External genital abnormalities associated with Wilms tumor.

The records of 170 patients with unilateral and 18 patients with bilateral Wilms tumor and 6 patients with congenital mesoblastic nephroma were reviewed for abnormalities of the external genitalia. There were 4 patients with cryptorchism, 1 with hypospadias, 1 with mixed gonadal dysgenesis, and 3 with male pseudohermaphroditism. Of the group, these 9 patients had earlier symptoms; 6 of them were under two years old. Five patients (27.7%) with bilateral Wilms tumors had external genital anomalies in contrast to only 4 (2.3%) of those with unilateral tumors. One of the 9 children with genital an

Wilms' tumor, nephron disorder and ambiguous genitalia.

Two male cases with Wilms' tumor in association with ambiguous genitalia and nephrotic syndrome are described. Seventeen similar cases reported in the literature are also analyzed. In these patients, nephrotic syndrome was noticed at younger ages, and progressed more rapidly than usual. Renal damage was so serious that at least 11 out of 9 patients died of renal failure, though not of Wilms' tumor itself. It should be stressed that the clinical course and associated anomalies were quite similar among these patients. We suggest that Wilms' tumor, ambiguous genitalia and this type of nephrotic s

Long-term survival determinants in Wilms' tumor.

This is a retrospective report of 54 patients with the diagnosis of Wilms' tumor treated at Roswell Park Memorial Institute from 1946 to 1977. The age ranged from 1 month to 57 years with a mean of 7.2 years and a median age of 5 years. An analysis of the survival results showed that patients continued to die of disease until 66 months post-diagnosis, making the 5-year survival a much better evaluation criterion for this group than the traditional 2-year survival. Males fared significantly better in this series (p less than 0.01). This was not due to detectable differences in age group, or his

Association between Wilms tumor and gonadal dysgenesis.

The classification of intersexuality was determined in 10 patients with an intersex disorder who subsequently suffered a Wilms tumor. Of these 10 patients 7 (70 per cent) had a disorder of gonadal dysgenesis: 5 had mixed gonadal dysgenesis, 1 had dysgenetic male pseudohermaphroditism and 1 had 46XY pure gonadal dysgenesis. Because the gonads of patients with either of these 3 disorders have a high predilection for the development of testicular neoplasia, the concomitant occurrence of renal tumors in such a patient population suggests that a common defect in the urogenital ridge, the embryonic

Intraoperative tumor lysis syndrome in a child with Wilms' tumor.

Tumor lysis syndrome in an onco-metabolic emergency resulting from massive lysis of rapidly proliferating malignant cells seen commonly in patients with hematological malignancies such as acute lymphocytic leukemia and Burkitt's lymphoma and is quite rare in solid tumors. Spontaneous development of tumor lysis has been described among other trigger factors such as corticosteroid therapy, anesthesia, tumor manipulation during surgery and pyrexia. We describe such a case in a 5-year-old boy posted for excision and staging of a massive Wilms' tumor who developed a hyperkalemic cardiac arrest duri

Synchronous bilateral Wilms' tumor with liver metastasis.

BACKGROUND: Wilms' tumor (nephroblastoma) is mostly unilateral; however, bilateral Wilms' tumors are seen in about 5-8% of patients. This can be synchronous or metachronous. It is uncommon to get liver metastasis from bilateral Wilms' tumor. CASE PRESENTATION: An 8-year-old male Ugandan presented with a history of abdominal swelling and flank pains for 1 year. There was no history of hematuria. Both ultrasound and computed tomography of the abdomen demonstrated multiple solid lesions in both kidneys and a huge solid mass in segments V, VI, VII and VIII of the liver. Histological examination o

Brain Metastasis of Wilms Tumor in Adult.

BACKGROUND: Wilms tumor is a rare renal tumor in adults. To the best of our knowledge, only a small number of cases of brain metastasis have been reported in the literature. We report the case of a 29-year-old female with headache and dizziness, with a parietal mass and pathologic diagnosis of Wilms tumor metastasis. CASE DESCRIPTION: The patient was admitted with a 3-month history of lumbar pain and 2 months of progressive headache associated with dizziness. Abdomen magnetic resonance imaging showed a renal mass. Post nephrectomy, the neurologic signs worsened and a head magnetic resonance i

Pharmacotherapeutic Management of Wilms Tumor: An Update.

Although differences exist in treatment and risk-stratification strategies for children with Wilms tumor (WT) between the European [International Society of Paediatric Oncology (SIOP)] and American [Children's Oncology Group (COG)] study groups, outcomes are very similar, with an overall survival of > 85%. Future strategies aim to de-intensify treatment and reduce toxicity for children with a low risk of relapse and intensify treatment for children with high-risk disease. For metastatic WT, response of lung nodules to chemotherapy is used as a marker to modify treatment intensity. For recurren

Laparoscopic nephroureterectomy for Wilms' tumor: oncologic considerations.

