Our approach to the conservative treatment of the dislocated hip in developmental dysplasia.

The purpose of this study is to evaluate the effectiveness of the use of traction in achieving closed reduction of a dislocated hip. We evaluated 38 hips in 29 consecutive patients who had been treated at our department between January 1999 and December 2001. There were 26 girls and 3 boys and the average age was 8 weeks. The minimal duration of follow-ups was 12 months after closed reduction. We achieved 32 concentricaly reduced hips, the barriers to the concentric reduction were demonstrated by arthrography in 5 hips an 1 hip redislocated after removal of the spica cast. Avascular necrosis o

Occult spinal dysraphism and its association with hip dysplasia in females.

We examined the pelvic radiographs of two groups of patients (more than 12 years of age) from six medical centres. Hip dysplasia was considered to be present if Shenton's line was broken and more than one third of the femoral head was revealed to be uncovered in an antero-posterior radiograph of the pelvis. Patients with hip dysplasia due to teratological or neurological causes were excluded. There were 291 patients with treated or untreated hip dysplasia in the dysplastic group. The control group of 415 individuals was collected from consecutive outpatients (with a pre-set standardised female

Magnetic resonance imaging after reduction for congenital dislocation of the hip.

BACKGROUND: Reliable concentric reduction of the femoral head and subsequent retention in a centred position are indispensable preconditions for the remodelling of the acetabulum in developmental dysplasia of the hip (DDH) and to prevent damage to the hip joint, i.e. avascular necrosis. The objective of this study is to evaluate the necessity of verifying the reduced position of the articulation in the plaster cast. METHOD: MRI was carried out in 15 infants with 21 unstable hip joints after reduction under arthrographic control and fixation in a plaster cast in the 'human' position with the h

Complications in primary total hip arthroplasty: avoidance and management of dislocations.

Dislocation in primary total hip arthroplasty is common and problematic and is attributable to several factors, including previous hip surgery, neuromuscular disorders, cerebral dysfunction, psychosis, alcoholism, and female gender. Factors under the control of the surgeon include component orientation and restoration of soft-tissue tension. Prosthetic factors lowering the risk of dislocation include increasing the size of the prosthetic femoral head, keeping femoral neck circumference to a minimum, and optimizing the geometry of the acetabular component. Postoperatively, patients should be ex

Comparison of hydroxyapatite- and porous-coated stems in total hip replacement.

24 patients underwent bilateral hip replacement, using the SROM modular system (Depuy Johnson & Johnson, MA, USA) with the porous-coated sleeve in one hip and an HA-coated sleeve in the other. 20 patients were followed for at least 4 years with clinical and radiographic evaluations. We found no difference between the two sides as regards the time of disappearance of thigh pain and hip scores. Radiographs showed a buttress sign in 18 HA-coated and 15 porous coated stems. There were no differences in the serial bone-remodeling pattern around the proximal sleeve and measurement of wear volume bet

Two alternative methods for the measurement of MZ distance on a hip radiograph.

BACKGROUND: The aim of this study was to assess the use of two alternative measurement methods for the MZ distance, the distance between the acetabular centre (M) and the femoral head centre (Z), on an anteroposterior hip radiograph. METHODS: The MZ distance was measured in 900 normal and 34 dysplastic adult hips by using the classic method (MZ-c) and two alternative methods (MZ-m and MZ-x). MZ-m utilized the vectorial distance between points M and Z, like MZ-c, but was expressed as a positive or negative value according to the localization of the points with respect to each other. MZ-x was t

Femoral shortening and cementless arthroplasty in high congenital dislocation of the hip.

We performed cementless total hip arthroplasty with femoral shortening in 28 hips of 23 patients with high-riding congenital dislocation of the hip. All the acetabular cups were placed in their original anatomic location. If needed, the acetabulum was reconstructed using the femoral head. The proximal diaphyseal step-cut shortening osteotomy was performed and stabilized by cerclage or cable grips. The follow-up results at average 48 months postoperatively were scored clinically by the Merle D'Aubigné and Harris hip scores and were good and excellent in 89.2%. One acetabular cup showed progress

Ultrasound screening of the infant's hip: introduction of a new combined angle.

