The first report of pseudoaneurysm secondary to Cunninghamella bertholletiae infection in a Haematopoietic stem cell transplantation recipient: a case report and literature review.

BACKGROUND: Mucormycosis is a life-threatening complication that occurs in haematopoietic stem cell transplantation (HSCT) recipients. Among mucormycosis patients, those infected with Cunninghamella bertholletiae have the poorest prognosis because of the high virulence and angioinvasive nature of this organism. Despite its high vascular invasiveness, pseudoaneurysm caused by C. bertholletiae in HSCT recipients has rarely been reported. CASE PRESENTATION: An 8-year-old HSCT recipient experienced recurrent fever, cough and pain after HSCT. Teicoplanin, acyclovir, posaconazole and caspofungin we

Diverse Presentations of Invasive Aspergillus and Mucorales Infections in Immunocompromised Patients: A Case Series.

Invasive fungal infections (IFIs) pose significant challenges in immunocompromised patients, particularly those with diabetes, malignancies, HIV, COVID-19, or prolonged corticosteroid use. Invasive infections due to Aspergillus and Mucorales, alone or in combination, are increasingly recognized, often with atypical presentations. These fungal infections can mimic bacterial or tubercular diseases and pose significant therapeutic challenges, as their management requires tailored antifungal regimens, particularly in cases of co-infections. We present three cases of diverse presentations of Asperg

Functional interplay between bradykinin receptors and transient receptor potential vanilloid-1 in lipopolysaccharide-induced acute lung injury in mice.

In this study, we investigated the functional interplay between bradykinin receptors and the transient receptor potential vanilloid-1 (TRPV1) channel in a mouse model of acute lung injury (ALI) induced by lipopolysaccharide (LPS). Lung and bronchoalveolar lavages were collected at 6 and 24 h after the induction of ALI and evaluated for changes in body weight, inflammatory marker levels, lung injury, and TRPV1 expression. Pretreatments with a TRPV1 antagonist (capsazepine) or B1 and B2 receptor antagonists, i.e., DALBK and HOE 140, respectively, were evaluated in this ALI mouse model. The histo

Validation of a Mouse Model of Dysfunctional Oropharyngeal Swallowing-Induced Aspiration Pneumonia.

OBJECTIVES: To validate a mouse nerve injury model of microaspiration pneumonia due to dysfunctional oropharyngeal swallowing. METHODS: C57BL/6J mice were divided into nerve transection (Nx; in which the left hypoglossal and vagus nerves were transected) and nonsurgical control groups. The lungs were removed the day after nerve transection. Then, the expression of inflammation-related genes (TNFα, CCL2, and IL-6) in the lung tissue was measured via qPCR and lung tissues were evaluated histologically via HE staining (Ctl = 6, Nx = 4). Bronchoalveolar lavage fluid (BALF) was collected at these

Evaluating the Potential of Galactosaminogalactan as a Diagnostic Target for Invasive Aspergillosis.

BACKGROUND: Early diagnosis of invasive aspergillosis (IA) is critical for the initiation of effective antifungal therapy. Currently, detection of galactomannan (GM), a secreted fungal glycan, is the most used culture-independent diagnostic test for IA. However, limitations in the sensitivity and specificity of this test have led to interest in identifying other target molecules. Galactosaminogalactan (GAG), a polysaccharide cell wall component secreted by Aspergillus hyphae, is a potential diagnostic marker for IA. OBJECTIVES: To evaluate the utility of GAG as a diagnostic target, we generat

The Usefulness of Mediastinal Cryobiopsy in the Diagnosis of Mediastinal Lymphadenopathy in HIV Patients.

INTRODUCTION: Mediastinal diseases in HIV patients show complex diagnostic challenges due to opportunistic infections and rare neoplasms. While Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is a key technique, its effectiveness is limited as it only provides cytological samples. Endobronchial ultrasound-guided mediastinal cryobiopsy (CryoEBUS) emerges as a promising alternative, yielding larger and higher-quality samples. This study, the first in Mexico and the first worldwide in HIV patients, evaluates its diagnostic efficacy compared to EBUS-TBNA, highlighting

A successful diagnostic case of Varicella zoster virus pneumonia by a Film Array assay for meningitis/encephalitis.