Wilms' tumor is the most common malignant renal tumor of childhood. Surgical resection is an important aspect of therapy and is traditionally performed through an open transabdominal approach. The advent of advanced laparoscopic techniques for benign renal lesions has led to interest in applying a minimally invasive approach to Wilms' tumors in children. We describe a 2-year-old girl who presented with a right renal mass measuring 18 × 13 cm with peritoneal seeding and pulmonary metastatic disease. Wilms' tumor was confirmed on open biopsy. After neoadjuvant chemotherapy, a laparoscopic resect

[Botryoid Wilms tumor extending into the duodenum].

We report on a rare case of botryoid Wilms tumor extending into the duodenum. This uncommon macroscopic form of nephroblastoma consists entirely of a polypoid renal intrapelvic mass. The main differential diagnosis of this unusual tumor is botryoid rhabdomyosarcoma. A 14-month-old boy presented with a painful abdominal mass. Radiology revealed a large heterogeneous mass in the renal calyx, protruding into the ureter. A right radical nephroureterectomy was carried out. The tumor was found to extend into the 2nd portion of the duodenum. The pathologic diagnosis was mixed type nephroblastoma, SIO

Association of wilms tumor with spinal dysraphism.

Previous data suggested an association of vertebral anomalies with Wilms tumor. At the same time, vertebral midline fusion defects are often indicated by dermal anomalies over the spine. In the present study the prevalence of both occult spina bifida and cutaneous signs of spinal dysraphism was significantly higher in 50 Wilms patients than in 180 control children (18.0 versus 4.4%, p <.01, and 35.9 versus 17.5%, p <.02, respectively). Family investigations are needed to answer the question whether signs of spinal dysraphism in parents and sibs of patients may be regarded as indicators of an i

Constitutional WT1 mutations in Wilms' tumor patients.

PURPOSE: Patients with Wilms' tumors (WT) who carry constitutional mutations in the WT1 gene have been described in case reports and small case series. We sought to determine the frequency of constitutional WT1 mutations in a larger cohort, and to identify clinical manifestations associated with the risk for carrying a WT1 mutation. METHODS: We collected clinical data and blood samples from 201 patients with a history of WT. Southern blot analysis, single-strand conformation polymorphism (SSCP) analysis, and direct DNA sequencing were performed on DNA isolated from peripheral-blood lymphocyte

Congestive heart failure caused by Wilms' tumor.

A 2-year-old child with unilateral Wilms' tumor presented with congestive heart failure (CHF). The heart failure was secondary to severe hypertension from hyperreninemia. After surgical removal of the tumor the CHF disappeared. This case is the first one reported of a child presenting with CHF due to unilateral Wilms' tumor.

Hepatic veno-occlusive disease in Wilms' tumor.

Invasive surgical procedures will be avoided by recognition of veno-occlusive disease (VOD) as a clinical syndrome which sometimes leads to serious complications in children receiving Actinomycin D for Wilms' tumor. In order to document the unusually frequent occurrence of VOD far beyond the observations of others, a prospective study was undertaken in 13 consecutive Actinomycin D-treated children. There were 9 children experiencing 27 events of mostly mild VOD. Six of them were below 3 years of age having in 5 cases a unilateral kidney tumor on the right side. The criteria used for VOD were p

Local control in synchronous bilateral Wilms tumor.

PURPOSE: To evaluate the role of radiation therapy (RT), chemotherapy (CT), and surgery in the local control of synchronous bilateral Wilms Tumor (WT). METHODS AND MATERIALS: Between 1962 to 1993, 45 children were treated for bilateral WT; 38 patients with synchronous tumors were reviewed. Initial surgery depended on the era of treatment and included unilateral nephrectomy (N)/partial nephrectomy (PN) and contralateral PN in 6, unilateral N/PN alone in 7, and biopsy only in 25. Chemotherapy (CT) consisted of vincristine, actinomycin-D, and adriamycin in 32 and vincristine/ actinomycin-D in 6.

Familial primary hyperparathyroidism complicated with Wilms' tumor.

We describe a familial occurrence of primary hyperparathyroidism. The proband is a 60-year-old woman who had a parathyroid adenoma. Her older sister had a parathyroid adenoma with cementifying jaw fibroma and her younger sister died of parathyroid adenocarcinoma with pulmonary metastasis at the age of 38. We have not yet obtained evidence for other endocrine disorders suggesting multiple endocrine neoplasia (MEN) in this pedigree. The proband is complicated with Wilms' tumor. It is now widely accepted that respective predisposed genes of MEN type 1 and Wilms' tumor, and PTH gene are located on

Cancer in twins of Wilms tumor patients.

We interviewed families of 71 patients registered in the National Wilms Tumor Study and identified as having a twin sibling. Questions concerning zygosity and the occurrence of congenital anomalies and other forms of cancer in the twins were asked. Of the 71 twin pairs, 35 were dizygotic, 31 were monozygotic, and 5 were of unknown zygosity. The only pair concordant for Wilms tumor was dizygotic, leading to a heritability estimate of zero. In a monozygotic pair, one twin was diagnosed with Wilms tumor and the other with medulloblastoma. The estimated relative risk of Wilms tumor and childhood c
