancy occurs w

PURPOSE: The purpose of this work was to determine the association of renal agenesis with the different types of mullerian duct anomalies (MDAs).
METHOD: A 5 year retrospective review of MR records identified 57 patients with MDAs. Associated renal anomalies were correlated with the various types of MDAs.
RESULTS: Renal agenesis was found in 17 (29.8%) of 57 patients. No other renal anomalies were identified. Renal agenesis was more frequent in patients with uterus didelphys (13/16 cases). Renal agenesis was also seen in patients with uterine agenesis (2/5 cases) and unicornuate uterus (2/7

INTRODUCTION: Uterocolon fistula is one of the complications of intrauterine device (IUD) insertion. Not only may IUD materials cause perforation, but some other risk factors may contribute to its development including uterine abnormalities, thus IUD is contraindicated in patients with anatomical anomaly.
CASE: P3A1 woman, 50 years old with a history of IUD use for 16 years presented with complaints of fecal discharge from the vagina 8 months ago which worsened after IUD extraction. Physical examination revealed no abdominal tenderness. Speculum examination found feces in the cervical canal.

Obstructed hemivagina and ipsilateral renal anomaly (OHVIRA) is a rare congenital malformation of the female urogenital tract characterized by a triad of uterine didelphys, obstructed hemivagina, and ipsilateral renal anomaly. It was formerly known as Herlyn Werner Wunderlich Syndrome (HWWS). The syndrome usually presents with cyclic pelvic pain following menarche. Endometriosis is a prevalent complication. Magnetic resonance imaging (MRI) helps in diagnosing OHVIRA syndrome and endometriosis due to its high contrast resolution and objectivity. We reported a 13- year-old girl who was evaluated

Uterine malformations and spontaneous triplet pregnancy are rare situations separately, and both are related to worse perinatal outcomes. In the 3 cases of combined triplet pregnancy and uterus didelphys previously published, at least 2 of the 3 fetuses died. Morbidity rates are also high. We report the case of a 24-year-old Caucasian woman with a uterus didelphys and two previous caesarean sections who carried a spontaneous triplet pregnancy. To our knowledge, this is the first case of a spontaneous triplet pregnancy in a uterus didelphys ever reported and the only one with uneventful perinat

INTRODUCTION: Didelphys uterus, two separate uterine horns or a double uterus with two separate cervices, is one of several congenital uterine anomalies (CUA), accounting for 5% of CUA. CUA could be associated with dysmenorrhea, pelvic pain, and decreased fertility. During pregnancy, it has been associated with higher risk of miscarriage, preterm birth, malpresentation, and fetal growth restriction. There still is insufficient evidence on the efficacy and safety of surgical interventions in CUA, including hemihysterectomy, in what improvement of reproductive performance is concerned.

OBJECTIVE: Although uterine didelphys per se is not associated with an impaired ability to conceive, the association between uterine anomalies and gestational trophoblastic neoplasia (GTN) remains unclear. The management of chemotherapy-resistant GTN in women with uterine didelphys raises a new issue regarding whether to perform a hemi-hysterectomy.
CASE REPORT: A 23-year-old, gravida 1, para 0 Japanese woman was referred with a failed intermittent cervical dilatation for hematometra. Four years previously, she developed a GTN Stage III, score 5. As two cycles of chemotherapy with methotrexat

Uterus didelphys is a rare congenital uterine anomaly resulting from the complete failure of fusion of the Müllerian ducts. Early pregnancy in a duplicated uterus can present a significant diagnostic challenge, as the gravid lateralized hemi uterus may be mistaken for an adnexal mass. A 31-year-old woman presented at six weeks of gestation with mild abdominal cramping. A two-dimensional transvaginal ultrasound revealed an empty main uterine cavity and a gestational sac adjacent to the right ovary. Given a serum beta-hCG level of 4,200 mIU/mL and concern for an ectopic pregnancy, urgent diagnos

Uterine malformations were detected in 8 of 300 patients (3%) referred for transvaginal ultrasound scan (TVS) for different indications. Six of them had a partially septate uterus and two had a uterus didelphys. As uterine malformations can be associated with both sterility and reproductive failure, we suggest that the study of uterine morphology and structure could be a part of routine TVS examination.

BACKGROUND: Twin gestation in a uterus didelphys is a rare condition, and the preferred labor course for these patients is unclear.
CASE: A woman with a uterus didelphys presented at 14 weeks' gestational age. Serial ultrasound examinations demonstrated normally growing, diamniotic, dichorionic twins situated one in each uterine horn. The prenatal course was marked by intrauterine growth retardation, elevated liver functions and significant proteinuria. At 31 weeks' gestation, labor was induced; however, due to chorioamnionitis and arrest of cervical dilatation, cesarean section was performed

RATIONALE: The incidence of uterine malformations is low (4%-7%). Currently, the National Comprehensive Cancer Network clinical practice guidelines in oncology recommend minimally invasive surgery for early endometrial cancer. Minimally invasive surgery for the treatment of uterine didelphys with endometrial cancer is rare due to the large size of the uterus. To date, only 2 such patients have been reported to have undergone laparoscopy. Whether such patients can be treated with minimally invasive surgery needs to be further explored.
PATIENT CONCERNS: A 40-year-old woman with uterine didelph

Uterine anomalies are one of the various processes in the pelvic organs which present themselves as adnexal masses in pregnancy. In pregnancy associated with congenital uterine anomalies, complications are likely to occur. Ultrasonography is of great importance in early recognition of the anomalies, which can prevent unnecessary surgical intervention. A case of ultrasonic detection of uterus didelphys with unicavitary pregnancy is described.