INTRODUCTION: Although varicella zoster virus (VZV) infection is generally considered self-limiting and shows a favorable outcome, herpes zoster (HZ) can cause several complications such as neurological pain or meningitis, which lowers the patients' quality of life. HZ rarely manifests VZV pneumonia, resulting in a poor prognosis with a high mortality rate of 10-30 % if requiring a mechanical ventilation. Here, we present a rare successfully managed case of VZV pneumonia. CASE PRESENTATION: A 90-year-old man with a medical history of angina pectoris, prostate cancer, dementia, and brain infar

Role of Sequential Approach to Sample Collection in Microbiological Diagnosis of Smear Negative Pulmonary Tuberculosis.

BACKGROUND: The current diagnostic tools for pulmonary tuberculosis (PTB) fail in detecting up to 43% of cases, leading to inappropriate use of antibiotics and delayed diagnosis. Induced sputum and bronchoalveolar lavage (BAL) are more accurate samples, but their use is not clearly recommended. This study evaluates the real-world feasibility and effectiveness of using a sequential collection of respiratory samples to diagnose PTB. MATERIALS AND METHODS: PTB cases with sputum-scarce or smear-negative status were included. Sputum induction (SI) and BAL samples were collected. We used a composit

Early mucosal responses following a randomised controlled human inhaled infection with attenuated Mycobacterium bovis BCG.

The development of an effective vaccine against Mycobacterium tuberculosis is hampered by an incomplete understanding of immunoprotective mechanisms. We utilise an aerosol human challenge model using attenuated Mycobacterium bovis BCG, in BCG-naïve UK adults. The primary endpoint of this study (NCT03912207) was to characterise the early immune responses induced by aerosol BCG infection, the secondary endpoint was to identify immune markers associated with in-vitro protection. Blinded volunteers were randomised to inhale 1 × 107 CFU aerosolised BCG or 0.9% saline (20:6); and sequentially alloca

Lung Microbiota in People Diagnosed with HIV and Pneumonia: A Colombian Cohort Study.

The lung microbiota plays a key role in respiratory health, but its composition in individuals living with HIV and diagnosed with community-acquired pneumonia (CAP) remains underexplored. A prospective cohort study in Medellín, Colombia recruited individuals with CAP and/or HIV between 2016 and 2018. Clinical and microbiological data were collected at baseline, with bronchoalveolar lavage samples obtained at baseline and induced sputum samples collected at baseline and 6-month follow-up. Microbiota composition was analyzed in these samples using Illumina MiSeq sequencing of the 16S ribosomal R

Coexistence of cat scratch disease lymphadenitis and active pulmonary tuberculosis in an immunocompetent host - a case report with metagenomic diagnosis and literature review.

Cat scratch disease (CSD), caused by Bartonella henselae (B. henselae), typically presents as localized swelling of lymph nodes following a scratch or bite from a cat. It is crucial to differentiate CSD from tuberculosis (TB), particularly in regions where TB is prevalent. This report describes a 56-year-old man who exhibited bilateral swelling of the cervical lymph nodes. Initially, he was suspected to have tuberculous lymphadenitis due to the granulomatous changes observed in a biopsy of the lymph nodes, typical signs of TB on a chest CT scan, and a positive result from an interferon-gamma r

ASIC1a regulates airway epithelial cell pyroptosis in acute lung injury by NLRP3-Caspase1-GSDMD pathway.

BACKGROUND: Acidosis is the most common complication that seriously affects the prognosis of acute respiratory distress syndrome (ARDS). Acid-sensitive ion channel 1a (ASIC1a) is activated in acidic environments to regulate inflammatory process. However, the role of ASIC1a in ARDS is unclear. METHODS: In this study, we examined the expression of ASIC1a in airway epithelial cells in an acidic environment. We then investigated whether blocking ASIC1a could inhibit pyroptosis of airway epithelial cells and the molecular mechanism. In the mouse acute lung injury (ALI) model, we observed the chang

S100A9-/- alleviates LPS-induced acute lung injury by regulating M1 macrophage polarization and inhibiting pyroptosis via the TLR4/MyD88/NFκB signaling axis.