This study evaluates the most widely used sonographic measurements for judging an infant's hip morphology. The alpha and beta angles of Graf, the bony coverage percentage of Morin and a new combined angle were analysed. This latter angle records the direction of the whole acetabulum (cartilagenous and bony) as one unit. One-hundred sonograms were examined independently by two observers to calculate the interobserver and intraobserver limits of agreement. The new combined angle proved to be the most reproducible measurement. Graf's alpha angle was the second most reliable measurement followed b

Dynamic magnetic resonance guided treatment of developmental dysplasia of the hip.

This study demonstrates the feasibility and advantages of near real-time, multiplanar, dynamic magnetic resonance image-assisted treatment of patients with developmental dysplasia of the hip. Pathoanatomy and dynamic blocks to reduction are visualized with anatomic clarity not otherwise possible. Continuous imaging allows accurate assessment and maintenance of optimum positioning throughout the casting procedure. Patient charges for this new technique are less than standard methods of treatment, and the child receives no ionizing radiation.

Clinics in diagnostic imaging (69). Bilateral developmental dysplasia of the hip.

Developmental dysplasia of the hip (DDH) is a condition encompassing a spectrum of abnormality. The ultrasonography features of DDH in a male infant is illustrated. Follow-up after treatment with a Pavlik harness shows resolution of the DDH. In the neonatal period, ultrasonography (US) is the modality of choice used to evaluate babies with suspected developmental dysplasia of the hip. The unossified cartilaginous femoral head and much of the acetabulum can be visualised on US. When the femoral head begins to ossify, pelvic radiographs can be used to follow-up patients with developmental dyspla

Conditions before collapse of the contralateral hip in osteonecrosis of the femoral head.

BACKGROUND: Osteonecrosis of the femoral head is a disease which may cause rapid femoral head collapse in relatively young patients. Clinical results of prosthesis replacement in osteonecrosis patients are generally poorer than in age-matched patients with other diagnoses. The ultimate goal in treating these patients is therefore to preserve not replace the involved femoral head. The purposes of this study were to analyze outcomes by examining osteonecrotic hips before collapse and to justify the role of early surgical intervention for osteonecrosis of the femoral head. METHODS: Between 1993

Stability of total hip arthroplasty in patients 75 years or older.

Increasing patient age (> 75 years) is a known risk factor for dislocation of total hip arthroplasty. This is a study of total hip arthroplasties by one surgeon in patients 75 years or older to determine the prevalence of dislocation and a review of the surgical options for prevention and treatment of instability in this population. Of 140 primary total hip arthroplasties done in patients 75 years or older who were followed up for at least 1 year, the preoperative diagnosis was osteoarthritis in 82% and the mean followup time was 4 years. The acetabular component was cementless: modular in 121

Morphology of the knee in adult patients with neglected developmental dysplasia of the hip.

BACKGROUND: Although there have been numerous studies on the treatment of developmental dysplasia of the hip, few have focused on its natural history into adulthood. We observed valgus deformity of the knee in a group of adult patients with developmental dysplasia of the hip. To the best of our knowledge, there has been no detailed study of that condition. The purpose of this study was to investigate structural changes of the knee in patients with neglected developmental dysplasia of the hip. METHODS: Thirty-four adult patients with neglected developmental dysplasia of the hip and ten healthy

Lipid-lowering agents and the risk of hip fracture in a Medicaid population.

CONTEXT: Three recent nested case-control studies conducted in automated databases suggest that users of 3-hydroxy-3-methylglutaryl coenzyme A reductase inhibitors (statins) have a risk of hip and other osteoporotic fractures half that of non-users of any lipid-lowering drug. However, this comparison may be biased by unmeasured factors associated with treated hyperlipidemias. OBJECTIVE: To compare the risk of hip fracture among users of statins and other lipid-lowering agents, which is less susceptible to bias than the comparisons performed in the previous studies. DESIGN AND SETTING: Retros

Mathematical estimation of stress distribution in normal and dysplastic human hips.

By using a mathematical model of the adult human hip in the static one-legged stance position of the body, the forces acting on the hip, peak stress in the hip joint and other relevant radiographic and biomechanical parameters were assessed. The aims were to examine if the peak stress in dysplastic hips is higher than in normal hips and to find out which biomechanical parameters contribute significantly to higher peak stress. The average normalized peak stress in dysplastic hips (7.1 kPa/N) was markedly higher (to approximately 100%) than the average normalized peak stress in normal hips (3.5
