In vivo examination of pathogenicity and virulence in environmentally isolated Vibrio vulnificus.

Human exposure to Vibrio vulnificus, a gram-negative, halophilic environmental pathogen, is increasing. Despite this, the mechanisms of its pathogenicity and virulence remain largely unknown. Each year, hundreds of infections related to V. vulnificus occur, leading to hospitalization in 92% of cases and a mortality rate of 35%. The infection is severe, typically contracted through the consumption of contaminated food or exposure of an open wound to contaminated water. This can result in necrotizing fasciitis and the need for amputation of the infected tissue. Although several genes (rtxA1, vvp…

[A Case of Vibrio vulnificus Infection Improved with Conservative Treatment in Kagoshima].

We herein report on a case of Vibrio vulnificus infection that was improved by conservative treatment in Kagoshima, Japan. A 75-year-old Japanese woman with liver cirrhosis presented to our hospital with shaking chill and right lower leg pain. Her blood culture was positive for V. vulnificus, and bullae had newly appeared on the right leg. Further history taking revealed that she had eaten some raw seafood before admission. She recovered following administration of antibiotics and small incisions in the lesion. West Japan (especially, the northern parts of Kyushu island) is well known as an en…

Iron and Fur in the life cycle of the zoonotic pathogen Vibrio vulnificus.

In this study, we aimed to analyze the global response to iron in the broad-range host pathogen Vibrio vulnificus under the hypothesis that iron is one of the main signals triggering survival mechanisms both inside and outside its hosts. To this end, we selected a strain from the main zoonotic clonal-complex, obtained a mutant in the ferric-uptake-regulator (Fur), and analyzed their transcriptomic profiles in both iron-excess and iron-poor conditions by using a strain-specific microarray platform. Among the genes differentially expressed, we identified around 250 as putatively involved in viru…

In vitro efficacy of the combination of ciprofloxacin and cefotaxime against Vibrio vulnificus.

We performed time-kill studies of antimicrobial combinations that included minocycline, cefotaxime, and ciprofloxacin with Vibrio vulnificus ATCC 27562. Cefotaxime-plus-ciprofloxacin combinations acted synergistically against V. vulnificus in vitro, and this combination regimen can be a good choice as the empirical treatment for suspected necrotizing fasciitis due to V. vulnificus.

Vibrio vulnificus necrotizing fasciitis of the calf presenting with compartment syndrome.

We describe a 17-year-old boy with congenital spherocytosis and iron overload who presented with compartment syndrome of the calf as the initial manifestation of Vibrio vulnificus infection after minor trauma in a contaminated fish pond. The disease was complicated by necrotizing fasciitis requiring above the knee amputation. Childhood diseases associated with iron overload pose an increased risk for complicated V. vulnificus infections.

Vibrio vulnificus infection: an important cause of septicemia in patients with cirrhosis.

Vibrio vulnificus, a virulent gram-negative organism, is a normal inhabitant of coastal waters, including the Gulf of Mexico. Vibrio vulnificus infection has been recognized as a cause of fatal septicemia in chronically ill patients, particularly those with chronic liver disease. We report the case of a patient with chronic liver disease who had V vulnificus septicemia 2 days after eating raw oysters harvested in the Gulf Coast. Vibrio vulnificus septicemia should be suspected in all patients with underlying medical illnesses, particularly cirrhosis, who present with a febrile illness days aft…

Necrotizing fasciitis of the foot caused by an unusual organism, Vibrio vulnificus.

In modern medicine, there are very few infectious disease processes occurring in the foot that can cause death within 48 hr. and have an overall mortality rate of 50% despite appropriate antibiotic and surgical treatment. Such a condition must be regarded as being potentially deadly. The authors report a case of necrotizing fasciitis of the left foot resulting from an unusual organism of Vibrio vulnificus.

Necrotizing soft-tissue infections and primary sepsis caused by Vibrio vulnificus and Vibrio cholerae non-O1.

BACKGROUND: Vibrio species are a rare cause of necrotizing soft-tissue infections and primary septicemia, which are likely to occur in patients with hepatic disease, diabetes, adrenal insufficiency, and immunocompromised conditions. These organisms thrive in warm seawater and are often present in raw oysters, shellfish, and other seafood. This study examined fulminating clinical characteristics of Vibrio vulnificus and Vibrio cholerae non-O1 soft-tissue infections and identified outcome predictors. MATERIALS: Thirty patients with necrotizing fasciitis and sepsis caused by Vibrio species were …

Predicting the Global Distribution of Vibrio vulnificus Under Changing Climate Scenarios.