Acute lung injury (ALI) is characterized by pulmonary diffusion abnormalities that may progress to multiple-organ failure in severe cases. There are limited effective treatments for ALI, which makes the search for new therapeutic avenues critically important. Macrophages play a pivotal role in the pathogenesis of ALI. The degree of macrophage polarization is closely related to the severity and prognosis of ALI, and S100A9 promotes M1 polarization of macrophages. The present study assessed the effects of S100A9-gene deficiency on macrophage polarization and acute lung injury. Our cohort study s

Impact of Pulmonary microbiota on lung cancer treatment-related pneumonia.

Background: The use of immunotherapy is progressively expanding for the treatment of lung cancer, either alone or in combination with radiotherapy. However, treatment-related adverse events, especially pneumonia, significantly limit the drug's effectiveness in treating lung cancer. The occurrence of lung cancer, immunotherapy, and pulmonary radiotherapy can all contribute to the imbalance in the pulmonary microbiota, rendering the lungs more susceptible to inflammatory reactions. Methods: Mouse models of lung transplantation tumor were treated with either PD-1 monoclonal antibody or radiothera

STAT3 Deficiency Alters the Macrophage Activation Pattern and Enhances Matrix Metalloproteinase 9 Expression during Staphylococcal Pneumonia.

Staphylococcus aureus is a significant cause of morbidity and mortality in pulmonary infections. Patients with autosomal-dominant hyper-IgE syndrome due to STAT3 deficiency are particularly susceptible to acquiring staphylococcal pneumonia associated with lung tissue destruction. Because macrophages are involved in both pathogen defense and inflammation, we investigated the impact of murine myeloid STAT3 deficiency on the macrophage phenotype in vitro and on pathogen clearance and inflammation during murine staphylococcal pneumonia. Murine bone marrow-derived macrophages (BMDM) from STAT3 LysM

Potential Impact of Rapid Molecular Microbiologic Diagnosis for Mechanically Ventilated Children in Intensive Care With Suspected Pneumonia.

BACKGROUND: Lower respiratory tract infections (LRTIs) remain a leading cause of community-acquired and nosocomial infection in children and a common indication for antimicrobial use and intensive care admission. Determining the causative pathogen for LRTIs is difficult and traditional culture-based methods are labor- and time-intensive. Emerging molecular diagnostic tools may identify pathogens and detect antimicrobial resistance more quickly, to enable earlier targeted antimicrobial therapy. METHODS: This is a single-center, prospective observational laboratory study evaluating the use of t

Anti-Inflammatory Effects of Japanese Herbal Medicine Hochuekkito in a Mouse Model of Acute Exacerbation of Chronic Obstructive Pulmonary Disease.

INTRODUCTION: The traditional Japanese herbal medicine hochuekkito (TJ-41) has been reported to ameliorate systemic inflammation and malnutrition in patients with chronic obstructive pulmonary disease (COPD). TJ-41 has also been known to have preventive effects against influenza virus infection. However, its role in the acute exacerbation of COPD (AECOPD) remains to be elucidated. Our previous study established a murine model of viral infection-associated AECOPD that was induced by intratracheal administration of porcine pancreatic elastase (PPE) and polyinosinic-polycytidylic acid [poly(I:C)]

Broncho-Vaxom Attenuates Lipopolysaccharide-Induced Inflammation in a Mouse Model of Acute Lung Injury.

Acute lung injury (ALI) is a condition associated with acute respiratory failure, resulting in significant morbidity and mortality. It involves cellular changes such as disruption of the alveolar-capillary membrane, excessive neutrophil migration, and release of inflammatory mediators. Broncho-Vaxom® (BV), a lyophilized product containing cell membrane components derived from eight bacteria commonly found in the respiratory tract, is known for its potential to reduce viral and bacterial lung infections. However, the specific effect of BV on ALI has not been clearly defined. This study explored
