Case Report: Staged limb salvage and delayed ankle arthrodesis for chronic distal tibial osteomyelitis after failed fixation.

Chronic osteomyelitis of the distal tibia following failed fracture fixation is a challenging condition because of limited soft-tissue coverage, compromised vascularity, persistent infection, and difficulty achieving stable reconstruction. We report the case of a 34-year-old female who developed chronic post-traumatic osteomyelitis of the distal tibia after failed initial management of a distal tibial fracture. She presented with persistent pain, swelling, inability to bear weight, and a discharging sinus. Radiographic evaluation showed persistent distal tibial fracture pathology and failure o

Chitooligosaccharide endowed tunable adhesion to self-gelling powders for rapid hemostasis and sutureless skin wound closure.

Self-gelling powders have recently emerged as promising tissue adhesives for bleeding wound care. However, to simultaneously achieve strong tissue adhesion and on-demand removal without debridement remains a significant challenge. Here, we developed an ultrafast self-gelling powder compositing of acrylic acid/2-aminoethyl methacrylate copolymers (AMA) and chitooligosaccharide (COS). Upon absorbing water/blood from wet tissue surfaces, AMA-COS powders could quickly transform into an integral hydrogel that firmly adhered to tissues (adhesion strength up to 37.74 kPa) based on strong electrostati

Surgical treatment of infectious severe calcaneal bone defects in children by the Ilizarov technique.

INTRODUCTION: The soft tissue of the heel is weak, and calcaneal bone defects occur easily post-infection, resulting in the inability of paediatric patients to walk normally. Calcaneal reconstruction is challenging. We aimed to evaluate the methodology and clinical effects of the Ilizarov technique in the treatment of calcaneal infectious bone defects. METHODS: We retrospectively analyzed the cases of 12 children with infectious calcaneal bone defects treated by the Ilizarov technique in our center from January 2018 to August 2022.Stump lengthening of the calcaneus was performed in nine cases

Role of placental extracts in enhancing periodontal flap surgery healing: insights from periostin biomarker analysis.

BACKGROUND: The elimination of the causative agent and the facilitation of tissue regeneration are the fundamental objectives of periodontal therapy. Various adjunctive agents have been investigated to optimize treatment outcomes with surgical interventions. Periostin is a matricellular protein predominantly expressed in periodontal tissues, playing a key role in tissue remodeling, inflammation, and wound healing. The human placental extract has been used in periodontal surgery and compared with open flap debridement alone, with gingival crevicular fluid (GCF) periostin levels assessed to gaug

From Maggots to Recovery: Exploring an Integrative Medicine Approach to Chronic Non-Healing Ulcer with Leech Therapy and Panchvalkal Ointment: A Case Study.

BACKGROUND: Chronic, non-healing ulcers, referred to as dushtavrana in Ayurveda present a challenge in medical care since ancient times. Ayurvedic literature, notably Acharya Susrutha Chikithsa Sthana, dedicated a chapter to the treatment of dushtavrana, encompassing sixty procedures (shashti upakramas). Chronic ulcers, influenced by diverse factors, resist a singular therapeutic approach. The inflammatory stage is critical, and disruptions in the order of stages of healing can lead to chronicity. Despite conventional treatments, many ulcers remain resistant, causing physical, financial, and e

Tibial Plafond Fracture Healing in Coral Reef Aorta - A Case Report.

INTRODUCTION: Orf disease, or Ecthyma contagiosum, is a zoonotic skin infection caused by the orf virus. The orf virus belongs to the family parapoxvirus and is commonly seen in sheep, goats, and deer. Transmission to humans is most commonly caused by direct contact with infected animals or fomites. Septic arthritis is a serious orthopaedic condition that requires prompt identification and early medical treatment. Septic arthritis is often precipitated by bacteremia, which allows for hematogenous spread of the bacterial load to the affected joint. Without prompt effective treatment, septic art

BACKGROUND: Diabetes related foot ulcer (DFU) is a leading cause of impaired quality of life, disability, hospitalisation, amputation and mortality in people with diabetes. It is therefore critical that podiatrists across all settings, including community settings, are confident and capable of providing care for diabetes-related foot complications. This study aims to describe current practice, confidence and anxiety levels of community podiatrists in the management of patients with foot ulceration. Furthermore, current barriers to service provision and interest in future educational opportunit

Systemic antibiotics are not required for successful two-stage revision hip arthroplasty.

The duration of systemic antibiotic treatment following first-stage revision surgery for periprosthetic joint infection (PJI) after total hip arthroplasty (THA) is contentious. Our philosophy is to perform an aggressive debridement, and to use a high local concentration of targeted antibiotics in cement beads and systemic prophylactic antibiotics alone. The aim of this study was to assess the success of this philosophy in the management of PJI of the hip using our two-stage protocol. The study involved a retrospective review of our prospectively collected database from which we identified all