Vibrio vulnificus is a Gram-negative bacterium and an emerging pathogen globally distributed in brackish and marine waters. It can be transmitted to humans through contaminated food, which causes primary septicemia, or through wound exposure, causing necrotizing fasciitis. Increased abundance and range expansion have recently been associated with climate change. Further understanding of the spatial epidemiology of V. vulnificus is necessary, and species distribution modeling is a technique to correlate all the factors related to the spread of V. vulnificus. Through meta-analysis aided by Rayya…

Novel Vibrio vulnificus sequence type 540 infection in a hepatitis patient.

Vibrio vulnificus infection is associated with high morbidity and mortality in high-risk patients. Poor prognoses could lead to >50% mortality rate. The present report describes a case of V. vulnificus bacteremia in a cirrhotic patient with underlying hepatitis C. He presented with generalised abdominal pain associated with distention and could not ambulate for one week. He also complained of fever for six days and pruritus for 10 days. Tea-coloured urine was noted in continuous bag drainage. The abdomen was distended but soft, with mild tenderness palpated over the left lumbar and iliac regio…

Vibrio vulnificus RtxA1 modulated calcium flux contributes reduced internalization in phagocytes.

AIMS: Vibrio vulnificusis an opportunistic pathogen that causes primary septicemia and wound infection with high mortality rate. This pathogen produces an RTX toxin (RtxA1) which can cause host cell rounding, cell death and interference with internalization by host phagocytes. However, the mechanism of RtxA1-induced phagocyte paralysis is not clear. MAIN METHODS: Using the murine macrophage cell line RAW264.7, we measured cytotoxicity and phagocytosis of V. vulnificusin normal and calcium-depleted media. To deplete extracellular and cytosolic Ca(2+), cells were exposed to the calcium chelator…

Structure of vulnibactin, a new polyamine-containing siderophore from Vibrio vulnificus.

A new siderophore named vulnibactin has been isolated from low iron cultures of Vibrio vulnificus, a human pathogen. The structure was established as N-[3-(2,3-dihydroxybenzamido)propyl]-1,3-bis[2-(2-hydroxy-phenyl)- trans-5 - methyl-2-oxazoline-4-carboxamido]propane by a combination of acid hydrolysis, nuclear magnetic resonance spectroscopy and positive fast atom bombardment mass spectrometry. Vulnibactin is characterized as containing one residue of 2,3-dihydroxybenzoic acid as well as two residues of salicylic acid, both of which are involved in the formation of oxazoline rings with L-thre…

Comparative persistence of opportunistic Vibrio alginolyticus and major foodborne Vibrio species (Vibrio cholerae, Vibrio parahaemolyticus, and Vibrio vulnificus) in ready-to-eat seafood under different storage conditions.

Vibrio alginolyticus is an opportunistic marine bacterium capable of infecting both humans and aquatic animals, in addition to major foodborne pathogenic Vibrio species (Vibrio cholerae, Vibrio parahaemolyticus, and Vibrio vulnificus). Although these species are increasingly detected in seafood, differences in their persistence under conditions simulating real food systems remain poorly understood. This study investigated the growth, survival, and recovery of V. alginolyticus and compared its behavior with those of the three pathogenic species in ready-to-eat seafood (flatfish, smoked salmon, …

Septic Shock Induced by Vibrio Vulnificus in Northern Poland, a Case Report.

INTRODUCTION: Vibrio vulnificus infections are a growing problem worldwide. In recent years, infections with this bacteria have been reported in Central Europe, especially in the German Baltic coast but also in France and Italy. Climate warming causes the sea temperature to increase every year, which translates to an increased risk of infections from the Vibrio group. Most of these are mild and present as wound infections, but some patients develop life-threatening sepsis from either ingestion of infected mollusks or wound lesions that develop into generalized infections. Illness may be associ…

[Amputation of an arm due to infection with Vibrio vulnificus after beach holiday].

Infection with Vibrio vulnificus is a rare condition with approximately 11 cases reported in the Danish literature. It is characterized by soft tissue infection/wound, necrotizing fasciitis and septicaemia. In this case report we present a patient admitted with a rapid progression and sepsis consistent with V. vulnificus infection but with no informa-tion of water exposure. The initial treatment was surgery and sepsis management including broad-spectrum antibiotics. On day eight the patient's right arm was amputated. On day 16 the patient was discharged from the intensive care unit, and on day…

Identification of 62-kDa protein as an immunogenic antigen of Vibrio vulnificus for humans.

Vibrio vulnificus infection can cause necrotizing fasciitis and sepsis and can develop within a few days despite intensive care. The mortality rate is up to 60% in vulnerable people. Most patients infected with this microbe have chronic liver disease, especially liver cirrhosis or cancer, as an underlying disease. V. vulnificus infection is opportunistic, and there is an urgent need to develop an anti- V. vulnificus vaccine. Thus, it is important to identify immunogenic antigens. We collected human sera from three subject groups: patients with V. vulnificus infection, patients with chronic liv…
